      摘要:Acupuncture anesthesia(AA)is an anesthetic method created by medical workers and researchers in China during medical practice in 1950 s.As a paragon of integrated traditional Chinese and Western medicine,AA clinical practice and basic theoretic research have gained continuous improvement and progress,and obtained a series of outstanding achievements in the past 60 years.Consequently,the theory of traditional Chinese medicine was enriched,and the acupuncture therapy was pushed forward to the world.The present paper reviews the course of formation and development of clinical application of AA from the late years of 1950 sup to now,including the initial stage(1958—1966),rapid propulsion stage(1967—1979),consolidation period(1980—2004)and newly developing period(2005—now).The AA has experienced flexural pathway from simple acupuncture analgesia,to acupuncture-drug compound anesthesia(or acupuncture-assisted anesthesia),and to the application of whole perioperative period.Nowadays,it keeps unceasing development and innovation from simple analgesia and sedation to organ protection during perioperative period and reduction of post-operative complications.The AA possesses unique prosperous vitality,and also has a plenty room for improvement,being worthy of further popularization and research.

      摘要:Objective To explore the effect of transcutaneous electrical acupoint stimulation(TEAS)combined with transversus abdominis plane(TAP)block in the enhanced recovery after surgery(ERAS)in patients undergoing laparoscopic colorectal cancer resection(LCCR).Methods A total of 101 patients undergoing LCCR were randomly divided into three groups:control(n=34),TAP(n=35)and TEAS+TAP(n=32).Conventional perioperative anesthesia management of the 3 groups was performed according to the ERAS guidelines.All the patients experienced patient controlled epidural analgesia(PCEA),and those of the TAP and TEAS+TAP groups received TAP block by injection of 0.3% Roperca hydrochloride(15 mL)into the space between the internal oblique and the transverse abdominis after induction of anesthesia.For patients of the TEAS+TAP group,TEAS(2 Hz/10 Hz,an endurable stimulation strength)was applied to bilateral Zusanli(ST 36)from 30 min before anesthesia to the end of the surgery.The blood glucose value and dosage of Remifentanil used were recorded.The pain severity was assessed at 4,12,24 and 48 hafter surgery by using visual analogue scale(VAS).Moreover,postoperative anal exhaust time,postoperative oral feeding time,postoperative first ambulation time and postoperative hospital stay length were recorded.Results The total dosages of Remifentanil used during surgery,and the blood glucose levels were significantly lower in the TAP and TEAS+TAP groups than in the control group(P<0.05),but had no significant differences between the TAP and TEAS+TAP groups(P>0.05).The VAS scores of the TAP and TEAS+TAP groups were considerably lower than those of the control group at 4,12,24 and 48 hafter surgery(P<0.05,except 48 hof TAP group).Of the 34,35 and 32 cases in the control,TAP and TEAS+TAP groups,5(14.7%),3(8.6%)and 1(3.1%)on the 1 st day post-surgery,and 2(5.9%),0(0)and 0(0)on the 2 nd day after surgery experienced nausea and vomiting.The postoperative anal exhaust time and postoperative oral feeding time were significantly earlier in both TAP and TEAS+TAP groups than in the control group(P<0.05),and the exhaust time of the TEAS+TAP group was even earlier than that of the TAP group(P<0.05).No significant differences were found among the 3 groups in the postoperative ambulation time and postoperative hospitalization time(P>0.05).Conclusion TEAS combined with TAP block analgesia is superior to simple TAP block analgesia in relieving postoperative pain,shortening the recovery time of gastrointestinal function and promoting postoperative rehabilitation in patients undergoing LCCR.

      摘要:Objective To observe the analgesic effect and safety of electroacupuncture(EA)intervention for patients undergoing total knee arthroplasty(TKA).Methods A total of 40 patients undergoing TKA were randomly assigned to control group(simple multi-mode analgesia,n=20)and EA group(EA combined with multi-mode analgesia,n=20).Both groups were treated with epidural anesthesia during surgical operation,and conventional epidural automatic analgesia and oral Celecoxib after surgery.Following surgery,EA was applied to Liangqiu(ST 34)-Xuehai(SP 10),Yinlingquan(SP 9)-Zusanli(ST 36),Fenglong(ST 40)-Qiuxu(GB 40)on the operation side for 30 min,once daily for 7 consecutive days.The patients' pain state during rest and motion was assessed by using visual analogue scale(VAS).The active and passive knee flexion range of motion(ROM),use of painkillers including the number of patient's controlled epidural analgesia(PCEA)during 48 hafter surgery,and other complications were recorded.Results After the treatment,the VAS scores under rest and motion state were both significantly lower in the EA group than in the control group on day 3,5 and 7 after surgery(P<0.05).During 48 hafter surgery,the number of PECA was significantly lower in the EA group than in the control group(P<0.05).Of the two 20 cases in the control and EA groups,3 and 1 asked to receive muscular injection of Tramadol Hydrochloride for pain relief,3 and 2 experienced nausea-vomiting,2 and 1 had dizziness and headache,and 2 and 1 had a chest distress feeling,respectively,which had no significant differences between the two groups(P>0.05).The white blood cell(WBC)count in both groups were decreased gradually from day 1 to 7 after surgery,and plasma C-reactive protein content on day 5 and 7 were also lowered in both groups,without statistical differences between the two groups in the post-operative complications,dosages of additional postoperative analgesic drugs,and levels of plasma WBC and C-reactive protein(P>0.05).Conclusion EA can effectively improve the early postoperative pain of TKA,reduce the incidence of postoperative complications and the use of analgesic drugs in TKA patients.

      摘要:Objective To observe the effect of manual acupuncture intervention on the reproductive endocrine and p-Akt protein expression in polycystic ovary syndrome with insulin resistance(PCOS-IR)rats,so as to reveal its mechanism in relieving PCOS-IR.Methods Female Wistar rats were randomly divided into control,model,medication(Metformin)and acupuncture groups(n=8 in each).The PCOS-IR model was established by subcutaneous injection of Dehydroepiandrosterone(DHEA,6 mg/100 g),once daily for 20 days.Rats of the medication group were treated by gavage of Metformin solution(100 mg/kg),once daily for 28 days.For rats of the acupuncture group,manual acupuncture stimulation(lifting-thrusting and rotating of needles)was applied to the bilateral"Shenshu"(BL 23),"Yishu"(EX-B 3),"Zigong"(EX-CA 1),"Sanyinjiao"(SP 6),"Fenglong"(ST 40)and"Guanyuan"(CV 4)for 5 min,once a day for 28 days.H.E.staining was used to observe the histopathologic changes of the ovary tissue.Serum levels of follicle-stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH),testosterone(T)and fasting insulin(FINS)were measured by ELISA.Fasting plasma glucose(FPG)was measured by blood glucose meter,and homeostasis model assessment-insulin resistance index(HOMA-IR)was calculated.Western blot was used to detect the expression of Akt and p-Akt proteins in ovarian tissues.Results Following modeling,the ovaries showed polycystic changes and an increase of atresia follicles with a larger diameter and no corpus luteum in the model group.After the intervention,there were various types of follicles and a small amount of corpus luteum at different stages of development in both acupuncture and medication groups.In comparison with the control group,serum FSH and E2 levels as well as ovarian Akt and p-Akt protein expression were considerably decreased(P<0.05,P<0.01),and serum LH,T and FINS contents and HOMA-IR were significantly increased in the model group(P<0.01).Following the intervention,the decreased serum FSH and E2 levels as well as ovarian Akt and p-Akt protein expression,and the increased serum LH,T and FINS contents and HOMA-IR were remarkably reversed in both acupuncture and medication groups(P<0.05,P<0.01),without significant differences between the two groups in the levels of those serum and ovary indexes(P>0.05).Conclusion Acupuncture intervention can regulate the levels of reproductive endocrine hormones and reduce insulin resistance in PCOS-IR rats,which may be related to its effect in up-regulating the expression of p-Akt protein in the ovaries.

      摘要:Objective To investigate the effect of electroacupuncture(EA)on IL-6 and vimentin protein expression in white adipose tissue(WAT)of diet-induced obesity(DIO)rats,so as to reveal its mechanism underlying losing weight.Methods Thirty-four Wistar rats were randomly divided into normal(control,n=10),model,sham-EA and EA groups(n=8 in each of the latter 3 groups).The obesity model was established by feeding the rats with high fat diet.EA(2 Hz/15 Hz,1 mA)was applied to bilateral"Zusanli"(ST 36)and"Tianshu"(ST 25)for 30 min,5 times per week for a total of 8 weeks.For sham-EA group,two sham points(about 5 mm lateral to ST 36 and to ST 25)were only punctured with filiform needles but without electrical stimulation.During EA treatment,all rats were fed with normal fodder,and their body weight were measured once a week.Histopathologic changes(diameters of adipose cells)of abdominal WAT were observed under microscope after sectioning and H.E.staining,and the expression levels of IL-6 and vimentin in the WAT were detected by Western blot.Results Compared with the control group,the body weight,diameter of fat cells and the expression levels of IL-6 and vimentin in the WAT were significantly increased in the model group(P<0.05).Following EA,the body weight,diameter of fat cells and the expression levels of IL-6 and vimentin proteins were considerably down-regulated in the EA group(P<0.05),rather than in the sham-EA group relevant to the model group(P>0.05).Conclusion EA intervention can effectively down-regulate the expression of IL-6 and vimentin in WAT of DIO rats,which may contribute to its action in reducing body fat by relieving chronic inflammation.

      摘要:Objective To observe the effect of electroacupuncture(EA)on the rhythm of running-wheel activity of hepatocellular carcinoma(HCC)mice and the expression of Per 1 and Per 2(circadian rhythm genes)in the hypothalamic suprachiasmatic nucleus(SCN),so as to investigate its mechanism underlying regulating circadian rhythm.Methods A total of 108 male C 57 BL/6 Jmice were randomly divided into control,HCC model and EA groups which were further assigned to six zeitbeger(environmental light-dark cycle)time(ZT)point(ZT 0,ZT 4,ZT 8,ZT 12,ZT 16 and ZT 20)subgroups.The HCC model was established by injection of H 22 cancer cell(abdominal 3 rd generation,10!L)suspension into the larger live lobe.Mice of the control group received saline injection of the liver lobe.EA(2 Hz/15 Hz,0.2 mA)was applied to bilateral"Ganshu"(BL 18)and"Zhiyang"(GV 9)for 15 min,once daily for 10 days.Mice of the control and model groups received the same binding-fixing to those of the EA group.Circadian running-wheel activity of 12 h∶12 hlight darkness(LD)cycle(activity onset and acrophase of actogram,amplitude or peak of periodogram)was recorded by using ClockLab(ACT-500)software and analyzed by MATLAB(R2007 b)before and after EA treatment.The pathological changes of liver cells were observed under light microscope after sectioning and H.E.staining.The expression levels of Per 1 mRNA and Per 2 mRNA in the liver tissues were determined by fluorogenic quantitative real time-PCR.Results(1)Following modeling,the amplitude of periodogram of running-wheel activity was significantly lowered at ZT 0,ZT 4,ZT 8,ZT 12,ZT 16,and ZT 20 relevant to the control group(P<0.05).After EA intervention,the amplitude of periodogram at ZT 8(15:00)was considerably increased relevant to the model group(P<0.05),and the acrophase at ZT 8 was remarkably advanced(P<0.05).No significant changes were found in the onset time and periods of periodogram at the 6 time-points after modeling and EA intervention(P>0.05).(2)The expression levels of Per 1 mRNA and Per 2 mRNA in the SCN were significantly up-regulated at the 6 time-points in the model group relevant to the control group(P<0.05),and obviously down-regulated at ZT 8 after EA intervention relevant to the model group(P<0.05).Conclusion EA can benignly regulate the rhythm of running-wheel activity of HCC mice,which may be closely related to its effect in down-regulating the expression of circadian rhythm genes Per 1 and Per 2 in the SCN.

      摘要:Objective To observe the effect of electroacupuncture(EA)intervention on hyperplastic mammary glands,serum estradiol(E2)and progesterone(P)contents,estrogen receptor alpha(ERα)and progesterone receptor(PR)expression of breast tissues in mammary gland hyperplasia(MGH)rats,so as to reveal its mechanisms underlying improvement of MGH.Methods Sixty female SD rats were randomly divided into control,model,EA,EA+ovariectomy(OVX)and EA+sham-OVX groups(n=12 in each).The MGH model was established by injection of Estradiol Benzoate injection(0.5 mg/kg,once daily for25 d)and P injection(0.5 mg/kg,once daily for 5 dafter estradiol injection)into the medial hind-leg muscle.After model establishment,bilateral OVX was performed for rats of the EA+OVX group,and sham OVX(only exposure of ovaries)was performed for rats of the EA+sham OVX group.EA(2 Hz,2 mA)was applied to bilateral"Tianzong"(SI 11),"Ganshu"(BL 18),"Zusanli"(ST 36)for point group A,and"Wuyi"(ST 15),"Hegu"(LI 4),"Danzhong"(CV 17)for point group B for 20 min,once daily,5 days a week for 4 weeks.The two acupoint groups were used alternately.The height of the rats' nipples(the 2 nd pairs)were measured.Serum E2 and P levels were assayed by ELISA.The expression of ERαand PR in mammary tissues was detected by immunofluorescence staining and Western blot,separately.Results The height of nipples,serum E2 content,and mammary ERαprotein expression level were significantly higher in the model group than in the control group(P<0.01),while serum P content and PR expression were remarkably lower in the model group than in the control group(P<0.01).Following EA intervention,the height of nipples,serum E2 content and mammary ERαprotein expression level in both EA and EA+sham OVX groups,and ERαexpression in the EA+OVX group were considerably lower in comparison with those of the model group(P<0.01),and serum P content and PR expression in the 3 EA groups were significantly higher than those in the model group(P<0.01,P<0.05).But there were no significant changes in the nipple height and serum E2 levels in the EA+OVX group relevant to the model group(P>0.05),suggesting an important role of the ovary in producing EA's effect.Conclusion EA intervention can improve hyperplastic mammary glands by down-regulating serum E2 content and mammary ERαprotein expression,and up-regulating serum P content and PR expression in MGH rats,which has a close relation with the intact ovaries.

      摘要:Objective To observe the effect of acupuncture combined with medication(Rule Granule)on serum prolactin(PRL),estradiol(E2)and progestone(P)contents and expressions of prolactin receptor(PRLR),estrogen receptor(ER)and progestrone receptor(PR)proteins in mammary gland(MG)tissues of MG hyperplasia(MGH)rats,so as to explore its mechanisms underlying improvement of MGH.Methods A total of 55 female SD rats were randomized into blank control group(n=10),model group(n=10),acupuncture group(n=11),medication group(n=10),and acupuncture plus medication group(n=9).The MGH model was established by muscular injection of E2 benzoate(0.5 mg/kg,once daily for 20 days),followed by injection of P(5 mg/kg)into the lateral muscle of the hind-limb,once daily for 5 days,and the rats of the control group were treated by muscular injection of normal saline at the same site and in the same procedures.Acupoint group A composed of bilateral Tianzhong(SI 11),Ganshu(BL 18)and Zusanli(ST 36),and group B composed of Tanzhong(CV 17),and bilateral Wuyi(ST 15)and Hegu(LI 4)were alternatively punctured with filiform needles and stimulated swiftly by twirling the needle in each acupoint for 20-30 sec,once daily for 30 days.Rats of the medication group and acupuncture +medication group were treated by gavage of Rule Granule fluid containing Baishao(Paeonia Iactiflora Pall),Danggui(Angelica sinensis),etc.(1.5 mL/100 g for each rat)and those of the other groups treated by gavage of distilled water(1.5 mL/100 g).The treatment was given once daily for continuous 30 d.At the end of the experiments,the rats' abdominal aorta blood was collected for assaying the contents of serum PRL,E2 and P with ELISA,and the pathological changes of breast tissue(the left 2 nd pairs)were observed under microscope after sectioning and H.E.staining.The expression of PRLR,ER and PR proteins in the breast tissue was detected by immunohistochemistry.Results After modeling,the hyperplasia of mammary gland in the model group was obvious;the serum PRL and E2 contents and the immunoactivities of PRLR,ER and PR were significantly increased(P<0.01);and the content of serum P was considerably decreased in the model group in comparison with the blank control group(P <0.01).After the treatment,the hyperplasia of mammary gland was improved;the serum PRL and E2 contents and the expression of PRLR,ER and PR were significantly decreased(P<0.05,P<0.01);and the content of P was notably increased in the acupuncture,medication and acupuncture plus medication groups relevant to the model group(P<0.05,P<0.01).The therapeutic effects of acupuncture plus medication were significantly superior to those of both simple medication and simple acupuncture in down-regulating serum PRL and E2 contents and PRLR,ER and PR immunoactivity,as well as in up-regulating serum P content(P <0.05).No significant differences were found between the medication and acupuncture groups in the above mentioned 3 serum and 3 mammary indexes(P>0.05).Conclusion Acupuncture,Rule Granule,and acupuncture combined with Rule Granule can improve hyperplasia of mammary gland in rats,which may be related to their effects in reducing serum PRL and E2 and breast ER,PR and PRLR expression levels,and in increasing serum P level.The therapeutic effects of acupuncture plus Rule Granule are better than those of simple acupuncture and simple Rule Granule.

      摘要:Objective To observe the effect of electroacupuncture(EA)stimulation of single and multiple acupoints on sleep and concentrations of interlukin-1β(IL-1β),brain-derived neurotrophic factor(BDNF),prostaglandin D2(PGD2)and melatonin(MLT,sleep-promoting factors)and corticosterone(CORT,awakening-promoting factor)in the serum in insomnia rats,so as to explore its efficacy difference and the mechanism underlying improving sleep.Methods Fifty-four male SD rats were randomly divided into control,model,EA-Baihui(GV 20),EA-Shenmen(HT 7),EA-Sanyinjiao(SP 6)and EA-GV 20+HT 7+SP 6 groups(n=9 rats in each group).The insomnia model was established by intraperitoneal injection of para-chlorophenylalanine(PCPA,300 mg/kg)once daily for 2 days.In the EA-GV 20,EA-HT 7,EA-SP 6 and EA-GV 20+HT 7+SP 6 groups,EA stimulation was administrated for 30 min,once a day for 4 days.The sleep onset latency and sleep duration were measured after intraperitoneal injection of pentobarbital sodium(35 mg/kg).The concentrations of IL-1β,BDNF,MLT,PGD2 and CORT in the serum were detected by ELISA.Results After EA stimulation of GV 20,HT 7,SP 6 and GV 20+HT 7+SP 6,the sleep latency was significantly shortened(P<0.05,P<0.01,except SP 6),and the sleep duration was remarkably prolonged in comparison with the model group(P<0.05,P<0.01),and the therapeutic effects of EA-GV 20+HT 7+SP 6 were significantly superior to those of EA-GV 20,EA-HT 7 and EA-SP 6 in shortening the sleep latency and lengthening the sleep duration(P<0.05).Following modeling,the concentrations of IL-1β,BDNF,PGD2 and MLT were significantly down-regulated,and the CORT level was markedly upregulated in the model group relevant to the control group(P<0.05).Following EA,modeling induced dramatic decrease of serum IL-1β,BDNF,PGD2 and MLT was considerably up-regulated,and the increased CORT level markedly down-regulated in the EA-GV 20,EA-HT 7,EA-SP 6 and EA-GV 20+HT 7+SP 6 groups(P<0.05).The effects of EA-GV 20+HT 7+SP 6 were evidently superior to those of EA-GV 20 and EA-SP 6 in up-regulating serum IL-1β,BDNF and PGD2 levels,and to those of HT 7,GV20 and SP 6 in up-regulating serum MLT level,and significantly superior to those of EA-ST 7 and EA-SP 6 in down-regulating serum CORT(P<0.05).Conclusion EA stimulation of HT 7,GV 20,SP 6 and GV 20+HT 7+ SP 6 can significantly improve the sleep in insomnia rats,which is closely associated with its effects in regulating serum sleep-promoting factors and awakeningpromoting factor.Joint administration of EA of GV 20+HT 7+ SP 6 has a better effect than the single acupoint mentioned above.

      摘要:Objective To explore the efficacy of moxibustion combined with medication in the treatment of refractory nausea and vomiting and quality of life(QOL)in advanced cancer patients.Methods A total of 266 advanced cancer patients with nausea and vomiting were randomly assigned to Metoclopramide group(M group,n=70),Metoclopramide plus Haloperidol group(MH group,n=65),moxibustion+ M group(n=63),and moxibustion+ MH group(n=68).Moxibustion was applied to bilateral Zusanli(ST 36),and Guanyuan(CV 4),Qihai(CV 6)for 20 min every time,twice a day for 2 weeks.The Rhodes' Index of nausea,Vomiting and Retching(INVR)was used for assessing the state of these symptoms in duration,frequency and severity(40 points),the 24-item Hamilton Depression Rating Scale(HAMD,0-76 points)was employed to measure the patients' depression severity,and the Functional Assessment of Cancer Therapy-General(FACT-G,including physical,social/family,emotional,and functional dimensions,27 items,108 points)was adopted to measure the cancer patients' QOL.Results Following the treatment,the INVR and HAMD scores were significantly decreased in the M,MH,moxibusion+M and moxibustion+MH groups in comparison with their own pretreatment(P<0.05)and were significantly lower in the moxibustion+MH group than in the M,MH and moxibustion+M groups(P<0.05).The FACT-G scoring outcomes showed that the scores of physical well-being,emotional well-being,and total score of QOL were significantly higher in the MH,moxibustion+M and moxibustion+MH groups than in the M group,and were significantly higher in the moxibustion+MH group than in the MH and moxibustion+M groups(P<0.05).No significant differences were found between the MH and moxibustion+M groups in the INVR and HAMD scores,and in the scores of physical well-being,emotional well-being and total score of QOL(P>0.05).Conclusion Moxibustion plus Metoclopramide and Haloperidol can relieve refractory nausea and vomiting,and better depression and QOL in advanced cancer patients,being worthy of popularization in clinical practice.

      摘要:Objective To compare the therapeutic effect of acupuncture plus moxibustion and simple acupuncture in the treatment of patients with knee osteoarthritis(KOA)of yang-deficiency syndrome.Methods Fifty-eight KOA patients with yang-deficiency syndrome were chosen and randomly divided into acupuncture plus moxibustion group(n=30)and acupuncture group(n=28).Neixiyan(EX-LE 4),Dubi(ST 35),Liangqiu(ST 34),Heding(EX-LE 2),Xuehai(SP 10),Yanglingquan(GB34)on the affected side of the body were punctured with filiform needles or/and stimulated with moxibustion using seed-sized moxa cones.The treatment was conducted once daily for 10 days,followed with another 10 days after 2 days interval.The pain severity was evaluated by using visual analogue scale(VAS),and the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores were used to measure the KOA pain,stiffness and function before and after the treatment,and 1 month after the treatment.The therapeutic effect was also evaluated according to the"Standards for Diagnosis and Therapeutic Effect Evaluation of Diseases/Syndromes of Traditional Chinese Medicine"(issued by the State Administration of Traditional Chinese Medicine of China in 1994).Results Twenty days and 1 month after the treatment,the scores of VAS,and KOA pain,stiffness and motor function of WOMAC were significantly decreased in both groups in comparison with their own pre-treatment(P<0.01),and were obviously lower in the acupuncture plus moxibustion group than in the acupuncture group(P<0.05,P<0.01).Of the 28 and30 cases in the acupuncture and acupuncture plus moxibustion groups,7 and 12 experienced marked improvement,12 and 16 were effective,9 and 2 ineffective,with the therapeutic effect being 67.86% and 93.33%,respectively.The therapeutic effect of acupuncture plus moxibustion was apparently superior to that of simple acupuncture(P<0.05).Conclusion Acupuncture plus moxibustion is significantly superior to simple acupuncture therapy in relieving symptoms of KOA patients,and also has a better post-effect.

      摘要:Objective To compare the effect difference between the thermosensitive moxibustion(TSM)and conventional mild moxibustion(CMM)at Yifeng(TE 17)plus acupuncture intervention in the treatment of facial paralysis patients.Methods The prospective cohort study design was used in the present study.A total of 183 patients were divided into a thermosensitive moxibustion(regional heat penetrating,heat extending,warmer in deep-tissue,etc.)group(n=132)and a CMM group(n=51)according to the patientssubjective feeling to moxibustion stimulation of TE 17.After propensity score matching(PSM,a statistical matching technique of observational data)processing by using SPSS 19.0 software,outcomes of 33 cases in the TSM group and 33 cases in the CMM group were analyzed.In the 1 st course(10 days)of treatment,TSM was applied to TE 17 till the thermosensitive feelings disappeared for patients in the TSM group,or CMM employed to TE 17 for 45 min for patients in the CMM group,followed by manual acupuncture stimulation of Cuanzhu(BL 2),Yangbai(GB 14),Sibai(ST 2),Quanliao(SI 18),Jiache(ST 6),etc.on the affected side(shallow needling,uniform reinforcing-reducing needle-manipulation)for 30 min,once daily.In the 2 nd course(10 days),only manual acupuncture was performed,the same acupoints were stimulated with the same methods used in the 1 st course,and in combination with bilateral Zusanli(ST 36)which was stimulated with reinforcing-needling technique.The interval between the 2 courses was 2 days.The modified Portmann scale(for movements of eyebrow raising,eye closing,cheek bulging,pouting,teeth showing and nostril widening,and symetry during resting state)was used to evaluate the severity of the facial palsy before and after the treatment.Results After the treatment,the Portmann scores were significantly higher than those of their own pre-treatment in each of the two groups(P <0.01),being markedly higher in the TSM group than in the CMM group(P <0.01),suggesting a better therapeutic effect of TSM.Conclusion The thermosensitive moxibustion is considerably superior to the conventional mild moxibustion in improving the symptoms of facial paralysis patients.

      摘要:Objective To analyze the regularity of acupoint selection in the treatment of insomnia by using catgut embedding so as to provide reference and evidence for clinical application.Methods Sixty-six studies about acupoint catgut embedding treatment of insomnia published from 1996 to 2017 were finally selected from the databases of China National Knowledge Infrastructure(CNKI),Chinese Journal of Science and Technology of VIP(VIP),and WanFang Database.The Excel 2016 software was used for data processing and analysis,SPSS 21.0 was employed for descriptive and cluster analyses,and the Clementine 12.0 software was employed to analyze the association regularities of the most frequently used acupoints or acupoint recipes.Results The most frequently selected main acupoints for insomnia by using catgut embedment are Xinshu(BL 15),Shenshu(BL 23),Sanyinjiao(SP 6),Pishu(BL 20),Ganshu(BL 18),combined with adjuvant acupoints Danshu(BL 19)and Fenglong(ST 40).The most frequently selected channels are the Bladder and Stomach Meridians,and the most frequently involved body positions are the back and lower limbs.Effective clusterings are BL 20—SP 6,Shenmen(HT 7)-Anmian,ST 40-Shenting(GV 24),Qihai(CV 6)-Jueyinshu(BL 14).Descriptive analysis indicated:(1)The stronger correlation acupoint groups between back-shu points are BL 18—BL 23;BL 18,BL 15—BL 23;BL 18,BL 20,BL 15—BL 23.(2)The stronger correlation acupoint groups between common acupoints are Neiguan(PC 6),Zusanli(ST 36)—SP 6;PC 6,HT 7 — SP 6.(3)The stronger correlation acupoint groups between common acupoints and back-shu points are BL 18,SP 6—BL 23;PC 6,BL 23 — SP 6;ST 36,PC 6,BL 15 — SP 6;BL 18,BL 15,SP 6 — BL 23.Conclusion Acupoint catgut embedding therapy is effective in the treatment of insomnia,being worthy of further popularization in clinical practice.

      摘要:Brain plasticity is referred to the ability of the brain to change in structure and functional activities from microscopic aspects(synapses)in individual neurons to larger-scale aspects such as cortical remapping,neuronal remodeling,etc.in response to injury.Acupuncture has a positive effect in the treatment of cerebral diseases,thus leading to an increasing research on its underlying mechanisms in regulating brain plasticity.The present paper reviewed recent development of studies on acupuncture treatment of Alzheimer's disease,vascular dementia,cerebral ischemia,bone cancer pain,autism,etc.from 1)regulating synaptic plasticity(up-regulating the proportion of hippocampal synapses,increasing synaptic curvature,post-synaptic density,and numerical density,areal density,and lowering synaptic cleft);2)increasing hippocampal post-synaptic potential slop,peak and peak area of population spikes to evoke long-term potentiation(LTP)by regulating the expression of glutamate N-methyl-Daspartate(NMDA)and alpha amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid(AMPA)receptors,and growth associated protein(GAP-43);3)down-regulating the expression of glial fibrillary acidic protein(GFAP)to suppress the activity of astrocytes in the ischemic cerebral area and hippocampus in cerebral ischemia rats,and in the spinal cord dorsal horns of bone cancer rats,etc.and promoting vimentin and connexin 43 expression;and 4)promoting neuronal remodeling,and repair and regeneration of the injured nerve in the central nervous system via regulating neuron-glia network,expression of nerve growth factor,brain derived neurotrophic factor,etc.The future studies should pay more attention to 1)the contribution of acupuncture intervention to the correlation between the initiation of the endogenous protective effect and the brain plasticity during the onset of nerve injury and repair of the injured nerve,and 2)standarization of stimulating quantity,manipulation and duration of acupuncture treatment,as well as the relationship between the stimulating quantity and efficacy.
