      摘要:Objective To observe the effect of electroacupuncture(EA) on miRNA-126-3 p and mammalian target of rapamycin(mTOR)/hypoxia-inducible factor-1α(HIF-1α) signaling pathway in rats with cerebral ischemia(CI), so as to explore the underlying mechanism of EA on angiogenesis. Methods Male SD rats were randomly divided into control group, model group, EA group and EA+inhibitor group(inhibitor group), which were further divided into 3, 7 and 14 d subgroups, with 12 rats in each sub-group. The CI model was established by occlusion of the middle cerebral artery. EA(2 Hz/20 Hz, 0.5 mA) was applied to “Dazhui”(GV14), “Baihui”(GV20) for 20 min, once daily for 14 days at most. Rats of the inhibitor group were given an intraperitoneally injection of mTOR inhibitor(0.1 mg/mL, 0.3 mg/kg) before daily EA. The neurological function was evaluated by modified neurological severity score(mNSS). The ultrastructure of cortical neurons and microvascular endothelial cells in ischemic penumbra was observed by transmission electron microscope, and the microvessel density(MVD) of cortical endothelium in ischemic penumbra was detected by immunohistochemistry. Western blot and quantitative real-time PCR were used to detect the protein and mRNA expression of mTOR, HIF-1α and the expression of miR-126-3 p in the cortex of ischemic penumbra, respectively. Results After modeling, compared with the control group at the same time point, the mNSS of the model group was increased(P<0.01), and decreased over time(P<0.01). The cortical neurons and brain microvascular endothelial cells in the ischemic penumbra were edema, and the cell structure was damaged obviously in the model group.The MVD value and the expressions of mTOR、HIF-1α proteins and mRNAs were increased(P<0.01), while the expression of miR-126-3 p decreased(P<0.01) in the model group relative to the control group. Compared with the model group at the same time point, the mNSS of both intervention groups was significantly reduced(P<0.01, P<0.05), the neuron and cerebral microvascular structure improved to varying degrees, and the MVD value, the expressions of mTOR and HIF-1α protein and mRNA, and the expression of miR-126-3 p of the two treatment groups were increased(P<0.01, P<0.05) at all time points(excep MVD at day 7 in the inhibitor group). Compared with the EA group at the same time point, MVD, the expressions of mTOR, HIF-1α proteins and mRNAs and miR-126-3 p in the inhibitor group were all decreased(P<0.05,P<0.01). Compared with the group itself at 4 hours after modeling and day 3 and day 7, the mNSS was decreased at day 14(P<0.01) in the model, EA and inhibitor groups. Compared with the group itself at day 3, the MVD value and the expression of mTOR protein were increased at day 7 and day 14 in the model, EA and inhibitor groups(P<0.01, P<0.05). Compared with the group itself at day 3 and day 7, the expression of mTOR mRNA and miR-126-3 p were up-regulated at day 14 in the model and EA groups(P<0.01, P<0.05).Compared with the group itself at day 3, the mRNA expressions of mTOR and HIF-1α were increased at day 7 and day 14(P<0.01, P<0.05) in the inhibitor group. Conclusion EA at GV14 and GV20 can alleviate neurological deficit and improve angiogenesis in rats with CI, which may be related with its effect in up-regulating the expression of mTOR and HIF-1α, improving activation of miR-126-3 p in the cortex of ischemic penumbra.

      摘要:Objective To study the protective effect of electroacupuncture(EA) at “Biao”-acupoints, “Fenglong”(ST40) and “Zhongwan”(CV12), used for treating symptoms of the disease, and “Ben”-acupoints, “Zusanli”(ST36) and “Guanyuan”(CV4), for treating the root cause of the disease on oxidative stress injury of renal mitochondria through SIRT1/PGC-1α signal pathway in rats with diabetic nephropathy(DN). Methods A total of 33 male Wistar rats were randomized into normal(n=10), model(n=12) and EA(n=11) groups. The DN model was established by feeding the rats with high-sugar and high-fat diet for 6 weeks combined with streptozotocin(STZ, 35 mg/kg, i.p.). EA(4 Hz/60 Hz, 1 mA)was applied to ST36-ST40 and CV4-CV12 for 15 min, once every other day for 8 weeks. The rats' body weight was recorded, urine in 24 hours(24-h UP) was collected to measure the urine protein level, and the fasting blood glucose(FBG) level detected by using a glucometer. The levels of serum glycosylated hemoglobin(HbA1 c), creatinine(Scr) and urea nitrogen(BUN) were assayed using immunoturbidi-metry, picric acid method and urease method, respectively, and those of serum triglyceride(TG), total cholesterol(TC), low density lipoprotein cholesterol(LDL-C) and high density lipoprotein cholesterol(HDL-C) detected using an automatic biochemical analyzer. The kidney tissue was collected for assaying the activity of superoxide dismutase(SOD) with xanthine oxidase method, glutathione(GSH) activity with dithio-dinitrobenzoic acid method, catalase(CAT) activity with ammonium molybdate spectrometric method, and malondialdehyde(MDA) content with thiobarbituric acid method. Histopathological changes of the kidney tissue were observed by microscope after hematoxylin-eosin staining(HE), periodate Schiff staining(PAS) and Masson staining, separately, and its subcellular structure was observed under transmission electron microscopy. The expression levels of renal SIRT1 and PGC-1α mRNAs and proteins were detected by quantitative real-time PCR and Western blot, and the immunoactivity of renal α-smooth muscle actin(α-SMA), and immunofluorescence density of renal collagen Ⅰ(Col Ⅰ), collagen Ⅳ(Col Ⅳ) and fibronec-tin(FN) detected by immunohistochemistry and immunofluorescence assay, respectively.Results Compared with the normal group, the levels of FBG, HbA1 c, BUN, Scr, 24-h UP, TG, TC, LDL-C, MDA, α-SMA, Col Ⅰ, Col Ⅳ and FN proteins were significantly increased(P<0.01), and the levels of body weight, HDL-C, SOD, GSH, CAT, SIRT1 and PGC-1α mRNAs and proteins were decreased in the model group(P<0.01, P<0.05). In comparison with the model group, the levels of FBG, HbA1 c, BUN, Scr, 24-h UP, TG, TC, LDL-C and MDA, and the expressions of α-SMA, Col Ⅰ, Col Ⅳ and FN proteins were markedly down-regulated(P<0.05, P<0.01), and those of body weight, HDL-C, SOD, GSH, CAT, and the expressions of SIRT1 and PGC-1α mRNAs and proteins significantly up-regulated(P<0.01, P<0.05) in the EA group. HE staining showed mesangial dilatation, glomerular hypertrophy and mesangial matrix accumulation; PAS staining showed an increase of the glomerular extracellular matrix deposition; and Masson staining displayed an enhancement of glomerular fibrosis and interstitial space expansion; and electron microscope revealed foot process fusion, basement membrane thickening and organelle injury in the rat's kidney of the model group, which were relatively milder in the EA group. Conclusion EA of ST36-ST40 and CV4-CV12,“Biao-Ben” acupoints combination, can alleviate oxidative stress and mitochondrial dysfunction in DN rats, which may be associated with its functions in up-regulating SIRT1/PGC-1α signaling, and decreasing renal fibrosis.

      摘要:Objective To observe the effect of heat-reinforcing needling on the expression of serum inflammatory factors and autophagy of knee synovial tissue in rheumatoid arthritis(RA) rabbits with cold syndrome, so as to explore its mechanism of anti-inflammatory in the treatment of RA. Methods Fifty rabbits were randomly divided into normal, model, heat-reinforcing needling, inhibitor and agonist groups(n=10 rabbits in each group). The model of RA with cold syndrome was established by Freund's adjuvant and ovalbumin mixed solution injection combined with freezing and wind-cold dampness method. Heat-reinforcing needling was applied at “Zusanli”(ST36) for 30 min, once a day for 14 days. Rabbits of the inhibitor and agonist groups were given intraperitoneally injected with autophagy inhibitor 3-methyladenine(3-MA) or autophagy agonist rapamycin, once every 2 days for 7 days. The knee circumference and skin temperature of the rabbits in each group were measured. Color doppler ultrasonography was applied to examine the synovial membrane, joint effusion and blood flow signals in the knee joints of the rabbits in each group. Serum tumor necrosis factor(TNF)-α, interleukin(IL)-1β, IL-6 and C-creactive protein(CRP) were detected by ELISA. Transmission electron microscopy was applied to observe the ultrastructure and autophagosomes of synovial cells. The protein expressions of autophagy-related protein Atg5, serine/threonine protein kinase-dysregulated 51-like kinase 1(ULK1), microtubule-associated protein light chain 3 B(LC3 B), and Beclin-1 were detected by Western blot. Fluorescence quantitative PCR was used to detect the mRNA expressions of NOD-like receptor 3(NLRP3) and nuclear factor-κB(NF-κB). Results Compared with the normal group, the circumference of the knee joint was increased(P<0.01), the skin temperature was decreased(P<0.01), the knee joint synovium was thickened and the blood flow signal was abundant, the contents of serum TNF-α, IL-1β, IL-6, and CRP were increased(P<0.01), the protein expressions of Atg5, ULK1, Beclin-1 and LC3 BⅡ/LC3 BⅠof synovial tissue were significantly decreased(P<0.01), the mRNA expressions of NLRP3 and NF-κB were increased(P<0.01) in the model group. In comparison with the model and inhibitor groups, the circumference of the knee joint was decreased(P<0.01), whlie the skin temperature was increased(P<0.01), the synovial membrane became thinner and the blood flow signal was wea-kened, the contents of TNF-α, IL-1β, IL-6 and CRP were decreased(P<0.01), the protein expressions of Atg5, ULK1, Beclin-1 and LC3 B Ⅱ/LC3 B Ⅰ were increased(P<0.01), and the mRNA expressions of NLRP3 and NF-κB were decreased(P<0.01) in the heat-reinforcing needling and agonist groups. Conclusion Heat-reinforcing needling can alleviate the inflammatory response of the knee joint synovium in RA rabbits with cold syndrome, which may be related to its function in enhancing the autophagy activity of synovial cells and inhibiting the synthesis and release of inflammatory factors TNF-α, IL-1β, IL-6 and CRP.

      摘要:Objective To observe the effect of acupuncture preconditioning combined with PI3K blocker LY294002 on the expression of PI3K and Akt proteins and genes in the lung tissue and the contents of serum IL-12 and IL-13 in asthmatic rats, so as to explore its preprotective mechanism underlying improving asthma. Methods Sixty male Wistar rats were randomly divided into blank control, model, acupuncture pretreatment + blank, acupuncture pretreatment, acupuncture pretreatment + LY294002 and LY294002 groups(n=10 in each group). The asthma model was established by intraperitoneal injection of mixture solution of OVA and Al(OH)_3 and followed inhalation of 1%OVA for 30 min, once daily for 7 days. Rats of the blocker groups received inhalation of atomized LY294002 solution for 30 min before inhalation of 1% OVA, and acupuncture was applied to “Feishu”(BL13), “Dazhui”(GV14) and “Fengmen”(BL12) for 20 min, once daily for 7 days before modeling. H.E. staining was used to assess histopathological changes of the lung tissue, and ELISA was used to detect the contents of serum IL-12 and IL-13. The immunoactivity of PI3K and Akt and expression of Akt mRNA of the lung tissue were detected by using immunohistochemistry and fluorescence quantitative real-time PCR, separately.Results Compared with the blank control group, the content of serum IL-12 was significantly decreased(P<0.01), and the content of serum IL-13, the expression levels of PI3K, Akt protein and Akt mRNA were remarkably increased(P<0.01) in the model group. In comparison with the model group, the content of serum IL-12 in the pretreatment, pretreatment + LY294002 and LY294002 groups was significantly increased(P<0.01, P<0.05), while the content of IL-13 and the expression levels of PI3K, Akt protein and Akt mRNA were considerably decreased(P<0.01, P<0.05) in the acupuncture pretreatment, acupuncture pretreatment+LY294002 and LY294002 groups. The therapeutic effect of acupuncture pretreatment+LY294002 was obviously superior to that of simple acupuncture pretreatment and LY294002(except PI3K and Akt in the LY294002 group) in up-regulating serum IL-12 level, and in down-regulating serum IL-13, and PI3K and Akt protein levels in the lung tissue(P<0.01). H.E. staining showed severe inflammatory factor infiltration in the bronchus and pulmonary interstitium, and obvious bronchial lumen narrowing with increased exudate in rats of the model group, which was relatively milder in rats of the acupuncture pretreatment, acupuncture pretreatment+LY294002 and LY294002 groups. There were no significant diffe-rences between blank control and pretreatment+blank groups in all of the above indicators(P>0.05). Conclusion Acupuncture preconditioning can inhibit airway inflammation in asthmatic rats, which may be associated with its functions in down-regulating the levels of pulmonary PI3K and Akt and serum IL-13 and up-regulating the content of serum IL-12. Acupuncture preconditioning combined with LY294002 has the best effect.

      摘要:Objective To observe the effect of electroacupuncture(EA) at “Tianshu”(ST25) on nitrergic neurons in jejunum and distal colon in type 2 diabetic rats, so as to explore its mechanism of regulating different intestinal segments. Methods Twenty-four SD rats were randomly divided into control, model and EA groups(n=8 in each group). The diabetes model was established by intraperitoneal injection of streptozotocin(35 mg/kg) and high-sugar and high-fat diet for 2 weeks. EA(2 Hz/15 Hz, 2 mA) was applied to bilateral ST25 for 20 min, once a day, 6 days a week for 4 weeks. The intestinal motility was evaluated by observing the first red stool excretion time and the distal colon bead excretion time. HE staining was used to observe the histological changes of jejunum and distal colon. The positive expression and protein expression of intestinal total neuronal marker protein gene product 9.5(PGP9.5) and neuronal nitric oxide synthase(nNOS) in jejunum and distal colon were detected by immunofluorescence staining and Western blot, respectively. Results After modeling, the blood glucose was significantly increased(P<0.01), the first red stool excretion time and the distal colon bead excretion time were shortened(P<0.01), the expression levels of PGP9.5 and nNOS in jejunum and distal colon were decreased(P<0.01) in the model group relevant to the control group. After treatment, compared with the model group, the blood glucose was decreased(P<0.01), the first red stool excretion time and the distal colon bead excretion time were prolonged(P<0.01, P<0.05), and the expression levels of PGP9.5 and nNOS in jejunum and distal colon were increased(P<0.05, P<0.01) in the EA group. HE staining showed disordered structure in intestinal mucosa of the jejunum and distal colon, and reduction of the number of goblet cells in the model group, which was relatively milder in the EA group. Conclusion EA can effectively improve the intestinal mucosal damage and restore intestinal motor function in type 2 diabetic rats, which may be related to its function in regulating the number of nitrergic neurons in the intestinal nervous system.

      摘要:Objective To explore the protective effect and molecular mechanism of electroacupuncture(EA) preconditioning on renal injury in type 2 diabetic rats. Methods Fifty male Wistar rats were randomly divided into control, model, EA, EA+inhibitor, and inhibitor groups, with 10 rats in each group. Diabetes model was established by high fat and high glucose diet and intraperitoneal injection of streptozotocin(40 mg/kg). EA(2 Hz, 1 mA) preconditioning was applied to “Guanyuan”(CV4), “Zhongwan”(CV12), bilateral “Zusanli”(ST36) and “Fenglong”(ST40) for 15 min, once every other day for 8 weeks. Rats of the inhibitor and EA+inhibitor groups were given intraperitoneal injection of 3-TYP(50 mg/kg) once every other day for a total of 3 times. The body weight, kidney mass, and renal index were recorded. The contents of urine microalbumin(ALB), 24 h urine 8-hydroxydeoxyguanosine(8-OHdG) and activities of reactive oxygen species(ROS), superoxide dismutase(SOD), catalase(CAT), glutathione glycine peroxidase(GSH-Px) in kidney were detected by ELISA. The activities of mitochondrial respiratory chain enzyme complex(RCCⅠ—RCCⅣ) in kidney were detected using spectrophotometric method. HE staining, Masson staining and transmission electron microscopy were used to observe the changes of renal structure. The protein and mRNA expressions of silent information regulator 3(Sirt3), manganese superoxide dismutase(MnSOD) in kidney were detected by Western blot and quantitative real-time PCR, respectively. Results After modeling and compared with the control group, the contents of ALB, the renal index, activity of ROS and content of 8-OHdG, and the renal collagen volume fraction(CVF) were increased(P<0.01), while the activities of SOD, CAT and GSH-Px, RCCⅠ—RCCⅣ, and the mRNA and protein expressions of Sirt3 and MnSOD were decreased(P<0.01). After the treatment and compared with the model group, the contents of ALB, the renal index, ROS, 8-OHdG, and the CVF were decreased in the EA group(P<0.01, P<0.05), while the activities of SOD, CAT, GSH-Px, RCCⅠ—RCCⅣ, and Sirt3 and MnSOD expression levels were increased(P<0.01, P<0.05);the RCCⅡ activity and the expression level of MnSOD mRNA were increased(P<0.05) in the EA+inhibitor group; the ALB and 8-OHdG contents and the CVF in the inhibitor group were increased(P<0.05), while the activity of SOD, and Sirt3 and MnSOD expression levels were decreased(P<0.05). In comparison with the EA group, the contents of ALB, the renal index, activities of ROS and 8-OHdG contents, and the CVF were increased(P<0.05, P<0.01), activities of SOD, CAT and GSH-Px and RCCⅠ and RCCⅡ, and the mRNA and protein expressions of Sirt3 and MnSOD were decreased(P<0.05, P<0.01) in both EA+inhibitor group and inhibitor group, whereas the activities of RCCⅢ and RCCⅣ were decreased in the inhibitor group(P<0.05). The therapeutic effect of inhibitor was notably inferior to that of EA+inhibitor in decreasing ALB and 8-OHdG contents, and CVF(P<0.01), and in up-regulating SOD and RCCⅡ activities, Sirt3 and MnSOD expression levels(P<0.01, P<0.05). Conclusion EA preconditioning can increase the expressions of renal Sirt3 and MnSOD in type 2 diabetic rats, thereby reducing the oxidative stress response and protecting the kidneys.

      摘要:Objective To observe the effect of electroacupuncture(EA) combined with Zhuang-medicine-thread moxibustion on expression of apoptosis-related factors in gastric antrum of diabetic gastroparesis(DGP) rats, so as to explore its mechanism underlying improvement of DGP. Methods Male SD rats were randomly divided into normal, model, medication, EA, Zhuang-medicine-thread moxibustion(moxibustion) and EA+moxibustion(combination) groups(12 rats in each group). The DGP model was established by intraperitoneal injection of streptozotocin(STZ). Rats of the medication group were treated by gavage of 0.15 mg/mL mosapride citrate suspension(10 mL/kg). EA(10 Hz/50 Hz, 2 mA, 20 min) or Zhuang-medicine-thread moxibustion(3 cones) was applied to “Zhongwan”(CV12), bilateral “Neiguan”(PC6) and bilateral “Sanyinjiao”(SP6) of the related groups, once a day for 3 weeks. The blood glucose, gastric emptying rate and intestinal propulsion rate of rats were measured. The apoptosis index of gastric antrum cells were observed by TUNEL staining. The protein and mRNA expressions of Caspase-3, B-cell lymphoma/leukemia-2(Bcl-2) and Bcl-2 associated X protein(Bax) in gastric antrum were detected by Wes-tern blot and real-time quantitative PCR, respectively. Results Compared with the normal group, the blood glucose, the apoptosis index, the protein and gene expressions of Caspase-3 and Bax were significantly increased(P<0.01), and the gastric emptying rate, intestinal propulsive rate, the protein and gene expressions of Bcl-2 were considerably decreased(P<0.01) in the model group. In contrast to the model group, the blood glucose in the EA, moxibustion and combination groups, the apoptosis index in the 4 treatment groups, as well as Caspase-3 protein, Bax protein and mRNA expressions in the medication, EA and combination groups, Caspase-3 protein and mRNA, Bax mRNA expressions in the moxibustion group were significantly decreased(P<0.01, P<0.05); while the gastric emptying rate and intestinal propulsive rate in the 4 treatment groups, and Bcl-2 protein and mRNA expressions in the medication and combination groups, Bcl-2 mRNA expressions in the EA and moxibustion groups were obviously increased(P<0.01). The effects of EA+moxibustion were significantly superior to those of simple EA, moxibustion or medication in increasing gastric emptying rate and intestinal propulsive rate, and in lowering blood glucose(P<0.05, P<0.01). And the effects of the combination treatment were better than those of EA in lowering Caspase-3 protein and Bax mRNA expressions(P<0.01), and in increasing Bcl-2 protein and mRNA expressions(P<0.05, P<0.01). Also the effects of the combination treatment were better than those of moxibustion in lowering the apoptosis index, Caspase-3 protein, and Bax protein and mRNA expressions(P<0.01, P<0.05), and in increasing Bcl-2 protein expression(P<0.05). Conclusion EA combined with Zhuang-medicine-thread moxibustion can reduce blood glucose and improve gastrointestinal motility in DGP rats, which may be related to its effect in regulating of Caspase-3, Bax and Bcl-2 expression.

      摘要:Objective To observe the effects of thunder-fire moxibustion on the visual analogue scale(VAS) score, Young's modulus of multifidus and 6 m walking speed in the patients of osteoporosis with low skeletal muscle mass. Methods Sixty patients of osteoporosis with low skeletal muscle mass were randomly divided into a medication group(30 cases) and a medication+thunder-fire moxibustion group(30 cases). In the medication group, caltrate was prescribed for oral administration, 2 tablets/day(600 mg/tablet), for 4 weeks. In the medication+thunder-fire moxibustion group, on the base of oral administration with caltrate, thunder-fire moxibustion was exerted at Mingmen(GV4), Yaoyangguan(GV3), and bilateral Shenshu(BL23), Ganshu(BL18) and Dachangshu(BL25), 30 minutes at GV4, GV3 and BL18, and another 30 minutes at BL23 and BL25, once every other day, 3 times a week for 4 weeks. Before and after the treatment, VAS score, Young's modulus of the 4(th) lumbar multifidus and the average speed of 6 m walking were assessed. Results After the treatment, the VAS score was decreased(P<0.05, P<0.01) and the speed of 6 m walking was increased(P<0.05, P<0.01) in both groups in comparison with their own pre-treatment. Compared with the medication group, VAS score was decreased remarkably(P<0.05) and the speed of 6 m walking remarkably increased(P<0.01) in the medication+thunder-fire moxibustion group after the treatment. Self-comparison showed that, compared with the same side before the treatment, the value of Young's modulus after the treatment was decreased on both sides in the medication+thunder-fire moxibustion group(P<0.01). After the treatment, compared with the medication group on the same side, the value of Young's modulus was decreased on both sides(P<0.01) in the medication+thunder-fire moxibustion group. Conclusion Thunder-fire moxibustion can relieve pain intensity, decrease the tension of the multifidus, and increase the walking speed.

      摘要:Objective To observe the effects of pricking and cupping therapy combined with Chinese herbal wet compress on Th1/Th2 balance and the concentration of substance P(SP) in treatment of patients with herpes zoster at acute stage and explore the mechanism of clinical efficacy of this combined therapeutic method. Methods The patients of herpes zoster at acute stage were randomly divided into a treatment group(58 cases), a control group No.1(57 cases), a control group No.2(58 cases) and a control group No.3(59 cases). In the treatment group, the therapeutic regimen was pricking and cupping+Chinese herbal wet compress+basic treatment. In the control group No.1, pricking and cupping + basic treatment were administered. In the control group No.2, Chinese herbal wet compress+basic treatment were provided. In the control group No.3, only basic treatment was delivered. The treatment duration was 9 days in each group. The score of pain degree, the score of sleep quality and the rehabilitation conditions of pain and herpes were observed in 4 groups. The concentration of serum interleukin 4(IL-4), interferon gamma(IFN-γ) and SP were detected by ELISA. The clinical efficacy was evaluated. The occurrence of adverse reaction during treatment was observed. Results After treatment, the score of pain degree and the concentrations of serum IL-4 and SP were all lower than those before treatment in 4 groups(P<0.05); and the score of subjective sleep quality, the concentrations of serum IFN-γ and IFN-γ/IL-4 were all increased in comparison with those before treatment(P<0.05). In the treatment group and the control groups No.1 and No.2, the score of pain degree and the concentrations of serum IL-4 and SP were all lower than those in the control group No.3(P<0.05); and the score of subjective sleep quality, the concentrations of serum IFN-γ and IFN-γ/IL-4 were higher than those in the control group No.3, respectively(P<0.05). Besides, every indicator in the treatment group was improved more significantly than that either in the control group No.1 or the control group No.2(P<0.05). After treatment, compared with the control group No.3, the time of pain relief and disappearance, and the time of blister termination, incrustation and decrustation were all shorter in the treatment group, the control group No.1 and the control group No.2 separately(P<0.05); and every indicator in the treatment group was reduced more significantly in comparison with either the control group No.1 or the control group No.2(P<0.05). The total effective rate in the treatment group(96.55%, 56/58), the control group No.1(92.98%, 53/57) and the control group No.2(91.38%, 53/58) was higher than that in the control group No.3(74.58%, 44/59, P<0.05). No any adverse reaction occurred in the 4 groups. Conclusion The pricking and cupping therapy combined with Chinese herbal wet compress effectively promotes the recovery of Th1/Th2 balance and reduces the concentration of serum SP in patients with herpes zoster at acute stage. This combined therapeutic regimen is conductive to the recovery of patients, the improvement of therapeutic effects and the decrease of the risk of pos-therpetic neuralgia.

      摘要:Objective To evaluate the effectiveness of acupuncture and moxibustion in the treatment of anxiety and depression in irritable bowel syndrome(IBS) by meta-analysis. Methods China National Knowledge Infrastructure(CNKI), Wanfang Data, China Scientific Journal Database(VIP), China Biomedical Literature Service System(SinoMed), PubMed, Embase, and Cochrane Library were searched for randomized controlled trials(RCTs) on acupuncture and moxibustion in the treatment of IBS anxiety and depression from database inception to June 30, 2021. RevMan5.3 and Stata16.0 were used to conduct Meta-analysis of relevant outcome indicators. Results A total of 11 RCTs were included in this study, involving 774 IBS patients(431 in the intervention group and 343 in the control group). Eight of them used manual acupuncture(MA) intervention, and the other three used electroacupuncture(EA) intervention. Meta-analysis showed that compared with the western medicine, MA improved the sedation-agitation scale(SAS) scores(MD=5.24, 95%CI=[3.49, 7.00], P<0.000 01) and self-rating depression scale(SDS) scores(MD=8.50, 95%CI=[4.68, 12.32], P<0.000 1) in IBS patients, and there was no significant difference between MA or EA and western medicine in the improvement of Hamilton anxiety rating scale(HAMA) scores(MD=1.10, 95%CI=[-0.36, 2.56], P=0.14) and Hamilton depression rating scale(HAMD) scores(MD=0.53, 95%CI=[-0.89, 1.94], P=0.47). In terms of total effective rates(RR=1.21, 95%CI=[1.12, 1.31], P<0.000 01) and the irritable bowel syndrome severity scoring system(IBS-SSS) scores(MD=42.16, 95%CI=[27.40, 56.91], P<0.000 01), both MA and EA therapy was superior to western medicine in alleviating gastrointestinal symptoms of IBS patients. Conclusion MA and EA can improve the anxiety and depression of IBS patients, while the influence of other acupuncture and moxibustion interventions needs to be deeply explored with more high-quality RCTs.

      摘要:Objective To analyze the characteristics of acupoint catgut embedding methods, tools, catgut types, and the treatment cycles in the clinical research in recent ten years both at home and abroad, so as to summarize its regularities and to provide technical references for further studies. Methods Articles about clinical researches on catgut embedding therapy published in recent ten years(from January 1, 2010 to December 31,2020) were retrieved from the databases of CNKI, Wanfang, VIP, and PubMed by using key words of “acupoint embedding” “acupoint catgut embedding” and “catgut implantation at acupoint”. According to the inclusion and exclusion criteria, a new database was established for analyzing the data mentioned above. Results 1) A total of 1 196 articles were collected, including 15 English articles and 1 181 Chinese articles, presenting a fluctuating increasing trend in recent ten years. 2) The commonly used acupoint embedding methods included disposable catgut embedment needle method(399 times, 38.89%) and disposable syringe needle catgut embedding method(347 times, 33.82%), for which two or multiple methods were mentioned in the same one article. 3) The most frequently used top two tools for catgut embedding were the dispo-sable catgut embedment needle(463 times, 43.03%) and disposable syringe needle(406 times, 37.73%), with a significant increase in the application of disposable syringe needle. The most commonly used size of tools included No. 7(283 times, 39.86%), No. 9(196 times, 27.61%) and No. 8(109 times, 15.35%). 4) The most frequently implanted surgical suture was still the common catgut(671 times, 58.15%) despite of a reduction in clinical application year by year, and the types of the implanted suture materials were gradually enriched since 2018, such as the absorbable surgical suture, polyethylprolactide(PGLA), collagen protein thread, polydioxanone(PPDO), etc. The commonly used implanted catgut size was 3-0(227 times, 30.15%), 2-0(176 times, 23.37%), 4-0(131 times, 17.40%), 0(103 times, 13.68%), with the commonly used catgut length being 1 cm(332 times, 35.55%), 1.5 cm(103 times, 11.03%), 1-2 cm(92 times, 9.85%) and 2 cm(92 times, 9.85%). 5) The intervals of the catgut implantation were 7 days(313 times, 28.95%), 14 days(262 times, 24.24%), 10 days(174 times, 16.10%), and 15 days(162 times, 14.99%). Conclusion In recent ten years, clinical research on acupoint catgut embedding is growing rapidly, and the embedding methods, tools, implanted sutures, and embedding intervals are various, which may provide certain technical references for the future researches and suggest an urgent need of formulation of the standardized and unified standards in this field.

      摘要:Cartilage damage is the key pathological mechanism in the progressive development of osteoarthritis(OA). Slowing down cartilage damage and accelerating cartilage repair are strategies for effective treatment of OA. Acupuncture and moxibustion therapies are widely used in relieving symptoms of OA and have a protective effect on cartilage. In this paper, we reviewed the mechanisms of acupuncture and moxibustion underlying relieving cartilage damage from three aspects: 1) promoting chondrocyte homeostasis by inhibiting apoptosis and improving cellular autophagy, 2) regulating extracellular matrix(ECM) metabolism(inhibiting decomposition and promoting synthesis) by suppressing the release of inflammatory factors and the activity of proteolytic enzymes, and 3) improving OA microenvironment by reducing the number of macrophagocyte 1(M1) and increasing the ratio of M2/M1 in the local inflammatory locus. In addition, most studies on the mechanisms of acupuncture and moxibustion underlying remission of OA focus on the improvement of pathological changes, such as joint histopathology, cartilage morphology, synovial inflammatory reaction and infiltration, subchondral bone remodeling, etc., thus, the exact functions of acupuncture and moxibustion in ameliorating cartilage injury remain unknown. In view of the important role of mitochondrial dysfunction in promoting OA development and cartilage damage and the current use of tissue engineering methods of chondrocytes and mesenchymal stem cells to repair articular cartilage injury, it is highly recommended that future studies should pay more attention to these aspects.

      摘要:The corticospinal tract(CST), descending from the frontoparietal cortex and traveling down to terminate at the anterior horn of the spinal cord to mediate voluntary movements, is frequently injured from the infarcted or hemorrhagic cerebrovascular insults due to stroke. Under the circumstances, motor dysfunction seriously affects the patient's quality of life. Acupuncture therapy has a sequelae, especially in improving motor deficits. In the present paper, we reviewed the current development of researches on acupuncture treatment of poststroke motor dysfunction and its biological mechanisms from 1) delaying patients' development of neuronal degeneration and white matter fibrosis(Wallerian degeneration), 2) improving patients' upper limb motor function and daily life ability by promoting the repair of white matter tracts and CST on the affected side, 3) promoting the compensation of CST on the healthy side, 4) reconstructing the motor conduction pathway to strengthen the bilateral brain connection in ex-perimental animals, and 5) strengthening the sprouting of the contralateral CST to dominate the affected side again across the midline. In addition, acupuncture stimulation induced improvement of axonal rewiring for corticospinal innervation is also possibly related to its functions in accelerating the synthesis and release of neurotrophic factors, down-regulating Nogo-A/RhoA signaling and activating vascular epithelial growth factor/Dll4/Notch signaling pathways.

    • Design and application of a new type of long snake moxibustion box

      摘要:Long snake moxibustion, a kind of indirect moxibustion therapy for stimulating the midline part of the back of the patient's body after playing a layer of ginger or garlic mud, is frequently used to treat spine disorders and deficiency-cold type syndromes. In the present paper, we introduced our newly made moxibustion box for applying long snake moxibustion which is safe and easy to operate and can be freely moved at any time in the treatment of vertebral diseases, and conveniently makes the ignited-moxa close to the locus. This newly-designed moxibustion box is made of two lines of paralleled flexible connection side plates hinged ends to ends and two U-like connection plates at the opposite two ends to construct a rectangular frame device. The hinged design makes the long snake moxibustion box conform to the physiological curvature of human body and its length can be adjusted according to the height of patients. When used in clinical practice, it can enhance the patients' safety and reduce the operator's working intensity, and may be helpful to the popularization and development of long snake moxibustion therapy.
