摘要:This paper presents 19 cases of subtotal gastrectomies performed under“acupuncture-drug combined anesthesia”and 20 cases of the same operationwith simple epidural block(as a control)with the mere purpose of studyingthe effect of acupuncture anesthesia.Epidural block was used in both groups. To the cases of the former, needle puncturing and electric stimulation on Zusanli and Shangjuxu pointswere added bilaterally. The primary requisition for the anesthesia was thatthe dosage of 1.6% lidocaine to be injected into epidural space must efficien-tly provide against pain through the operation. The total dose and the meandose per hour of the drug used were calculated.A formula was designed toevaluate the actual efficacy of acupuncture anesthesia(NSI).The formulawas designed as follows.NSI=mX-nX/mX ×100mX represents mean dose milliliter of 1.6% lidocaine used in control groupper hour.nX represents mean dose milliliter of 1.6% lidocaine used for acupuncture-drug combined anesthesia group. The calculated results for mX and nX were as follows:mX=13.0±2.5nX=8.9±2.5(p<0.01)This would mean that if the operation was made under the anesthesia ofboth drug and acupuncture, the dose of the drug would be smaller than that without acupuncture.Statistical analysis indicated that there was significantdifference between them,Using the formula designed above,then31.5%(NSI=13-8.9/13×100)was obtained. The result showed that tak-ing anesthesia effect as a whole there would be 31.5% due to the effect ofthe acupuncture. The E-rosette formation counts were performed in 17 cases with acu-puncture-drug combined anesthesia and 11 cases with simple epidural blockbefore and 30 minutes after the anesthesia,60 minutes after starting the opera-tion and also 24 hours after the operation. The percentage of the E-rosetteof two groups were then compared and the results showed that the percentageof E-rosette formation rate was significantly decreased at 30 minutes afteranesthesia and 60 minutes after starting the operation, with the t-values of2.1116 and 2.7778(p<0.05)respectively.In the simple epidural block group, the t-value of 30 and 60 minutes were 4.5475 and 3.7331(p<0.01)respec-tively.Evidently,the percentage of E-rosette formation rate of simple epi-dural block group decreased more markedly. The results of this study indicated that combined anesthesia with drugand acupuncture may have the following valuable points:Decrease of the dosage of anesthesia drug;Slight depression of immune reaction.Besides, the designed formula may be useful to evaluate the acupunctureeffect in anesthesia.

摘要:The purpose of this study is to observe the relationship between thesensitivity of cutaneous sensation and the effect of acupuncture anesthesia.Subjects were 106 patients for lung resection and 59 for brain resection underacupuncture anesthesia. As the results indicated, there was high correlation between the compo-und sensitivity of electric touch and pain sensations and the effect of acupu-ncture anesthesia, and the nonsensitives often had better effect. It seemsthat this method can be used as one of the ways for forecast of acupunctureanesthesia effect.

摘要:In recent years using cortical evoked potentials(CEP)as an indexmany researchers have studied the role of the cerebral cortex in acupunct-ure analgesia. However, reports on identifying the characteristics of CEPhave been scarcely found. Thus, there are still some contradictory opinionsabout whether or not CEP can be justly used as an index to reflect painfulsensation and whether or not the cerebral cortex is involved in the mechan-ism of acupuncture analgesia. The primary intent of this work is toidentify the relatioship between CEP and pain, and to further explore therole of the cerebral cortex in acupuncture analgesia by analizing the corticalpotentials evoked by noxious stimulation and observing the effect of electro-acupuncture on them. The experiments were carried out on 42 cats. The animals were cons-cious,but immobilized during the process of experiments. The right peronealnerve was electrically stimulated. The compound action potentials of thenerve and the CEP on the contralateral somatosensory area were simul-taneously recorded. Both of them were fed to a computer to be averagedthrough different channels via individual amplifiers. The points “Zusan Zusanli”and “Huantiao” on the right side were stimulated by electroacupuncture. The results were as follows:1.The pat erns of compound action potentials and corresponding corticalpotentials elicited by electrical stimuli with various intensities were differ-ent.The electrical stimulation with the intensities sufficient to activate Aδfibers caused significant increaing the amplitudes of the late components P2and N2(with mean latency of 49.4 ms and 117.5 ms respectively)of CEP.2.In 10 cats following an intravenous injection of ketamine(20mg/kg)the amplitude of each component of the cortical potentials induced by nox-ious stimuli With the intensities strong enough to excite Aδ and C fibers was reduced to various extent. The diminution of the amplitudes of P2 andN2 was of significance.3.Administering morphine(6-8mg/kg)intravenously resulted in remark-able inhibiton of the late components P2 and N2 of cortical potentialselicited by noxious stimulation in 10 cats.In the control experiments withequal volume of saline solution the CEP showed no considerable changes.4.Electroacupuncture at points“zusanli” and “Huantiao”obviously depressed the amplitudes of both late components and early ones of CEP in 11cats.It is established that the late components P2 and N2 of cortical po-tentials evoked by noxious stimulation are closely related to pain, and theCEP is inhibited by electroacupuncture. The present data are consistentwith our previous findings in human subjects. Therefore,it is suggestedthat CEP can be used as one of the nonspecific indexes reflecting painfulsensation. Thus,we further consider that the cerebral cortex does play arole in the mechanism of acupuncture analgesia.

摘要:We applied electro-acupuncture to the subcutis of the point as if it weredone to Huantiao and/or Xin Ming for man on both sides of the intact andthe sympathetic surperior cervical ganglionectomy animals, and hence thepain thresholds elicited were greatly enhanced.During acupuncture analgesia, tissue specimens were taken from the points of subcutis surrounding the ne-edles,and then examined for their innervation of afferent receptor and nervefibers by means of catecholamine fluorescence and cholinesterase(ChE).They are mainly free nerve endings and ChE-positive nerves. After sympa-thetic ganglionectomy, they are still surviving. We think that in the subcutissurrounding needles, the free nerve endings may be the afferent receptor ofacupuncture and the thin unmyelinated nerve fibers(ChE-positive nerves)may be the peripheral path of conducting needling sensation.Needling ofacupuncture points(Xin Ming and Huantiao)had noticeable elevation of localskin tempereture. The terminal branch of ChE-positive nerve seemed to bethe innervation to the resistance vessels and the subcutis near it.This kindof morphological relation is axon reflex which might be one of the causesto elevate the local skin temperature of these points.
