      摘要:Prehypertension is an independent risk factor for end stage of heart,brain and renal diseases.The immune inflammatory imbalance promotes the occurrence of damage of target organs in the pre-hypertension stage.This article focuses on the relationship between the immune-inflammation and prehypertension and its target organ damage.It was found that acupuncture treatment can lower blood pressure,postpone the development of prehypertension,improve vascular endothelia function and immune function,down-regulate the levels of serum proinflammatory cytokines,and reduce inflammatory reactions of the heart and kidney,possible by way of regulating cellular signal pathways as TLR 4/NF-κB,p 38/MAPK,CaM-eNOS-NO,TGF-β1/Smads,etc.and playing aprotective effect on the target organs.However,its detailed mechanisms remain largely unknown up to now.

      摘要:Objective To observe the effect of electroacupuncture(EA)at"Zusanli"(ST 36)and"Feishu"(BL 13)on pulmonary function,inflammatory reaction and expression of macrophage migration inhibitory factor(MIF)and its receptor complex CD 74-CD 44,etc.in rats with chronic obstructive pulmonary disease(COPD),so as to explore its mechanism underlying improvement of COPD.Methods Thirty male SD rats were randomly divided into normal,model and EA groups(n=10 in each group).The COPD model was established by intratracheal infusion of Lipopolysaccharide(LPS,1 mg/mL)and forced smoke-inhaling.EA was applied to bilateral ST 36 and BL 13 for 30 min,once daily for 7 days.The rats lung function(forced vital capacity[FVC],forced expiratory capacity ratio([FEV 0.1/FVC]and[FEV 0.3/FVC])was detected under anesthesia.Pathological changes of the lung tissue were detected by H.E.staining,and the contents of MIF,tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and IL-8 in serum,bronchoalveolar lavage fluid(BALF)and lung tissue were assayed by ELISA.The immunoactivity of CD 74 and CD 44 was detected by immunohistochemistry,and the expression levels of MIF,CD 74,CD 44 and p 38 MAPK mRNAs and proteins were examined by quantitative RT-PCR and Western blot,respectively.Results Compared with the normal group,the FVC,FEV 0.1,FEV 0.3,FEV 0.1/FVC and FEV 0.3/FVC levels were significantly decreased in the model group(P<0.01).After EA treatment,the FVC,FEV 0.1,FEV 0.3,FEV 0.1/FVC and FEV 0.3/FVC were significantly increased(P<0.01,P<0.05),suggesting an improvement of the pulmonary function after EA.H.E.staining showed that the severity of modeling induced alveolar expansion and inflammatory cell infiltration in the lung tissue was relatively milder in the EA group relevant to the model group.The contents of MIF,TNF-α,IL-1βand IL-8 in the serum,BALF and lung tissues were significantly higher in the model group than in the normal group(P<0.01),and significantly down-regulated in the EA group relevant to the model group(P<0.01).The expression levels of MIF,CD 74,CD 44 and p 38 MAPK mRNAs and proteins and the immunoactivity levels of CD 74,CD 44 in the lung tissue were obviously higher in the model group than those in the normal group(P<0.01),and considerably lower in the EA group than those in the model group(P<0.01).There was a positive correlation between p 38 MAPK and MIF in mRNA and protein expression levels(P<0.01).Conclusion EA intervention can improve the pulmonary function in COPD rats,which may be related to its effects in inhibiting inflammatory reaction,and MIF/CD 74-CD 44/p 38 MAPK signaling pathway.

      摘要:Objective To explore the effect of electroacupuncture(EA)on the expression of synovial AMP-activated protein kinase(AMPK)proteinα,arginase-1 mRNA,nitric oxide synthase 2(NOS 2)mRNA,NOD-like receptor protein 3(NLRP 3)mRNA,and interleukin-1β(IL-1β)mRNA in acute gouty arthritis(AGA)rats,so as to explore its mechanisms underlying improvement of AGA via M 1/M 2 macrophage polarization.Methods Male Wistar rats were randomly divided into normal control,model,medication(colchicine)and EA groups(n=15 rats in each group).The AGA model was established by injection of sodium urate crystal(MSU)suspension(0.2 mL)into the articular cavity of the left knee.The rats of the normal control group received articular injection of normal saline(0.2 mL)of the left knee,and those of the medication group were treated by gavage of the colchicine(0.3 mg·kg-1·d-1)once daily for 7 days.EA(2 Hz/10 Hz,1.0 mA)was applied to"Zusanli"(ST 36)and"Sanyinjiao"(SP 6)of the left hind limb for 10 min,once daily for 7 days.The inflammatory conditions of the synovial membrane tissue of the left knee joint were observed by H.E.staining.The expression levels of phosphorylated AMPKα(p-AMPKα)protein,and arginase-1(a maker of M 2 macrophages)mRNA,NOS 2(a maker of M 1 macrophages)mRNA,NLRP 3 mRNA,and IL-1βmRNA in the knee joint synovial tissue were detected by Western blot and quantitative real-time PCR,respectively.Results Compared with the normal group,the inflammatory cell infiltration of the synovial tissue was more severe,the expression of p-AMPKαprotein was significantly decreased(P<0.01),and the expression levels of arginase-1,NOS 2,IL-1βand NLRP 3 mRNAs were considerably increased in the model group(P<0.01).The expression levels of p-AMPKαprotein and arginase-1 mRNA were significantly up-regulated,and those of NOS 2,IL-1βand NLRP 3 mRNAs obviously down-regulated in both EA and medication groups relevant to the model group(P<0.01,P<0.05),suggesting an increase of M 2 macrophage and a decrease of M 1 macrophage activity after EA.No significant differences were found between the EA and medication groups in up-regulating p-AMPKαexpression and in down-regulating NOS 2,IL-1βand NLRP 3 mRNA expression(P>0.05),except higher up-regulation of arginase-1 mRNA in the medication group(P<0.05).Conclusion EA intervention can up-regulate the expression of arginase-1 mRNA and p-AMPKαprotein,and down-regulate the expression of NOS 2,IL-1βand NLRP 3 mRNAs in synovial tissues in AGA rats,which may contribute to its anti-inflammatory effect by promoting conversion of macrophages from M 1 pro-inflammatory phenotype to M 2 anti-inflammatory phenotype.

      摘要:Objective Yang deficiency constitution may bring with different types of illnesses in many people.The present study was designed to observe the effect of ginger-separated snake moxibustion on yang deficiency symptom,and serum adrenocorticotropic hormone(ACTH)and cortisol(CORT)contents in yang deficiency constitution subjects,so as to explore its mechanism underlying improvement of yang deficiency constitution.Methods Ninety subjects with yang deficiency type constitution were equally and randomly assigned to control A group(19 men and 26 women,18-60 years in age)and moxibustion group(16 men and 29 women,18-60 years in age)and 45 normal subjects with mild constitution(18 men and 27 women,18-60 years in age)were assigned to control B group.An snake-like moxa-cone was placed at a moxa-holder covering the middle back of human body from Dazhui(GV 14)to Yaoshu(GV 2)after paving a layer(about 1 cm thick)of fresh ginger in the holder,and then ignited,followed by the next moxa-cone,3 cones altogether every time for each participant.The treatment was conducted once a week,12 times in total.The subjects' constitution was scored according to the "Classification and Evaluation of Constitution of Traditional Chinese Medicine"formulated by China Association of Chinese Medicine.The serum ACTH and CORT contents were measured by radioimmunoassay before and at the end of the treatment and 6 months after the treatment.ResultsThe constitation scores of the moxibustion group and control A group were significantly higher than that of the control B group before treatment(P<0.01).At the end of the treatment,the constitution score was significantly decreased in the moxibustion group(P<0.01)rather than in the control A group relevant to its own pre-treatment(P>0.05).The serum CORT and ACTH contents were significantly decreased in both the moxibustion and control A groups in comparison to the control B group before treatment(P<0.01),and obviously increased at the end of treatment in the moxibustion group compared with its own pre-treatment(P<0.01).No significant differences were found among the three groups in the constitution score,and serum CORT and ACTH levels at the 6 th month after the treatment compared with their own post-treatment(P>0.05).Conclusion The ginger-separated snake moxibustion treatment may improve the constitution of yang deficiency participants by increasing the levels of serum ACTH and CORT,which may be helpful in resisting diseases.

      摘要:Objective To observe the influence of acupuncture and moxibustion on humoral immunity and cellular immunity related indexes in patients with bra-chiocephalic Takayasu arteritis(BCTA),so as to explore its underlying mechanisms in the treatment of Takayasu arteritis(TA).Methods A total of 31 BCTA patients(9 men and 22 women at age of 16—70 years)were treated by manual acupuncture stimulation of main acupoint Renying(ST 9)and auxillary acupoints as Jiquan(HT 1),Chize(LU 5),Fengchi(GB 20),bilateral Xinshu(BL15),Feishu(BL 13)and Geshu(BL 17),Jingming(BL 1),etc.according to clinical symptoms,with the needles retained for 20 min.Additionally,moxibustion was applied to Taiyuan(LU 9),BL 15,BL 13 and BL 17.The treatment was conducted twice daily for 4 weeks.Serum IgG,IgA,IgM,IgE,C3,C4 contents,and CD3+ ,CD4+ ,CD4+ ,CD8+ ,CD8+ T lymphocytes contents were assayed by using immue turbidimetry and cell counting.Results Before the treatment and in these 31 BCTA patients,an abnormal increase was found in serum IgG content in 10 cases,IgM in 3 cases,IgE in 3 cases,C3 in11 cases,CD4+ T lymphocytes contents were assayed by using immue turbidimetry and cell counting.Results Before the treatment and in these 31 BCTA patients,an abnormal increase was found in serum IgG content in 10 cases,IgM in 3 cases,IgE in 3 cases,C3 in11 cases,CD4+ in 6 cases,and CD8+ in 6 cases,and CD8+ in 9 cases,and an abnormal decrease was found in serum CD3+ in 9 cases,and an abnormal decrease was found in serum CD3+ level in 11 cases,CD4+ level in 11 cases,CD4+ in 7 cases,as well as CD8+ in 7 cases,as well as CD8+ in 4 cases.After the treatment,the increased levels of IgG,IgM,C3,CD4+ in 4 cases.After the treatment,the increased levels of IgG,IgM,C3,CD4+ and CD8+ and CD8+ in patients with abnormally higher values were considerably down-regulated(P<0.05),the decreased levels of CD3+ in patients with abnormally higher values were considerably down-regulated(P<0.05),the decreased levels of CD3+ ,CD4+ ,CD4+ and CD8+ and CD8+ in patients with abnormally lower values were evidently up-regulated(P<0.05).No significant changes were found in IgG,IgA,IgM,IgE,C3 and C4,CD3+ in patients with abnormally lower values were evidently up-regulated(P<0.05).No significant changes were found in IgG,IgA,IgM,IgE,C3 and C4,CD3+ and CD8+ and CD8+ in patients with normal values of these indexes after the treatment(P>0.05).The CD4+ in patients with normal values of these indexes after the treatment(P>0.05).The CD4+ level in patients with normal value was significantly deun-regulated after the treatment(P<0.05),but still in the range of normal value.Conclusion Acupuncture plus moxibustion can regulate some of the abnormally elevated and lowered humoral immunity and cellular immunity related indexes in BCTA patients,possibly having a favorable role in the treatment of BCTA.

    • Acupuncture and Moxibustion and Immunity:the Actuality and Future

      摘要:Immunity reaction has been regarded as a key step for clinical acupuncture and moxibustion treatment.In the present paper,we review current situations about studies on acupuncture-moxibustion induced immunoregulation from 1)related project fundings of National Natural Science Foundation(NCFS)of China from 1989—2017;2)papers published in SCI and Chinese medical journals from 2010—2018;3)clinical conditions or disorders treated by acupuncture and moxibustion and their clinical therapeutic effects;4)the commonly used acupoints for studying immune regulation functions;5)some mechanisms of innate immunity and adaptive immunity involved;and 6)immune adjustment pathways involved.Moreover,in our future studies,we suggest to pay more attention to 1)the detailed cellular molecular mechanisms;2)interactions among the immune cells,the immune cells and non-immune cells and cytokines responsible for regulation effects of acupuncture-moxibustion;3)interrelationship of different systems as skin-brain axis,brain-intestinal axis,nerve-blood vessel unit of brain tissues,etc.involving acupuncture-moxibustion induced immunoregulation by using new techniques as proteomics,genomics,two-photon imaging technology,tracer technique,cryo-electronic microscope technology,etc.

      摘要:Objective To observe the effect of electroacupuncture(EA)on histopathological changes of cartilage and subchondral bone and osteoprotegerin(OPG),receptor activator of nuclear factor-κB(RANK),and RANK ligand(RANKL)(OPG/RANK/RANKL)signaling and the expression of matrix metalloproteinase-13(MMP-13)in ovariectomized(OVX)rats,so as to explore its mechanism underlying improvement of osteoporosis.Methods Three-month female SD rats were randomly divided into sham operation,model and EA groups(n=8 in each group).The ovoariectomy model was established by resection of bilateral ovaries.Rats of the sham group were treated by simple removal of a piece of adipose tissue around the bilateral ovaries.EA(3 Hz/15 Hz,1 mA)was applied to bilateral"Sanyinjiao"(SP 6),"Yanglingquan"(GB 34),"Yinlingquan"(SP 9)and"Zusanli"(ST 36)for 30 min,once daily(except the weekends)for 12 weeks.The histopathological changes of the subchondral bone of the right knee-joint were observed after Saffron O dyeing and evaluated by Mankins score,and its anatomical structure including the bone volume fraction(bone volume/tissue volume,BV/TV),trabecular bone number(Tb.N)and trabecular thickness(Tb.Th),trabecular separation(Tb.Sp)was observed by using Micro CT imaging.The urine C-terminal cross-linking telopeptide of typeⅠcollagen(CTX-Ⅰ),CTX-Ⅱ(two bone resorption markers)and serum estrogen(E 2)contents were assayed by ELISA,and the expression levels of OPG,RANKL and MMP-13 mRNAs in the cartilage tissue of the left knee-joint were detected by quantitative RT-PCR.Results Following modeling,the BV/TV,Tb.N and Tb.Th levels were significantly decreased(P<0.01)and Tb.Sp and Mankin's score obviously increased in the model group compared with the sham group(P<0.01),suggesting a formation of osteoporosis and degeneration of the cartilage tissue.The serum E 2 content and OPG mRNA level in the cartilagetissue were significantly down-regulated(P<0.01),and urine CTX-Ⅰ and CTX-Ⅱcontents and RANKL mRNA and MMP-13 mRNA expression levels cartilagetissue were considerably up-regulated in the model group relevant to the sham group(P<0.01).After EA intervention,modeling-induced decrease of BV/TV,Tb.N,Tb.Th,E 2 and OPG mRNA levels and OVX-induced increase of Tb.Sp,Mankin's score,CTX-Ⅰ,CTX-Ⅱ,RANKL mRNA and MMP-13 mRNA levels were all completely reversed in the EA group relevant to the model group(P<0.01,P<0.05).Conclusion EA intervention can inhibit subchondral bone osteoporosis and articular cartilage degeneration of knee-joint in OVX rats,which is closely associated with its effects in inhibiting the downregulation of serum E 2 and OPG mRNA expression and up-regulation of CTX-Ⅰ,CTX-Ⅱ,RANKL mRNA and MMP-13 mRNA levels,including adjusting OPG/RANK/RANKL signaling.

      摘要:Objective To observe the therapeutic effect of electroacupuncture(EA)at Zusanli(ST 36)and Neiguan(PC 6)on stress responses of patients undergoing gastrointestinal surgery.Methods A total of 40 patients undergoing gastrointestinal surgery were randomized into conventional treatment group(control)and EA group(n=20 in each group).Patients of the EA group received conventional treatment(pre-and post-surgical fasting,measures for gastrointestinal decompression,parenteral nutrition support,and patient controlled analgesia pump,etc.)and EA stimulation(2 Hz,30 min)of bilateral ST 36 and PC 6(twice after surgery,at an interval of 6 h),and patients of the control group received conventional treatment only.The visual analogue scale(VAS)score was used to assess the patients' pain severity and the blood glucose levels were detected once every4-6 hwithin 24 hafter operation.Serum cortisol(Cort)and adrenocorticotropic hormone(ACTH)levels were detected by chemiluminescence method,and serum D-lactic acid level(for assessing gastrointestinal mucosal injury)was assayed by ELISA.Results After the treatment,the levels of serum Cort,ACTH,D-lactate acid and the highest blood glucose were significantly lower in the EA group than those in the control group(P<0.05,P<0.01),suggesting a reduction of stress reactions after EA.But no significant difference was found between the control and EA groups in the VAS score(P>0.05).Conclusion EA at ST36 and PC 6 can alleviate stress responses and reduce intestinal mucosal damage in patients undergoing gastrointestinal surgery.

      摘要:Objective To investigate the effect of electroacupuncture(EA)at different time-points on postoperative gastrointestinal function in patients undergoing colorectal cancer surgery.Methods Eighty patients with colorectal cancer undergoing laparotomy were randomly assigned to intravenous anesthesia,EA A,EA B,and EA C groups(n=20 cases in each group).All the patients in the four groups received intravenous anesthesia with midazolam,sufentanil,cisatracurium besylate and Propofol,postoperative gastrointestinal decompression and drug analgesia.EA(2-3 mA,2 Hz)was applied to Zhongwan(CV 12)and Tianshu(ST 25),Neiguan(PC 6),Zusanli(ST 36),Shangjuxu(ST 37),Xiajuxu(ST 39)on the right side for 30 min,once(one day before surgery)in the EA A group,twice(one day and 30 min before surgery)in the EA B group,and 3 times(one day,30 min before and one day after surgery)in the EA C group.The acupoints used after surgery were PC 6,ST 36,ST 37 and ST 39.The time of postoperative ventilation,defecation,food-intake and water drinking,stomach tube removal and abdominal drainage tube removal,the volumes of stomach tube drainage and abdominal drainage,and postoperative adverse reactions were recorded.Results The first ventilation time,after surgery in the EA C group was significantly earlier than those in the intravenous anesthesia,EA A and EA B groups(P<0.05);and the water intake and abdomicnal drainage tube removal time after surgery in the EA C group were significantly earlier than those in the intravenous anesthesia group(P<0.05).No significant differences were found among the 4 groups in the time of defecation,food intake,stomach tube removal,stomach tube drainage and abdominal drainage volumes,and numbers of patients with nausea,vomiting,fever and other adverse reactions(P>0.05).Conclusion EA treatment combined with intravenous anesthesia conducted before and after surgery is effective in promoting the recovery of gastrointestinal function in patients undergoing colorectal cancer laparotomy,and is obviously better than simple pre-operative EA.

      摘要:Objective To analyze the regularities of selection of meridian acupoints for coronary heart disease(CHD)recorded in the ancient Chinese medical literature,in order to provide a reference for clinical application of meridian acupoint recipes to treat CHD nowadays.Methods Papers were retrieved from 1 156 ancient Chinese medical books collected in"Encyclopedia of Traditional Chinese Medicine"(Fifth Edition)by using key words"Xin Tong"(cardiac pain),"Zhen Xin Tong"(true heart pain),"Jue Xin Tong"(precordial pain with cold limbs),"Xiong Bi"(obstruction of qi in the chest),"Zheng Chong"(severe palpitation),"Xin Ji"(palpitation),followed by establishment of a database.Then,the association analysis data mining technology was used to analyze the characteristics and regularities of application of meridian acupoints in the treatment of CHD.Results A total of 347 items of ancient document data were collected,containing acupoints of the 12 regular meridians,Conception Vessel and Governor Vessel,with a frequency of usage being 625 times.Among the involved meridians,the Conception Vessel was most frequently used,followed by the Pericardium Meridian.Zhongwan(CV 12)was the most frequently used acupoint,and Rangu(KI 2)and Taixi(KI 3),CV 12 and Shangwan(CV 13),and Quze(PC 3)and Daling(PC 7)were the top 3 frequently used auxiliary acupoint pairs.Conclusion In ancient China,in the treatment of CHD,the main acupoints of the Conception and Pericardium Meridian,and the auxiliary acupoints i.e.,five-shu points are most frequently used.PC 3 and PC 7 combination is suitable for the treatment of CHD with negative emotion.

    • A Meta-analysis on Acupuncture and Moxibustion Treatment of Schizophrenia

      摘要:Objective To evaluate the clinical efficacy and security of acupuncture therapy combined with conventional antipsychotics in the treatment of schizophrenia.Methods We first comprehensively retrieved papers about acupuncture or moxibustion treatment of schizophrenia published from the time of establishment of the database to April of 2017 in Chinese databases such as Chinese National Knowledge Infrastructure(CNKI),Chinese Science and Technology Periodical Database(VIP),Wanfang,and Chinese BioMedical Literature(CBM)by using keywords"acupuncture""moxibustion""acupuncture+moxibustion""acupuncture+medication""electroacupuncture""filiform needle""scalp-acupuncture""auricular acupuncture",etc.Then,according to the principles and methods of evidence-based medicine,papers conformed to the included standards were enrolled to Meta-analysis by using Review Manager 5.3 software.Results A total of 618 studies were identified,with only 17 articles(containing 1 542 patients)conformed to our included standards and analyzed finally.In regard to the total effective rate of acupuncture or moxibustion for schizophrenia,the combined OR=3.09,95% CI(2.31,4.13),the diamond was on the right side of the vertical line(suggesting being effective),OR checked by Ztest(Z=7.61,P<0.000 01).In regard to the untoward effect,the combined OR=0.34,95% CI(0.16,0.75),the diamond was on the left side of the vertical line(suggesting less adverse effects),OR checked by Ztest(Z=2.67,P=0.008).Sensitivity analysis:the combined standardized mean difference(SMD)=-0.61,95% CI(-0.88,-0.34),the diamond was on the left side of the vertical line(suggesting having more profits than simple medication),SMD checked by Ztest(Z=4.39,P<0.000 1).Conclusion Acupuncture and moxibustion combined with conventional antipsychotics is significantly better than that of antipsychotics alone in the treatment of schizophrenia and has less side effects.But,high quality clinical studies with rigorous experimental design and large sample size are needed to provide more reliable evidence.
