摘要:In the present paper,the author retrospects the historical developmental stages of acupuncture anesthesiaanalgesia from 1)preliminary exploration:the creative application of acupuncture therapy to surgery with lower dosage or without anesthetics began in Shanghai and Xi'an in 1958;2)clinical popularization:the acupuncture anesthesia-analgesia became popularized from 1965,peaked in 1970s' and declined from 1980s';3)mechanism study:from 1970 to 2000,a large quantity of research papers were published to confirm the validity of acupuncture analgesia(AA),and in 1997,NIH Consensus Conference for acupuncture was held,promoting the related research in the world;4)focused research:from 2007 to now,the acupuncture anesthesia-analgesia was listed as one of the key research program in China;5)theoretical summary:in view of the uncertainty of the scientific basis of the meridian system of traditional Chinese medicine,the nerve fibers distributed under the acupoint are now considered to be the indispensable components for AA,and the knowledge of modern neuroscience is competent to explain the underlying mechanism of AA;6)returning to reality:AA has been accepted by the international scientific community,and the"acupuncture-assisted anesthesia",or"compound acupuncture and anesthetics anesthesia"has been used as the first choice for some types of surgical operation.The research on acupuncture anesthesia-analgesia greatly facilitates clinical application of acupuncture therapy in the world,and also promotes the development of acupuncture therapy itself and neuroscience.However,the transformation between theory and practice and their mutual interaction are endless endeavor.Thus,a careful review of the half-centurylong practice of acupuncture anesthesia-analgesia is painstakingly necessary in order to clarifying the mechanisms,to raise the efficacy of clinical practice and to correct the possible errors referred to this concept.

摘要:Objective To observe the effect of electroacupuncture(EA)serum on proliferation of multifidus muscle satellite cells(SCs)and expression of paired box transcription factor Pax-7,MyoD and protein kinase B(PKB or Akt)proteins of SCs,so as to explore its underlying mechanism in promoting repair of multifidus muscles.Methods Thirty-two SD rats were randomly assigned to control,model,EA-Weizhong(BL 40)and EA-Shenshu(BL 23)groups.The multifidus muscle injury(MFMI)model was established by injection of 0.5% bupivacaine hydrochloride(400μL)into the bilateral L4-L5 paravertebral muscles(4points,100μL for each point).EA stimulation was separately applied to bilateral BL 40 and BL 23 for 20min,once daily,4days altogether.Blood samples of the abdominal artery of rats in the above mentioned 4groups were separately collected for extracting serum,followed by deactivation and filtration,and then were respectively applied to the Dulbecco's Modified Eagle Media(DMEM)culturing each multifidus muscle SCs of the normal serum,model serum,EA-BL 40 serum and EA-BL 40serum+LY 294002(an inhibitor of phosphotidylinsitol-3-kinase,PI 3K),EA-BL 23 serum and EA-BL 23serum+LY 294002 groups for analyzing the impact of EA serum on the proliferation state of SCs by Cell Counting Kit-8(CCK-8)and 5-Ethynyl-2'-deoxyuridine(EdU)methods,respectively.The expression of Pax-7,MyoD and phosphorylated(p)-Akt proteins of the cultured SCs was detected for characterization of SCs by Western blot.Results Compared with the normal serum group,the proliferation levels(detected by both CCK-8-and EdU)and the expression levels of MyoD and p-Akt proteins of SCs in the model serum group were significantly increased(P<0.05,P<0.01),while in comparison with the model serum group,the proliferation and expression levels of MyoD and p-Akt proteins of SCs were further significantly increased in both EA-BL 23 and EA-BL 40 serum groups(P<0.01,P<0.05),but not in the EA-BL 40serum+LY 294002 and EA-BL 23serum+LY 294002groups(P>0.05),suggesting an involvement of PI 3Kin the proliferation of SCs.No marked differences were found in the proliferation levels between the EA-BL 23 and EA-BL 40 serum groups and in the expression levels of Pax-7proteins among the 6serum groups(P>0.05).Conclusion Both EA-BL 40 and EA-BL 23 serum can promote proliferation of multifidus muscle SCs,which may contribute to the effect of EA intervention in promoting repair of the injured muscle,partially by way of Akt/PI 3Ksignaling.
摘要:Objective To observe the effect of penetrative needling of"Baihui"(GV 20)to"Qubin"(GB 7)on neurologic functions and expression of tumor necrosis factor(TNF)-α,interleukin(IL)-6and toll-like receptor 4(TLR-4,involving in inflammatory reactions)in the tissue around the local cerebral hematoma in rats with intracerebral hemorrhage(ICH),so as to provide evidence for clinical treatment of ICH.Methods Fifty-four male Wistar rats were randomly divided into sham control,model and acupuncture groups,and then further divided into three time-point subgroups(1,3,7days after modeling,n=6/subgroup).The ICH model was established by injection of the rat's autoblood(50μL)into the putaman region(P:0.2mm,R:3.5mm)in a stereotaxic apparatus and confirmed by Berderson's neurologic examination grading system(0-3points).The neurologic function was assessed by using Longa's scoring(5-points)and footfault asymmetry testing[footfault index=(contra faultsipsi faults)/total steps in 2min].For penetrative needling,an acupuncture needle was inserted into GV 20 and controlled to advance to GB 7on the affected side and retained for 30 min,once daily.The expression of TNF-α,IL-6and TLR-4in the cerebral tissue around the putaman was detected by immunohistochemistry.Results After penetrative needling stimulation,the increased Longa's score and footfault asymmetry score in ICH rats were significantly decreased on day 1,3 and 7 after modeling(P<0.01),suggesting an improvement of neurologic function after the treatment.Immunohistochemical staining outcomes of the cerebral tissue surrounding the autoblood injection site showed that the expression levels of TNF-α,IL-6and TLR-4proteins on day1,3and 7were considerably higher in the model group than in the control group(P<0.01),and markedly lower in the acupuncture group than in the model group(P<0.01),suggesting a suppression of the proinflammatory factors and TLR-4levels around the locus of the brain after needling intervention.A positive correlation existed between the expression levels of TLR-4and IL-6/TNF-α.Conclusion Penetrative needling stimulation of GV 20 to GB 7can reduce the levels of proinflammatory factors TNF-αand IL-6,and TLR-4in the ICH tissues in rats with cerebral hemorrhage,which may contribute to its effect in improving neurological function.

摘要:Objective To observe the influence of scalp-acupuncture on the expression of acid-sensing ion channels(ASICs)1aand 2bof hippocampal CA 1region in cerebral ischemia(CI)rats,so as to investigate its mechanism underlying improvement of ischemic stroke.Methods Thirty-two male SD rats were randomly allocated to normal control,model,scalp-acupuncture and Amiloride group(n=8in each group).The model of focal CI was established by middle cerebral artery occlusion(MCAO).Scalp acupuncture stimulation was applied to bilateral Dingnieqianxiexian(MS 6)and Dingniehouxiexian(MS 7),once daily for 7days.Rats of the Amiloride group were fed with Amiloride solution,twice a day for 7days,and those of the normal control and model groups were grabbled and fixed in the same way with the acupuncture and Amiloride groups.The neurological deficit score was given according to Longa's method.The expression of hippocampal ASIC 1aand ASIC 2bwas detected by immunohistochemistry,and the Ca2+concentration in the hippocampal tissue assayed using flowing cytometry.Results After the intervention,the neurological deficit score of both the scalp-acupuncture and Amiloride groups were significantly decreased in comparison with pre-treatment(P<0.01),and the effect of scalp-acupuncture was markedly superior to that of Amiloride in reducing neurological deficit score(P<0.05).The expression of ASIC 1aand ASIC 2bin the hippocampal CA 1region and hippocampal Ca2+concentration were significantly up-regulated in the model group compared with the normal control group(P<0.01),and obviously down-regulated in both scalp-acupuncture and Amiloride groups(P<0.01,P<0.05),without significant differences between the two treatment groups in the ASIC 1aand ASIC 2bexpression and Ca2+concentration(P>0.05).Conclusion Scalp-acupuncture stimulation can improve neurological function in CI rats,which may be related to its effects in suppressing the increased expression of hippocampal ASIC 1aand ASIC 2bproteins and in reducing calcium overload in hippocampal neurocytes.

摘要:Objective To observe the influence of electroacupuncture stimulation(EA)of"Neiguan"(PC 6)/"Lieque"(LU 7)on ST segment of electrocardiogram(ECG)and the expression of myocardiac chloride channel(CLC)-2and calcium(Ca2+)-activated chloride channel accessory(CLCA)proteins in acute myocardial ischemia(AMI)mice,so as to explore its mechanisms underlying improvement of AMI.Methods Thirty ASIC 3-/-knock out mice were randomly divided into control,AMI model,EA-Neiguan(PC 6),EA-Lieque(LU 7)and EA-non-acupoint groups.The AMI model was induced by multiple subcutaneous injection of isoprenaline(ISO,0.5g/L,20mg/kg).EA was applied to bilateral PC 6,LU 7,or non-acupoint[the midpoint between"Tianshu"(ST 25)and "Shenque"(CV 8)]for 20 min,once daily for 7days.The ST segment of ECG of the standardⅡlimb-lead was recorded using a PowerLab data acquisition device,the change of myocardial histology observed by using microscope after HE staining,the activity of serum superoxide dismutase(SOD)detected using Colorimetric method,and the expression of CLC-2and CLCA proteins of the left ventricle myocardium detected by Western blot.Results Outcomes of HE staining showed that the ischemic injury(sarcoplasm swelling and necrosis,etc.)of the left ventrical myocardial tissue after modeling was relatively milder in the EA-PC 6group,not in the EA-non-acupoint group.The SOD activity was significantly lower in the model group than in the control group(P<0.01),and obviously increased in the EA-PC 6group(P<0.01),but not in the EA-non-acupoint group(P>0.05).The expression levels of myocardial CLC-2and CLCA proteins were significantly up-regulated in the model group compared with the control group(P<0.01)and markedly down-regulated in the EA-PC 6group(not in the EAnon-acupoint group)in comparison with the model group(P<0.01),suggesting a specificity of effects of EA-PC 6in improving myocardial injury and down-regulating CLC-2and CLCA protein expression.Conclusion EA of"Neiguan"(PC 6)can effectively suppress the increase of expression of CLC-2and CLCA proteins in the left ventricle myocardium,which may contribute to its effect in relieving myocardial injury in mice.
摘要:Objective To observe the effect of moxibustion stimulation of"Feishu"(BL 13)and"Xinshu"(BL 15)on pathological changes of myocardium and the expression of myocardial myeloid differentiation factor 88(MyD 88)protein and Caspase 3mRNA in chronic heart failure(CHF)rats,so as to explore its mechanism underlying improvement of CHF.Methods SD male rats were randomly divided into normal(n=9),model(n=8),moxibustion(n=8),medication(n=8)and moxibustion+ medication(n=8)groups.In addition,the other 6rats(3/normal and 3/model groups)were used for measuring cardiac ventricle weight and H.E.stain.The CHF model was made by intraperitoneal injection of Adriamycin(ADR,from 1to 4mg/kg,once every other day for 15days).Mild moxibustion was applied to bilateral BL13 and BL15for 15 min,once daily for 3weeks.Rats of the medication group were treated by Captopril(gavage)for 3weeks.The expression of myocardial Caspase 3mRNA and MyD 88 protein of the left ventricle was determined by quantitative real time-PCR and Western blot,respectively.Results In comparison with the normal group,the myocardial damage(cell swelling,cytoplasma vaculation,and disordered arrangement,rupture and lysis of some cardiac muscle fibers),and the expression levels of myocardial MyD 88 protein and Caspase 3mRNA were obviously increased in the model group(P<0.01).After the interventions,the myocardial damage was relatively milder,and the expression of myocardial MyD 88 protein and Caspase 3mRNA were significantly down-regulated in the moxibustion,medication and moxibustion+medication groups in comparison with the model group(P<0.05).No significant differences were found among the 3treatment groups in the expression levels of MyD 88 protein and Caspase 3mRNA(P>0.05).Conclusion Moxibustion intervention can suppress CHF induced up-regulation of expression of myocardial MyD 88 protein and Caspase 3mRNA in rats,which may contribute to its effect in relieving myocardial injury.
摘要:Objective To observe the effect of electroacupuncture(EA)intervention combined with clomiphene critate(CC)on the blastocyst implantation and pregnancy rate and expression of insulin receptor(INSR)and insulin receptor substrate 1(IRS 1)proteins in the endometrium in rats with polycystic ovary syndrome(PCOS),so as to reveal its mechanisms underlying improvement of PCOS.Methods One hundred and twenty-five female SD rats were randomly divided into normal control,PCOS model,medication(CC),EA and EA+CC groups(n=25in each group,15 for checking blastocyst implantation,and 10 for Western blot).The PCOS model was established by subcutaneous injection of Dehydroepiandrosterone(DHEA)and fed with high-fat diet.Rats of the normal control group were treated by subcutaneous injection of sesame oil and fed with the normal forage.EA stimulation was applied to"Zhongwan"(CV 12),"Guanyuan"(CV 4)and bilateral"Tianshu"(ST 25)for 30 min,3times a week,5weeks altogether.Rats of the CC and EA+CC groups were fed with CC(100mg·kg-1·d-1)for 2days after regular restriction(30min,3times a week,5weeks altogether).The pregnancy was determined by vaginal smear tests and the number of blastocyst implantation determined by examination of the uterus after execution.The expression of INSR and IRS 1proteins in the endometrium was detected by Western blot.Results The pregnancy rate and the number of blastocyst implantation were significantly lower in the model group than in the normal control group(P<0.05),and remarkably increased after EA and EA+CC interventions(P<0.05).The effects of EA+CC were obviously superior to those of simple EA and simple medication in raising the pregnancy rate and the number of blastocyst implantation(P<0.05).No significant differences were found between the EA and CC groups in the pregnancy rate and the number of blastocyst implantation(P>0.05).The relative expression levels of both INSR and IRS 1proteins were markedly lower in the model group than in the normal control group(P<0.05),and significantly higher in both EA and EA+CC groups than in the model group(P<0.05).No significant differences were found between the model and the CC groups and between the EA and EA+CC groups in the expression levels of INSR and IRS 1 proteins(P>0.05).Conclusion EA intervention can improve pregnancy rate and the number of blastocyst implantation in PCOS rats,which may be related to its effects in up-regulating the expression of INSR and IRS 1proteins in the endometrium.
摘要:Objective To compare the effects between electroacupuncture(EA)plus western medicine and simple western medicine in improving clinical symptoms and local joint function of rheumatoid arthritis(RA)patients with yin deficiency of Liver and Kidney.Methods A total of 68 RA patients of yin deficiency of Liver and Kidney were equally randomized into EA+medication group and medication group(n=34in each group).Both groups were given once-a-week methotrexate(7.5mg/time)and once-a-day leflunomide(10mg/time),while EA+medication group was additionally treated by EA at bilateral Ganshu(BL 18),Shenshu(BL 23),Xuanzhong(GB 39),Zusanli(ST 36),Taichong(LR 3),Hegu(LI 4)3times/week.The treatment lasted for 12 weeks.The visual analogue scale(VAS,for assessing rest pain),swollen joint count(SJC),tender joint count(TJC),patient's global assessment(PGA),physician's global assessment(PhGA),traditional Chinese medicine(TCM)symptom scoring,28 joints activity index(disease activity score,DAS 28),American College of Rheumatology 20(ACR 20,i.e.20% of clinical improving rate),and health assessment questionnaire(HAQ)were assessed and erythrocyte sedimentation rate(ESR),serum C-reactive protein(CRP)levels were examined for comparison.Results Statistical differences were observed in before-after-treatment comparisons in both groups in reducing rest pain,SJC,TJC,serum CRP content,PGA and PhGA,HAQ scoring and DAS 28(P<0.05,P<0.01).The ESR in the medication group and TCM symptom scoring of the EA+medication group were also significantly decreased after the treatment(P<0.05).The effects of the EA+medication group were superior to those of the medication group in reducing rest pain,SJC,TJC,TCM symptom and HAQ score,and the total effetive rate(P<0.05,90.9% vs 66.67%).Adverse reactions as pharyngeal obstruction sensation,anorexia,abdominal distension,etc.can be reduced by EA therapy coordinated with western medicine.Conclusion EA is effective in relieving symptom and joint function in RA patients with yin deficiency of Liver and Kidney.
摘要:Objective To observe the efficacy of acupuncture stimulation of the sciatic nerve trunk in the treatment of patients suffering from sciatica induced by lumbar disc herniation(LDH).Methods A total of 60 LDH sciatica patients met the inclusion criteria were randomly divided into treatment group and control group,with 30 cases in each.Patients of the treatment group were treated by directly needling the sciatic nerve and routine acupuncture of Ashi-points,Lumbar Jiaji(EX-B 2),Dachangshu(BL 28),etc.,and those of the control group treated by simple routine acupuncture.The treatment was conducted once a day,5times a week,4weeks altogether.The clinical effect was evaluated according to the"Standards for Diagnosis and Therapeutic Effect Evaluation of Syndromes of Chinese Medicine"and the pain intensity was assessed by using simplified Short-Form McGill Pain Questionnaire(SF-MPQ)containing pain rating index(PRI),visual analogue scale(VAS)and present pain intensity(PPI).Results After the treatment,of the two 30 cases of LDH sciatica patients in the control and treatment groups,11 and 18were cured,7and 7experienced marked improvement,10 and 4were effective,2and 1was invalid,with the effective rate being 93.3% and 96.7%,respectively.The cured+markedly effective rate of the treatment group was significantly higher than that of the control group(P<0.05,83.3% vs 60.0%).Compared with pre-treatment,the scores of PRI,VAS and PPI were evidently lowered in both groups(P<0.01),and the effect of the treatment group was notably better than that of the control group(P<0.01).Conclusion Acupuncture stimulation of the sciatic nerve trunk is effective in relieving sciatica in LDH patients,and is superior to simple routine acupuncture in the clinical efficacy.
摘要:Objective To evaluate the efficacy of electroacupuncture(EA)therapy on abdominal fat in obese women by using magnetic resonance imaging(MRI).Methods Thirty abdominal obesity women patients were randomly divided into control group(n=15)and EA group(n=15).The obesity patients of the control group did not receive any treatment for weight reduction,and those of the EA group were treated by EA stimulation of bilateral Neiting(ST 44),Fenglong(ST 40),Zusanli(ST 36),Huaroumen(ST 24),Tianshu(ST 25),Wailing(ST 26),Shuidao(ST 28),Fujie(SP 14),Daheng(SP 13),etc.for 25 min,once every other day,3times per week for 3months.The patient's body weight,height,waist circumference(WC)were measured with different devices,and the body mass index(BMI)was calculated,and the subcutaneous adipose tissue thickness at the inferior edges of L4,L5 and S3and the superior edge of the pubic symphysis and the total abdominal fat volume between the L4 and S3levels were detected using MRI systems before and after the treatment.Results The effects of the EA group were significantly superior to those of the control group in lowering difference values(between pre-and post-treatment)of BMI,WC and subcutaneous adipose tissue thickness at the levels of the inferior edges of L4,L5,S3 and the superior edge of the pubic symphysis(all P<0.01)and in reducing total abdominal fat volume between L4 and S3(all P<0.01).After the treatment,the subcutaneous adipose tissue thickness at the superior edge of the pubic symphysis(P<0.01)and the total abdominal fat volume between L4 and S3(P<0.05)were significantly decreased in the EA group compared to pre-treatment.There were no significant differences between post-and pre-treatment in BMI,WC,subcutaneous adipose tissue thickness at the levels of the L4,L5 and S3in both EA and control groups and in the subcutaneous adipose tissue thickness at the level of the superior edge of the pubic symphysis and the total abdominal fat volume between L4 and S3in the control group(P>0.05).Conclusion EA intervention can effectively reduce abdominal fat in obese women based on the evaluation of MRI.
摘要:Objective To evaluate the clinical efficacy of electroacupuncture(EA)stimulation of Zusanli(ST 36)and Neiguan(PC 6)in the treatment of persistent postoperative gastrointestinal dysfunction in patients undergoing laparotomy.Methods A total of 60 laparotomy patients were enlisted in the present study and were randomly divided into conventional treatment group(control)and EA group(n=30in each group).Patients of the EA group were treated by conventional treatment and EA stimulation(2Hz,1-10mA)of bilateral ST 36 and PC 6for 30 min,once daily for 5days,and patients of the control group treated by conventional treatment(pre-and post-surgical fasting,measures for gastrointestinal decompression,electrolyte stabilization,parenteral nutrition support,and anti-infection in necessity,etc).The first postoperative flatus and defecation time,postoperative hospital stays,abdominal bloating grading(0-4points,i.e.Likert scale),and adverse events were recorded and evaluated.Results After the treatment,the first postoperative flatus and defecation time,and hospital stay time and abdominal bloating scores from the 2nd to the 5th day post-surgery in the EA group were significantly lower than those of the control group(P<0.05).The abdominal bloating scores of the two groups were gradually and obviously decreased from the 2nd day on in comparison with pre-treatment(P<0.05).Of the two 30 cases in the control and EA groups,5and 14 were cured in their clinical symptoms,15 and 13experienced marked improvement,7and 3were effective,2and 0invalid,with the effective rates being66.7% and 90.0%,respectively.Conclusion EA is effective in improving persistent postoperative gastrointestinal dysfunction in cancer patients undergoing laparotomy.
摘要:Objective To evaluate the clinical effectiveness of acupuncture in the treatment of spastic cerebral palsy(CP)children by stimulating acupoints selected according to the day to match the heavenly stems and earthly branches(Ziwu Liuzhu Najia Zhenfa).Methods A total of 130 CP patients were randomized into a control group and a treatment group(n=65in each).Both groups were given with routine rehabilitation therapy,while the treatment group additionally received 3months' acupuncture intervention.All patients were assessed before and after treatment with gross motor function measure(GMFM-88),modified Ashworth scale(MAS)and activities of daily living(ADL)in cerebral palsy.Results After the treatment,the control group scored(25.12±2.74)points in GMFM-88,(1.53±0.71)points in MAS,(19.25±5.08)points in ADL,while the treatment group were respectively(31.87±2.58)points in GMFM-88,(1.07±0.74)points in MAS,(25.67±6.15)points in ADL.Both groups experienced significant improvement in increasing GMFM-88 and ADL scores,and reducing MAS score compared to those before treatment(P<0.05,P<0.01),and the effects of the treatment group were evidently superior to those of the control group in increasing GMFM-88 and ADL scores,and reducing MAS score(P<0.05).Conclusion The "Ziwu Liuzhu Najia Zhenfa"acupuncture achieved greater improvement in gross motor function and reduction in muscular tone and enhancement of capacity of daily living activity.

摘要:In the present article,the authors introduce a newly-made rat holder device for easily inserting acupuncture needles into the acupoints at any parts of the body.This device is easy in operation and higher in applicability,being worthy of popularization for researchers engaging in experimental studies.
摘要:Cytokines are closely related to the occurrence and development of rheumatoid arthritis(RA).Acupuncture exerts anti-inflammatory and analgesic effects via elevating anti-inflammatory cytokine level and reducing pro-inflammatory cytokine level and regulating Th 1/Th 2balance,which reflects the dual directional regulatory effect of acupuncture.However,problems exist in the current researches,such as inadequate depth,limited breadth and not so up-to-date research focus.The mediocre research level and the phenomena of some contradictory results among studies also need being concerned.Therefore,progress may be attained if a holistic,dynamic and networked study is conducted on the influence of acupuncture on cytokines by combining the latest progress in the mechanisms of network of cytokines and neuroendocrine immune network,and high throughput cytokine capture assay.In this way,the mechanism of cytokines in multi-targets(such as local joints,peripheral blood circulation,central nervous system)involving the effects of acupuncture in improving RA will be known more.
摘要:Using functional magnetic resonance imaging(fMRI)to investigate the underlying mechanisms of acupuncture therapy attracts an increasing attention nowadays.In the present article,the authors make a brief analysis on the progresses of researches about the intra-brain responses of acupuncture stimulation following collecting original research papers with key words of"acupuncture","fMRI"and "magnetic resonance imaging"in PubMed and CNKI databases from January 2003 to December2014.As a result,a total of 253 original research articles were collected.We found that:1)in the past 10 years,the amount of fMRI studies on acupuncture is growing;2)most of these studies(183)are conducted in healthy subjects,and fewer studies(44)conducted in patients with problems of neurology,psychonosology,gastroenterology,circulation system,pediatrics,myopia and dermatology,or with pain;3)a larger population of studies are designed to reveal the specificity of responses of acupuncture point stimulation,and fewer focus on analyzing the regularities of acupuncture points combination.The authors suggest that the coming acupuncture-neuroimaging studies need to expand the research ranges or contents,and improve experimental designs,data analysis and quality control(for example,reducing differences of baseline data,age span,psychological state,etc.,and increasing sample size,etc).
