      摘要:Objective To explore the effect of electroacupuncture(EA) combined with Schwann cell(SC) transplantation(SCT) on remyelination of axons and neuregulin(Nrg1) in rats with compressed spinal cord injury(CSCI),so as to explore the mechanism of EA and SCT underlying improvement of CSCI. Methods SD female rats were randomly divided into normal, mo-del, EA, SCT, and EA+ SCT groups(n=40 per group). A self-developed model of spinal compressed injury was adopted in this study. Rats of the model group were administrated laminectomy without treatment. Rats in the EA group were administrated EA stimulation at “Dazhui”(GV14), “Mingmen”(GV7), bilateral “Zusanli”(ST36) and “Taixi”(KI3) on the second day post-surgery for 10 min. Rats in the SC group were administrated SCT at 1 week post-surgery, and in the EA+SC group were given EA stimulation combined with SCT. The injured spinal cord tissue was obtained 0, 2, 4 and 8 weeks after compressed spinal injury. The functional recovery was assessed by Basso-Beattie-Bresnahan(BBB) score. The survivals and migration of SC after transplantation, myelination were observed by immunofluorescence. The ultrastructure of myelin in injured site was observed by transmission electron microscope, and the expression levels of glial fibrillary acidic protein(GFAP), protein zero(P0), and Nrg1 and Nrg1-ntf(cleavage protein of Nrg1) proteins of the spinal cord were detected by Western blot. Results Compared with the normal group, BBB scores in the model group was significantly decreased(P<0.05),nervous fibers were demyelinated, numbers of normal and newborn myelination were decreased(P<0.05),expression of P0 was significantly increased(P<0.05),expression of GFAP was significantly increased(P<0.05),and the expression levels of Nrg1 and Nrg1-ntf proteins were decreased(P<0.05). In comparison with the model group, the BBB scores in the EA, SCT and EA+SCT groups were significantly increased(P<0.05,P<0.01), demyelination was improved, numbers of normal and newborn myelinations were increased(P<0.05,P<0.01),expressions of P0 were significantly increased(P<0.05,P<0.01),expressions of GFAP were significantly decreased(P<0.05),and the expression levels of Nrg1 and Nrg1-ntf proteins were increased(P<0.05, P<0.01).The differences were most significant in the EA+SCT group among the three groups. Conclusion EA can improve the locomotor function in CSCI rats, which may be rela-ted to its functions in promoting the survival and migration of transplanted SC and remyelination, and increasing the expressions of Nrg1 and its cleavage protein after SC transplantation.

      摘要:Objective To observe the effect of electroacupuncture(EA) on tumor number, body conditions, inflammatory factors and expression levels of silent information regulator 1(sirtuin 1, SIRT1) and autophagy-related proteins Beclin-1, P62, and LC3 in colorectal tissues in inflammatory-transformed colorectal cancer mice, so as to explore its underlying mechanisms in resisting tumor growth.Methods A total of 100 C57 BL/6 male mice were randomly divided into normal control, model, EA, EA + SIRT1 inhibitor(EA+inhibitor) and agonist resveratrol(agonist) groups, with 20 mice in each group. EA(2 Hz, 1 mA) was applied to “Zusanli”(ST36)and “Fenglong”(ST40) for 20 min every time, 3 times a week for 11 weeks. Mice of the EA +inhibitor group received intraperitoneal injection of SIRT1 inhibitor EX527(5 mg/kg) at the same time of EA treatment, and those of the agonist group received gavage of resveratrol(200 mg/kg, an agonist of SIRT1), 3 times a week for 11 weeks. The body mass was measured weekly. The disease activity index(DAI), colorectal length and tumor number in each group were recorded. The histopathological changes of colorectal tissues were observed by H.E. staining; the contents of interleukin 6(IL-6), IL-10, IL-17, in the colorectal tissues were detected by enzyme-linked immunosorbent assay, and the expression levels of SITR1, Beclin-1, P62, and LC3 in colorectal tissues were detected by Western blot and real-time fluorescence quantitative PCR, respectively.Results Compared with the normal control group, the body weight, length of colorectum, the contents of IL-10, and the expression levels of SIRT1,Beclin-1 and LC3 mRNAs and proteins were significantly decreased(P<0.001), whereas the DAI score, the number of tumors, the contents of IL-6 and IL-17, and the expression levels of P62 mRNA and protein were significantly increased(P<0.000 1, P<0.001) in the model group. In comparison with the model group, the body weight, the length of colorectum, the contents of IL-10, and the expression levels of SIRT1,Beclin-1 and LC3 mRNAs and proteins were significantly increased(P<0.01,P<0.05,P<0.001), while the DAI scores, the numbers of tumors, the contents of IL-6 and IL-17, and the expression levels of P62 mRNA and protein were obviously decreased in the EA and agonist groups(P<0.01,P<0.05, P<0.001). No significant changes were found in all the above-mentioned indexes in the EA+inhibitor group in comparison with the model group(P>0.05). Conclusion EA can reduce the number of tumors and inflammation reaction in colorectal tissue and improve the body condition in mice with colorectal cancer, which may be related to its functions in activating the expression of intestinal SIRT1, and then facilitating cellular autophagy.

      摘要:Objective To observe the effect of electroacupuncture(EA) on motor function, serum Cystatin C(Cys C) content, and expressions of tumour necrosis factor-α(TNF-α) and nuclear factor-kappa B(NF-κB) in renal tissue of rats with acute cerebral infarction(ACI), so as to explore its underlying mechanisms in protecting renal tissue after ACI. Methods Seventy-two male SD rats were randomly divided into three groups: sham operation, model and EA groups which were further randomly allocated to 1 d, 3 d, 7 d and 14 d subgroups(n=6 per subgroup). The ACI model was established by occlusion of the middle cerebral artery(MCAO). Rats of the EA group received EA of “Neiguan”(PC6) and “Zusanli”(ST36) for 30 min, once daily for 1, 3, 7 and 14 days, respectively. The motor function and content of Cys C were determined on the 1(st), 3(st), 3(rd), 7(rd), 7(th) and 14(th) and 14(th) day after ACI. The expressions of TNF-α and NF-κB were detected by immunohistochemistry. Results Compared with the sham operation group, the motor function scores and the content of Cys C increased significantly on the 1(th) day after ACI. The expressions of TNF-α and NF-κB were detected by immunohistochemistry. Results Compared with the sham operation group, the motor function scores and the content of Cys C increased significantly on the 1(st), 3(st), 3(rd), 7(rd), 7(th) and 14(th) and 14(th) d(P<0.01), while the numbers of TNF-α and NF-κB positive cells of the model group increased significantly on the 3(th) d(P<0.01), while the numbers of TNF-α and NF-κB positive cells of the model group increased significantly on the 3(rd), 7(rd), 7(th) and 14(th) and 14(th) d(P<0.01). After EA treatment, the motor function scores and the content of Cys C on the 7(th) d(P<0.01). After EA treatment, the motor function scores and the content of Cys C on the 7(th), and 14(th), and 14(th) d, and the numbers of TNF-α and NF-κB positive cells on the 3(th) d, and the numbers of TNF-α and NF-κB positive cells on the 3(rd), 7(rd), 7(th) and 14(th) and 14(th) d obviously decreased(P<0.05). Conclusion EA at PC6 and ST36 can improve motor function and alleviate renal injury in ACI rats, possibly by regulating the expression of TNF-α, NF-κB in renal tissue and Cys C in serum.

      摘要:Objective To observe the effect of oblique needling at Ashi-point on behavior, and cell morphology, myogenic differentiation antigen(MyoD1) and paired box transcription factor Pax7(Pax7) of quadriceps femoris tissue in quadriceps femoris injured mice. Methods A total of 24 C57 BL/6 male mice were randomly divided into control, model, shallow insertion and deep insertion groups, with 6 mice in each group. The quadriceps femoris injury model was established by single intramuscular injection of 0.5% bupivacaine(BPVC). Twenty-four hours after modeling, mice of the two acupuncture groups were received oblique needling on the surface or through the muscle belly of quadriceps femoris for once, the oblique needling was lifted and inserted 3 times. The climbing pole test was conducted 24 h after modeling and 24 h after EA. Histopathological changes of quadriceps femoris was observed by H.E. staining. The expressions of MyoD1 and Pax7 were detected by immunohistochemistry. Results Compared with the control group, the score of climbing pole test was lower(P<0.01), and the expressions of MyoD1 and Pax7 significantly increased(P<0.01) in the model group. After the intervention and compared with the model group, the score of climbing pole test was higher(P<0.01), and the expressions of MyoD1 and Pax7 obviously increased(P<0.01) in the two acupuncture groups. The therapeutic effect of deep insertion group was apparently superior to that of shallow insertion group in up-regulating the climbing pole test score and expressions of MyoD1 and Pax7(P<0.05, P<0.01). H.E. stain showed large areas of inflammatory infiltration, muscle cells swelling, atrophy, rupture, degeneration and necrosis, different cell sizes and morphologies, enlarged intervals, nuclear aggregation, deep nuclear staining, nuclear pyknosis, and hemorrhage in the model group, which was relatively milder in both needling groups. Conclusion Oblique needling at Ashi-point can effectively promote the benign repair of injured quadriceps muscle and promote the recovery of exercise ability in mice, which may be associated with its effect in up-regulating the expression of MyoD1 and Pax7 protein. The role of deep insertion is superior to that of shallow insertion.

      摘要:Objective To observe the effect of acupuncture of “Yinlingquan”(SP9) and “Sanyinjiao”(SP6) on expression of phosphatidylinositol-3 kinase/protein kinase B/mammalian target protein of rapamycin(PI3 K/Akt/mTOR) signaling in adjuvant arthritis(AA) rats, so as to explore its mechanism underlying improvement of AA. Methods Forty-eight male Wistar rats were randomly divided into normal control, AA model, acupuncture and medication(tripterygium wilfordii) groups, with 12 rats in each group. The AA model was established by putting the rats in a windy, cold and wet environment for 12 h, once every day for 21 days and injection of Freund's complete adjuvant(CFA) into the sole of the right hindlimb on the 21(st) day. Manual acupuncture stimulation was applied at SP9 and SP6 for 30 min/time, once a day for 21 days. Rats of the medication group received gavage of tripterygium wilfordii tablets solution(8 mg/kg), once a day for 21 days, and those of the normal control group and model group received gavage of the same amount of normal saline, once a day for 21 days. The degree of joint swelling and arthritis index(AI) were detected 1 day before modeling, 3 days after modeling, and 21 days after the treatment. Twenty-four hours after the last treatment, the contents of serum cytokines interleukin(IL)-17, IL-6 and tumor necrosis factor(TNF)-α were detected by enzyme-linked immunosorbent assay(ELISA); and changes of synovial ultrastructure were observed under electron microscope. Western blot was used to detect the expression levels of PI3 K, Akt, phosphorylated protein kinase B(p-Akt), phosphorylated mammalian rapamycin target protein(p-mTOR), mTOR, microtubule associated protein 1 light chain 3 B(LC3-Ⅱ) and mammalian atg6 homologous protein(Beclin-1) in the synovial membrane tissue. Results Compared with the normal control group, the joint swel-ling degree and AI, contents of serum TNF-α, IL-6 and IL-17, and the expression levels of PI3 K, Akt, p-Akt, mTOR and p-mTOR in the synovium were increased in the model group(P<0.05), while the expression levels of LC3-Ⅱ and Beclin-1 proteins in the synovium were significantly decreased(P<0.05). Compared with the model group, the joint swelling degree and AI, contents of serum TNF-α, IL-6 and IL-17, and the expression levels of PI3 K, Akt, p-Akt, mTOR and p-mTOR were significantly decreased in the acupuncture and medication groups(P<0.05), while the expresion levels of LC3-Ⅱ and Beclin-1 proteins were significantly increased(P<0.05). Comparison between the two treatment groups showed that the therapeutic effects of acupuncture were obviously weaker than those of medication in down-regulating TNF-α and IL-6, Akt, p-Akt and mTOR levels(P<0.05) and in up-regulating Beclin-1 expression(P<0.05). Outcomes of electron microscope displayed widened nuclear membrane space, some fractured mitochondrial cristae with vacuoles, expanded rough endoplasmic reticulum, reduction of autophagosomes in the cytoplasm and ruptured synovial cell membrane in the model group, and increase of autophagosomes, deformed organelles in the acupuncture and medication groups.Conclusion Acupuncture can relieve the inflammatory reactions and joint synovial injury of the affected joint in AA rats which may be associated with its effects in inhibiting PI3 K/Akt/mTOR signaling and increasing the auto-phagy level of synovial cells.

      摘要:Objective To observe the effects of electroacupuncture(EA) on autophagosomes, unc-51-like autophage activating kinase 1(ULK1), Beclin1, and microtubule-associated protein light chain 3(LC3) expression, and synoviocyte ultrastructure in the synovial tissues of rheumatoid arthritis(RA) rats, and to explore the mechanism of EA in regulating the proliferation of synoviocytes via the autophagy pathway. Methods The SD rats were randomly divided into a normal group, a model group, a methotrexate(MTX) group, and an EA group, with 6 rats in each group. Following RA modeling with Freund's complete adjuvant, rats in the MTX group were treated with intragastric administration of 0.35 mg/kg MTX, twice a week, for 4 weeks, while those in the EA group received 20-min EA stimulation at “Zusanli”(ST36) and “Guanyuan”(CV4), once per day, for 4 weeks, with an interval of one day between weeks. The rat left hind toe volume was measured using the toe volume measuring instrument. HE staining was conducted for detecting the morphology of rat synovial tissues, followed by the observation of autophagosomes under the transmission electron microscope. The levels of serum interleukin(IL)-1 and tumor necrosis factor(TNF)-α were assayed by ELISA, and the protein expression levels of ULK1, Beclin1, and LC3 were detected by Western blot. Results Compared with the normal group, the left hind toe volume of the model group increased significantly(P<0.01), while serum IL-1 and TNF-α(P<0.01), synovial ULK1, Beclin1, and LC3 protein expression(P<0.01, P<0.05) up-regulated. HE stain and electron microscope showed obvious synovial hyperplasia, and doublemembrane autophagosomes scattering in the synoviocytes. The comparison with the model group showed that MTX and EA remarkably decreased the left hind toe volume(P<0.01), serum IL-1 and TNF-α(P<0.01), down-regulated the protein expression levels of ULK1 and LC3 in synovial tissues(P<0.05, P<0.01), and inhibited the synovial hyperplasia, with no obvious autophagosomes observed in the synoviocytes. The protein expression of ULK1 in the EA group was significantly lower than that in the MTX group(P<0.01). Conclusion EA alleviates the joint swelling and synoviocyte injury of RA rats possibly by regulating the expression of ULK1, LC3, and Beclin1 and inhibiting the synoviocyte autophagy and proliferation.

      摘要:Objective To observe the effect of acupuncture serum on the expression of microtubule associated protein-2(MAP-2) and nerve growth associated protein-43(GAP-43) in cultured hippocampal neurons of convulsive rats. Methods The acute convulsion model was induced by intraperitoneal injection of pentylenetetrazol in SD rats who were then randomized into model group and acupuncture group. Rats of the acupuncture group received manual acupuncture stimulation of “Baihui”(GV20) and “Dazhui”(GV14) for 30 min, once daily for 7 days. Then, the blood samples taken from the abdominal aorta of rats in the convulsion model and acupuncture groups were processed into serum samples, i.e. non-acupuncture serum and acupuncture se-rum. The primary-cultured hippocampal neurons of fetal rats were cultured for 10 days and then divided into normal extracellular fluid(normal) group, magnesium(Mg(2+)) free extracellular fluid group, acupuncture serum group and non-acupuncture serum group. At the 10 th day, the neurons in the normal group were cultured continuously in extracellular fluid for 3 h, and then cultured in DMEM/F12(1∶1) medium(planting fluid); neurons in the Mg(2+)) free extracellular fluid group, acupuncture serum group and non-acupuncture serum group. At the 10 th day, the neurons in the normal group were cultured continuously in extracellular fluid for 3 h, and then cultured in DMEM/F12(1∶1) medium(planting fluid); neurons in the Mg(2+) free group were cultured in magnesium-free fluid medium to induce epileptic-like discharge; neurons in the acupuncture serum group were cultured in the mixed medium of planting fluid and 10% acupuncture serum; and neurons in the non-acupuncture serum were cultured in the mixed culture medium of planting fluid and non-acupuncture serum(10%). At last, these neurons in the above-mentioned groups were cultured in the magnesium-free extracellular fluid continuously for 2, 12 and 48 h, respectively, followed by detecting the expression levels of MAP-2 and GAP-43 proteins at the 3 time points by using immunofluorescence and Western blot, separately.Results The rate of MAP-2 positive cells and protein expression at 2, 12 and 48 h, and the rate of GAP-43 positive cells and protein expression at 12 and 48 h in the hippocampal neurons were significantly down-regulated in the Mg(2+) free group were cultured in magnesium-free fluid medium to induce epileptic-like discharge; neurons in the acupuncture serum group were cultured in the mixed medium of planting fluid and 10% acupuncture serum; and neurons in the non-acupuncture serum were cultured in the mixed culture medium of planting fluid and non-acupuncture serum(10%). At last, these neurons in the above-mentioned groups were cultured in the magnesium-free extracellular fluid continuously for 2, 12 and 48 h, respectively, followed by detecting the expression levels of MAP-2 and GAP-43 proteins at the 3 time points by using immunofluorescence and Western blot, separately.Results The rate of MAP-2 positive cells and protein expression at 2, 12 and 48 h, and the rate of GAP-43 positive cells and protein expression at 12 and 48 h in the hippocampal neurons were significantly down-regulated in the Mg(2+) free group in contrast to the normal group(P<0.05,P<0.01). Compared to the Mg(2+) free group in contrast to the normal group(P<0.05,P<0.01). Compared to the Mg(2+) free group, the rates of MAP-2 and GAP-43 positive cells and protein expression at 2, 12 and 48 h were considerably up-regulated in the acupuncture serum group(P<0.05,P<0.01), but not in the non-acupuncture serum group(P>0.05).Conclusion Acupuncture serum can significantly up-regulate the expression of MAP-2 and GAP-43 proteins in hip-pocampal neurons, which may play a positive role in improving synaptic plasticity and neuronal damage in convulsion rats.

      摘要:Objective To explore the therapeutic effect of acupuncture combined with thunder-fire moxibustion on cervical spondylotic radiculopathy of wind-cold-damp type and its impacts on the conduction velocity of F wave of median nerve and ulnar nerve, as well as hypersensitive C-responsive protein(hs-CRP), interleukin 6(IL-6), IL-1β and tumor necrosis factor α(TNF-α) in serum. Methods A total of 94 patients with cervical spondylotic radiculopathy of wind-cold-damp type were randomly divided into a control group and a combined therapy group, 47 cases in each group. In the control group, acupuncture was applied to Fengchi(GV20), Tianzhu(BL10), Neck-Jiaji(EX-B5), Dazhui(GV14) and others, 30 min each time, once daily, 5 times a week, totally for 4 weeks. In the combined therapy group, on the base of the treatment as the control group, the thunder-fire moxibustion was adopted over GV20, EX-B5, GV14, Jianjing(ST21) and Tianzong(SI11), 20 min each time, once every other day, for 4 weeks in total. After the treatment, the curative effect was compared in the patients between two groups. The short-form McGill pain questionnaire(SF-MPQ), the neck specificity scale and the MOS 36-item short form health survey(SF-36) were scored. The conduction velocity of F wave in median nerve and ulnar nerve was detected by electromyography. The expression of hs-CRP was measured by immunoturbidimetry. IL-6, IL-1β and TNF-α in serum were determined by enzyme linked immunosorbent assay. Results The total effective rate in the combined therapy group was 95.7%(45/47), which was higher than 80.9%(38/47) in the control group(P<0.05). Compared with their own pretreatment, the scores of SF-MPQ, neck specificity scale and SF-36 were all obviously improved after treatment in the patients of either group(P<0.05), while the conduction velocity of F wave in median nerve and ulnar nerve was accelerated and hs-CRP, IL-6, IL-1β and TNF-α in serum were decreased(P<0.05). Compared with the control group, the improvements in the scores of SF-MPQ, neck specificity scale and SF-36 were more obvious(P<0.05), the increase of the conduction velocity of F wave in median nerve and ulnar nerve and the decrease of hs-CRP, IL-6, IL-1β and TNF-α in serum were more remarkable(P<0.05) in the combined therapy group. Conclusion Acupuncture combined with thunder-fire moxibustion relieves the clinical symptoms of cervical spondylotic radiculopathy of wind-cold-damp type.

      摘要:Objective To observe the effect and safety of injection of vitamin B1 into Zusanli(ST36) and Hegu(LI4) in the treatment of functional dyspepsia(FD). Methods A total of 100 FD patients were equally divided into medication group and acupoint injection group(n=50/group) according to a random number table. Patients in the medication group were ordered to take mosapride citrate tablets(5 mg) orally 30 min before each of the three meals, while those in the acupoint injection group received injection of vitamin B1 into ST36 and LI4, once every other day, three times a week. One week later, the clinical symptom scores, total effective rates, serum gastrin(GAS) and plasma motilin(MTL) contents, and gastric emptying rates between the two groups were compared, followed by the observation of adverse reactions. Two weeks' follow-up survey was conducted after the end of treatment, and the clinical symptom scores and total effective rates of the two groups were further compared. Results Compared with the data before treatment in the same one group, the clinical symptom scores and serum GAS contents of post-treatment as well as the follow-up symptom scores were all significantly decreased(P<0.05,P<0.01), while the plasma MTL levels and gastric emptying rates were obviously increased in both groups(P<0.01). Comparison between the two groups showed that the clinical symptom score, serum GAS content after the treatment and follow-up symptom score were considerably lower(P<0.01), but the total effective rate, plasma MTL, gastric emptying rate after the treatment and total effective rate of follow-up notably higher in the acupoint injection group than those in the medication group(P<0.01,P<0.05). Conclusion Injection of vitamin B1 into ST36 and LI4 is effective in improving symptoms of PD patients, which may be related to its functions in regulating the levels of GAS and MTL in blood, and facilitating gastrointestinal motility.

    • Roles of nociceptors in acupoint sensitization: recent advances

      摘要:Hyperalgesia and functional plasticity are the important components of acupoint sensitization. Reveal of the neuromechanism of acupoint sensitization may play a positive role in promoting the development of acupuncturology in the world. The nociceptors, including Aδ and C subtypes distributing in the acupoint region and target organs, are responsible for the transmission of signals of peripheral noxious stimuli and acupuncture-liking stimulation to the dorsal horns of the spinal cord and supraspinal levels. A previous study reveals that the C type nociceptors are involved in the acupoint sensitization. Recent studies indicate that there exists a subtype of mechanical responsiveness in the C type receptors, named “silent nociceptor” which is awa-kened when diseases occur, being very similar to the dynamic sensitization characteristics of acupoints. Hence, we, in the present review, make a discussion about the role of C-type silent nociceptor in the hyperalgesia and functional plasticity of the sensitized acupoint according to previous studies and recent advances, so as to provide more ideas and opportunities for the investigation on the scientific characteristics of acupoints.

      摘要:Objective A network Meta-analysis based on Bayesian theory was used to evaluate efficacy and safety of acupuncture and moxibustion in the treatment of dry eye disease(DED), so as to provide evidence-based research basis for clinical application. Methods Randomized controlled trials(RCT) for acupuncture and moxibustion treatment of DED published from the inception of database to November 25, 2020 were searched from PubMed, Embase, Cochrane Library, Web of Science, Sinomed, CNKI, Wanfang and VIP Database. Two reviewers independently screened the literatures, extracted the data. The quality of the included literature was evaluated, and network Meta-analysis was performed by using Stata14.0 and R4.0.3 software. Results A total of 71 literatures were identified, including 5 536 patients with DED, covering 11 different interventions. Network Meta-analysis showed that acupuncture+traditional Chinese medicine+artificial tears was the best treatment option in terms of the clinical effective rate, breakup time of tear film(BUT), Schirmer I test(SIT) with surface under cumulative ranking area value. Acupuncture+traditional Chinese medicine+artificial tears was better than artificial tears in the clinical effective rate(odds ratio[OR]=12.34, 95% confidence interval[CI][4.72, 36.89]), BUT(mean differenc[MD]=2.76, 95%CI[0.16, 5.40]), SIT(MD=4.76, 95%CI[1.23, 8.29]). Conclusion Acupuncture and moxibustion in the treatment of DED are generally better than artificial tears, and acupuncture-moxibustion combined with other traditional Chinese medicine therapy has the best effect.
