      摘要:Acupuncture-moxibustion, acupunctology and acupunctology discipline are three closely related concepts, with the first one being covered by the later one. In the present paper, we analyzed the formation, evolution and differentiation of acupunctology discipline and its significance. The knowledge system and academic institutions of acupuncture-moxibustion combine together to construct the acupunctology discipline. The introduction of interdisciplinary methods and academic perspectives is the core motivation of the formation, development and evolution of the acupunctology discipline, which not only enriches its problema-tic domains, but also interacts with the academically organizing force. The participation of interdiscipline promotes the broader and deeper development of the knowledge of acupunctology, and the knowledge expansion is also the inevitable result of infiltration of cross-disciplines. Hence, the maturity of the acupuntology discipline unavoidably leads to differentiation of the subject. At present, the acupunctology discipline can be divided into four main branches: clinical acupuncture-moxibustion, techniques, mechanisms and humanities of acupuncture and moxibustion. Currently, the acupunctology subject has formed a direction of expansion in multiple dimensions of the knowledge system due to continuous collision, nourishment and infiltration from many other related disciplines. However, because it is from the classical learning, its knowledge system inevitably has the characteristics of inertial stability, insufficient motivation and vitality in breaking the traditional framework. These issues now need being faced up to and being solved at the moment.

      摘要:In recent years, the newly appeared closed-loop neuromodulation technique composed of closed-loop controlled stimulation system and neuromodulation is the product of medicine and inter-disciplines. In medical clinical practice, the idea of closed-loop controlled stimulation can be seen everywhere. When both the patient and acupuncturist achieve the state of "Deqi" simultaneously, it is a kind of feedback adjustment process similar to that of the closed-loop stimulation system. Cross-functional multidisciplinary development is the future trend of medical science progress. The acupuncture-moxibustion discipline can draw lessons from the research experience and achievements of closed-loop neuromodulation techniques to explore the origin of the compatibility of meridians, acupoints, and needling manipulations.

      摘要:As one of the most widely used traditional Chinese medicine(TCM) treatment apparatus, the electric acu-point stimulation(such as electroacupuncture, transcutaneous acupoint electric stimulation, etc.) device plays an important role in improving the efficacy of acupuncture therapy and expanding its indications. The imbalance of human functional state has become an important factor affecting the health condition and quality of life. Acupuncture therapy has the advantage of regulating functional status of patients to restore the balance. Aiming at some problems of current acupuncture stimulation instruments in the stimulated depth, fuzzy range, lack of closed-loop feedback, insufficient combination with the theory of Chinese acupuncture-moxibustion, etc., we, in the present paper, put forward a new way of thinking that the classical theory of "Deqi" of acupuncture should be combined with the modern bio-electronic technology, multimode signal acquisition technology, real-time feedback control techno-logy, etc., in order to develop a series of effective, safe and comfortable integrated closed-loop feedback acupoint electrical sti-mulation devices. Interactions of multi-disciplines, reasonable coalesce between the medical treatment and theory, medical treatment and modern bioengineering techniques, as well as the integration of Chinese traditional and modern medicine will help promote the development of apparatuses of acupuncture-moxibustion for diagnosis and treatment of diseases. Such interdisciplinary efforts will make more contributions to the construction of healthy China and the health of all mankind.

    • Research progress in the integration of machine learning and acupunctology

      摘要:Machine learning is a newly emerged discipline. It develops rapidly and has been penetrated into all walks of life and made great achievements in medical field. The introduction of machine learning in the field of acupuncture and moxibustion should fully play the advantages of targeted therapy of acupuncture and moxibustion, optimize the resource allocation of traditional Chinese medicine and accurately serve the clinic rather than only limited to data mining and analysis. In the paper, through the analysis of the relevant literature in recent years, the combination of acupuncture-moxibustion with machine learning was discussed from the aspects of acupoint selection and prescription, clinical teaching application of acupuncture and moxibustion, the prediction on acupuncture curative effect, etc. Moreover, on the base of multidisciplinary intersection and by means of machine learning, the exploration was given on the future direction of acupuncture and moxibustion.

      摘要:Although the basic mechanism of acupuncture-moxibustion has been revealed from many aspects, there are still many shackles in the transformation of the related research achievements. The transformation of academic achievements of experimental acupunctology is an urgent issue to be solved at present. Network regulation is the basic action mode of acupuncture therapy. In the present paper, we proposed that the "acupuncture network drug" could carry a variety of effective active ingredients which may be the core component of network regulation of acupuncture therapy. The "exosomes", polyvesicle derived from the intracellular lysosomal microparticles invagination, contain complex RNAs and proteins and exist in the body fluids and function in secreting abundant activate substances to participate in intercellular communication, which is the research hotspot in the field of frontier life science in the world. They play an important role in the diagnosis and treatment of diseases, and drug development, etc.. Our studies using rats with adjuvant arthritis and mice with sepsis displayed that after intraperitoneal administration of serum exosomes derived from normal animals receiving acupuncture intervention, an acupuncture-like analgesic effect and an anti-inflammatory effect were achieved, respectively. It is thus possible that acupuncture network drugs could be developed from serum exosomes secreted by exosome autogenous living cells after acupuncture intervention by virtue of the characteristics of low immunogenicity and may have great advantages in drug development and modification. It is also expected to provide new ideas for the transformation of experimental research results and to in depth give explanations about the underlying mechanisms of acupuncture therapy.

      摘要:Objective To improve the accuracy of acupuncture manipulation modeling and inheritance, this article explores the feasibility of automatically classifying "twirling" and "lifting and thrusting", two basic acupuncture manipulations in science of acupuncture and moxibustion, with the computer vision technology. Methods A hybrid deep learning network model was designed based on 3 D convolutional neural network and long-short term memory neural network to extract the spatial-temporal features of video frame sequences, which were then input into the classifier for classification. Results The model discriminated between "twirling" and "lifting and thrusting" manipulations in 200 videos, with the training and verification accuracy reaching up to 95.4% and 95.3%, respectively. Conclusion This computer vision-based acupuncture manipulation classification system provides an effective way for the data extraction and inheritance of acupuncture manipulations.

      摘要:In learning and evaluation of acupuncture manipulations, there are lack of quantitative physical parameters on exertion strength, duration and direction of acupuncture technique at present. Based on the tactile parameters collected during "twirling" and "lifting-thrusting" of needling, a kind of array polyvinylidene fluoride(PVDF) tactile sensor was designed. Followed by, a window segmentation method for tactile signal was proposed and the time domain features of the window were extracted. Finally, an identification method of acupuncture manipulation based on FCM(Fuzzy C-Means) was constructed. Through the experiment, it was proved that this sensor can effectively identify the four kinds of basic acupuncture manipulations, i.e. reinforcing by twirling and rotating, reducing by twirling and rotating, reinforcing by lifting and thrusting and reducing by lifting and thrusting and it was conductive to the quantification and dissemination of acupuncture manipulations.

      摘要:For a long time, Chinese medicine has attached great importance to "pulse diagnosis for patients before acupuncture treatment", which emphasizes the close relationship between pulse diagnosis and acupuncture. Pulse diagnosis information is closely related to acupuncture research and runs through the whole process of diagnosis and treatment. However, the lack of repeatable and quantifiable real-time detection means of pulse diagnosis information has affected the application of pulse diagnosis in acupuncture research. Photo plethysmo graphy(PPG) technology has the advantages of convenient acquisition, low price, non-invasive, easy to use and so on. Under the guidance of TCM diagnosis theory, this paper discussed the principle and characteristics of fingertip volumetric pulse wave, studied its relationship with Cunkou pulse diagnosis, developed a new fingertip blood volumetric pulse wave measuring instrument, explored the acquisition and measurement methods of volumetric pulse wave signal, and proposed the relationship between the measuring instrument and acupuncture research. It can provide basic tools and data analysis and support for acupuncture research.

    • An intelligent glass cupping device with closed-loop control

      摘要:As the living standard improves and population aging continues, the traditional cupping manipulations fail to meet the ever-growing demand for high-quality medical and health services. Aiming to address such shortcomings of traditional cupping manipulations as difficulty in achieving real-time temperature control, unsatisfactory negative pressure effect and inconve-nience to observe the internal situation, we have developed a glass cupping device with closed-loop control that could automatically integrate the acupoint-symptom analysis models. This device, composed of two cups, boasts high safety, good sealing and sound economic efficiency. The PT100 thermocouple probe and the pressure sensor equipped with P89 C668 single chip microcomputer enable the multimodal integration of temperature and pressure, whose main parameters can be automatically displayed on the LED screen by the built-in data processing device. This glass cupping device with closed-loop control is an updated design for acupuncture and moxibustion based on multidisciplinary resources.

      摘要:Objective To evaluate the efficacy and safety of electrical stimulation at the rabbit sciatic nerve trunk with the body-insulated acupuncture needle whose body is painted with insulating material. Methods Eighteen male New Zealand rabbits were randomized into the body-insulated acupuncture needle(BIAN), the general acupuncture needle(GAN), and the blank control groups, with 6 rabbits in each group. The rats' sciatic nerve trunks in BIAN and GAN groups were stimulated by electroacupuncture with the body-insulated acupuncture needle(only allowing the uncoated needle handle and tip to conduct electricity) and the general acupuncture needle, separately. The current intensity was recorded when regular plantarflexion reflexes(sciatic nerve effector reflexes) were observed in the rabbit's foot. The pathological changes of the sciatic nerve at the acupuncture site were observed by H.E. staining, and the ultrastructural changes of the sciatic nerve trunk were observed by transmission electron microscope. Results The intensity of the current causing the regular plantar flexion reflection in BIAN group([0.29±0.07] mA) was significantly lower than that in the GAN group([0.86±0.08] mA, P<0.01). H.E. staining revealed nerve axon degeneration, forming eosinophilic bodies, nerve fiber edema, and focal loss of myelin sheath in the GAN group. While the nerve fiber damage was not obvious, and axons were only degenerated in a few areas in the BIAN group. Transmission electron microscopy observations showed that the nerve myelin sheath structure was separated, the layers were arranged disorderly and bubbled in the GAN group. while the nerve myelin sheath structure of the BIAN group was normal, and it presents a concentric circle-like light and dark lamellar structure, with fewer myelin vacuoles and fissures, only a small part of the mitochondria, microfilaments, and microtubules of the nerve axons were abnormal, and the overall vacuole-like degeneration was significantly reduced, with few of the myelinated fibers were slightly degenerated, and axonal disease was not obvious. Conclusion Insulated acupuncture needle is more accurate and safer than ordinary acupuncture needle for electrical stimulation of rabbit sciatic nerve trunk, and the required electric current intensity is smaller.

      摘要:Objective To observe the changes of microvascular structure of acupoints caused by myocardial ischemia, so as to explore the application of photoacoustic imaging technology in the research of acupoint sensitization. Methods Twelve BALB/c mice were randomly divided into normal, sham operation and acute myocardial ischemia(AMI) model groups, with 4 mice in each group. AMI model was established by ligating the left anterior descending coronary artery. Electrocardiogram(ECG) was recorded by physiological signal acquisition system at 12 h and on the 14(th) day after modeling, and serum cardiac troponin T(cTnT) and creatine kinase isoenzyme MB(CK-MB) levels were detected by enzyme-linked immunosorbent assay. The microvascular structure changes of acupoints "Feishu"(BL13), "Jueyinshu"(BL14), "Quze"(PC3) and "Chize"(LU5) were observed by photoacoustic imaging technology, and distance(DM), inflection count metric(ICM), sum of angle metric(SOAM)and microvessel density(MVD) were calculated by microvascular quantification algorithm. Results Compared with the normal and sham operation groups, the ST segment of ECG was obviously elevated, serum cTnT and CK-MB were significantly increased in AMI model group at 12 h and on the 14(th) day after modeling, and serum cardiac troponin T(cTnT) and creatine kinase isoenzyme MB(CK-MB) levels were detected by enzyme-linked immunosorbent assay. Compared with the normal group, the ICM of BL14 was significantly increased in AMI model group on the 7(th) day after AMI. Compared with the normal group, the ICM of BL14 was significantly increased in AMI model group on the 7(th) day after AMI(P<0.05). There were no significant changes in DM, ICM, SOAM and MVD at other acupoints on the 7(th) day after AMI(P<0.05). There were no significant changes in DM, ICM, SOAM and MVD at other acupoints on the 7(th) and 14(th) and 14(th) day(P>0.05) among the three groups.Conclusion The change of ICM may be one of the characteristics of acupoint sensitization and photoacoustic imaging technology can be used to study the structure of acupoint microvessels.

      摘要:Acupuncture is effective for the management and treatment of pain related diseases. At present, the patients' subjective evaluations were often used to measure the effect of acupuncture analgesia in clinical research, but it lacks objectivity and accuracy. In recent years, some studies had tried to analyze the magnetic resonance images of patients' brains using magne-tic resonance imaging and machine learning methods before and after acupuncture intervention, so as to identify specific neural markers. These markers not only helped explain the brain mechanism of acupuncture analgesia, but also provided objective indicators for the analgesic effect of acupuncture. This article analyzes the significance and feasibility of pain biomarkers research based on magnetic resonance imaging and machine learning technology, summarizes its research status in acupuncture analgesia, and makes suggestions in the future study.

      摘要:The network analysis method based on brain connectomics is an important entry point to explore the working mechanism of brain and is also the current trend of researches on acupuncture stimulation-induced changes of neuroimages. We, in the present review, summarized the common network analysis methods for exploring the underlying mechanisms of brain network-mediated regulatory effects of acupuncture interventions. Moreover, combining the current research development and our team's previous research findings, we extracted the characteristics of targeting, conditionity and dynamic of regulatory effects of acupuncture-activated brain network, and put forward our prospects about the future research from the aspects of new scanning modes(for instance, multimodal data acquisition of magnetic resonance, electroencephalogram, near infrared spectrum, etc.), network analysis(such as Granger causality analysis, complex network measures, whole-brain connectivity dynamics tracking, high-order resting-state functional connectivity analysis, etc.) and experimental research paradigms(for example, introduce of transcranial magnetic stimulation induced transient changes of brain functional activity, machine learning approach, etc.).

    • Acupuncture-material science: a new branch of acupuncturology

      摘要:Combining the current development of material science and acupunctology, we propose a new concept of "acupuncture-material science" as the new branch of acupunctology. We also hold that this new branch may be in the future the important direction of acupuncture-moxibustion instrument innovation. The future carding and development of the connotations and epita-xial contents of this discipline, the classification, applied range, research and development of the materials, as well as the related standard formulation, etc. should be paid more attention and improved in order to make greater contribution to the inheritance and innovation of acupunctology in the new era.

      摘要:Multidisciplinary intersection has gradually become a major source of scientific and technological innovation at present. But, how to leap over the discipline of acupuncture and moxibustion to promote the development of it is still a bottleneck problem. In the paper, based on the application of physical technology in acupuncture-moxibustion research, regarding the integration of traditional acupuncture and moxibustion with electrics, optics, acoustics and magnetics, the significance of it in theoretic study has been reviewed, as well as the current situation of its clinical application. It is anticipated to provide the ideas for the reform and development of acupuncture and moxibustion and the combination and innovation of acupuncture-moxibustion with other disciplines.

      摘要:At present, acupuncture and moxibustion widely spread around the world, and its development is closely rela-ted to multidisciplinary integration. Interdisciplinary approaches are of great concern to the innovation and promotion of acupuncture-moxibustion therapy. Acupuncture and moxibustion, combined with optics, electrics, magnetics, infrared ray, microwave and other technologies, create the advanced and practical instrument for diagnosis and treatment of acupuncture and moxibustion and have made important contribution to clinical and teaching work. In the paper, focusing on the unique advantages of the science of acupuncture and moxibustion, the application of interdisciplinary approaches was explored to the promotion of acupuncture-moxibustion technology in its innovation and development, while the existing problems and countermeasures are further considered in the translational process of acupuncture-moxibustion technology to clinical application.

      摘要:The theory of traditional Chinese medicine believes that meridian-collateral(jingluo) is an important system to maintain life and participates in the formation and cure of disease. Exploring the biological connotation of meridian-collateral is the key content in the modern research of traditional Chinese meridian. For more than 60 years, scholars at home and abroad have carried out the researches of meridian-collateral from the fields of biomedicine(e.g. physiology, anatomy and histomorphology), imaging, physics, chemistry, computer technology, etc. Meridian-collateral research in modern times demonstrates a multi-disciplinary historical development and provides multi-perspective scientific information for understanding meridian-collateral. The meridian-collateral research in future needs more accurate multidisciplinary intersection, which will be the light of hope to reveal the biological connotation of meridian-collateral.

      摘要:The meridian-collateral theory introduced in the classic book Huangdi Neijing(The Yellow Emperor's Canon of Internal Medicine,) has been being guiding the clinical diagnosis and treatment of diseases and health preservation by using traditional Chinese medicine(TCM) for thousands of years. However, despite of being the core theory of TCM, the meridian-colla-terals and their structural basis have not been scientifically clarified, which has become the "bottleneck" of the inheritance and innovation of TCM. It was reported in 2020 that the micro-channels formed by collagen fiber bundles in the skin and their contained components and the newly found interstitial cell, Telocyte, may be considered to be the ultra-micromorphological basis including their connection with the internal organs. In the present paper, we employed the Telocyte network and its relationships with other structures and cells to interpret, at the cellular level, various meridian hypotheses including 1) neurotheory, 2) humoral circulation theory, 3) biological field hypothesis, 4) fascia theory, 5) immune theory, 6) mast cell theory, 7) meridian-qi, and 8) elusive phenomenon of meridian. The absence of meridian-collaterals in morphology may be related to the difficulty of laboratory sample preparations of Telocyte processes in the continuity and completeness. We also proposed that the future research strategies should be aimed at revealing the morphological structure(including in vitro studies of the isolated and cultured Telocytes), establishment of relevant disease model, and clarification of the interrelation between the Telocyte network and the systems of nerve, blood circulation, immune, endocrine, and connective tissue.

      摘要:At present, the breakthrough of the key techniques of artificial intelligence(AI), including image recognition, deep learning, neural network, Robot technique, etc., greatly promote the development of discipline crossing and medicine. In the present paper, we make an in-depth discussion about the future application of inter-discipline techniques of acupuncture-moxibustion and AI. We think that some of the current instruments have been part of the new acupuncture-moxibustion devices and may have the potential to intersect with the AI discipline. Relying on these existing devices and those of meridian detection, we can obtain relatively objective data, and further conduct meridian-syndrome differentiation and big data collection to possibly realize remote medical treatment. In addition, we may also develop an AI system for studying the underlying mechanisms of acupuncture and moxibustion therapies. Nevertheless, there still exist a lot of problems and challenges in clinical, teaching and scientific researches. It is recommended that the discipline of acupuncture-moxibustion should be intersected with the AI subject and formulate appropriate development strategies, promoting a faster and better development of acupuncturology.

    • Progress of researches in acupotomy visualization under ultrasound

      摘要:Traditional acupotomy treatment can easily cause re-injury of soft tissues, and its safety is gradually being valued. With the development of imaging technology, visualized acupotomy operations are increasingly used in clinical practice. Starting from the development and evolution of acupotomy, the authors summarize the advantages and clinical application of ultrasound-guided acupotomy in cervical spondylosis, scapulohumeral periarthritis, lumbar disc herniation and knee osteoarthritis.
