摘要

应用ＨＡＮＳ辅助小剂量颈丛阻滞行甲状腺手术麻醉（Ａ组）、与对照组（Ｂ组）ＨＡＮＳ麻醉各４０例。观察麻醉效果及生理功能变化，Ａ组麻醉优良率为９３％，Ｂ组７５％，两组比较有显著性差异（Ｐ＜０．０５）；Ａ组甲状腺囊腺瘤切除的麻醉优良率为９６％，甲状腺次全切除的麻醉优良率为８６％，但两者比较没有差异性（Ｐ＞０．０５）；Ｂ组甲状腺囊腺瘤切除的麻醉优良率为８６％，甲状腺次全切除的麻醉优良率为５０％，两者比较有显著性差异（Ｐ＜０．０５）。病人手术前后１５分钟心率（ＨＲ）、平均动脉压（ＭＡＰ）、血氧饱和度（ＳｐＯ_２）的变化，Ａ组ＨＲ、ＳｐＯ_２术后较术前降低，有非常显著性差异（Ｐ＜０．０１），ＭＡＰ降低，有显著性差异（Ｐ＜０．０５）；Ｂ组ＨＲ术后稍升高，但无显著性差异（Ｐ＞０．０５），ＭＡＰ降低，有显著性差异（Ｐ＜０．０５），ＳｐＯ_２降低明显，有非常显著性差异（Ｐ＜０．０１）术后Ａ组与Ｂ组的ＨＲ、ＭＡＰ、ＳｐＯ_２两组间比较，有非常显著性差异（Ｐ＜０．０１）。

Abstract

In this paper the clinical efficacies of HANS combined with PCB and HANS alone applicated for anesthesia in thyroid operation were observed in 80 cases. They were divided into two groups

group A (40 cases)

using HANS and PCB; group B(40 cases)

using HANS alone. Results: ①The satisfactory rate of anesthesia of group A was 93%

while group B was 75 %

the difference was significant(P < 0. 05 ). ②The changes of patients' HR

MAP

SpO_2 at 15 minutes before and after operation: In group A

SpO_2 decreased after operation

which were more lower than that of preparation (P < 0. 01 ). MAP was also decreased significantly(p<0. 05). In group B

HR increased somewhat

but the difference was not marked(p >0. 05). MAP decreased

the difference was significant (P< 0. 05). SpoO_a2 decreased was significant (P < 0. 01 ). In group A

MAP and SpO_2 decreased significantly after operation as compared with those in group B(P < 0. 01 ).
