      摘要:For more than half a century, the modern bioresearch in acupuncture has made remarkable advancements, proving scientific basis underlying the traditional, intuitive treatment, as well as leading to some new discoveries with the potential to enhance the effectiveness of acupuncture as we know it. Meanwhile, the clinical researches have started to shift its paradigm from traditional individual observation to modern evidence-based medicine. However, there is little interaction between basic and clinic researches, which are like two separate worlds, not benefiting each other. Also the education and training of acupuncture are still traditional style, little combining with modern studies. To bridging the large gap, we need translational science involving in. In this article, with a critical reviews of the limitations of the traditional methods of acupuncture, the challenges faced by clinic practices and placebo-control studies, and the advantages and disadvantages of basic research, we propose a methodological paradigm of the translational research, Translational Acupuncture Research Spectrum, that meets the current situation of acupuncture researches, hoping to give insights into this field and to promote modern acupuncture to move towards a new stage.

    • The translational medicine approach to acupuncture and moxibustion

      摘要:The idea of translational medicine from “laboratory to bedside” has become more and more popular. The main purpose of translational medicine is to promote basic research to solve practical clinical problems and improve the level of clinical treatment. It is thus a whole new medical paradigm. However, the principle of “bench to bedside” still has some obvious deficiencies in the “bench” ring, whereas the subject of acupuncture and moxibustion is carried out in the “bench” ring relatively more systematic, but also does not achieve bench to bedside regression well. Nevertheless, as a bridge between basic research and clinical practice of acupuncture and moxibustion, translational medicine has constructed a data fusion channel between them. Our basic researchers should cooperate closely with clinical experts, exchange ideas, collide sparks of wisdom, and promote the leap-forward development of acupuncture and moxibustion.

      摘要:The development of translational medicine is on the rise, and its advocated relationship between preclinical research and clinical application and two-way translational research mode inspire and promote us to think about solving the corresponding problems faced by acupuncture and moxibustion medicine. The theoretical research of basic research should be closely connected with the practice and application due to the characteristics of the subject, but seems to be seriously disjointed at pre-sent. Therefore, four aspects as “the relationship between acupuncture theory and clinical practice” “correctly interpreting the connotation of traditional theory of acupuncture and moxibustion” “distinguishing which is based on the practice and which is the influence of cultural concepts” and “distinguishing what are the ancient medical discoveries and inventions and what are the theoretical explanations of such discoveries and inventions” should be grasped. In addition, the importance of concept consciousness and two research levels(uncovering unrecognized or buried medical discoveries and techniques and expounding valuable medical ideas) in the transformation research should be emphasized, and the evaluation mechanism for the transformation of research achievements should be established.

      摘要:Neuroimmunological anti-inflammation is one of the most successful areas of translational medicine in the past 50 years, leading directly to the success of anti-TNF biologics and vagal nerve stimulation therapy for anti-inflammation. The latter has many similarities with traditional acupuncture therapy and further research may reveal more intertwining and enlightenment to acupuncture. This paper briefly reviews the discovery and translational process of Cachectin/TNF, anti-TNF therapy, neural inflammatory reflex arc, vagus nerve stimulation therapy, and Stimulating Peripheral Activity to Relieve Condition(SPARC) project, and also summarizes the experience and lessons learned from the path of translational medicine in this field. The two key steps in the translational process are discussed in details. The purpose is to provide reference for the development of translational medicine in acupuncture. “Minimal injury with significant healing” should be considered as the principal hypothesis to further study the biological mechanism of acupuncture and it may result in more clinical translation in the future.

      摘要:It has become one of the major strategic directions to develop translational medicine and realize the stride leap from basic research to clinical application in the national biomedical field. In recent years, both basic and clinical researches of acupuncture and moxibustion have made landmark progress. However, the transformation of acupuncture and moxibustion techniques still faced with many difficulties, such as low conversion rate and high difficulty of transformation. In this article, we mainly made a preliminary discussion on the current situations of acupuncture and moxibustion transformation from the aspects of basic research content, evidence-based medicine and clinical trials, evaluation criteria for the conclusion of scientific research projects and transformation mode, put forward some existing problems in the transformation process, and proposed some corresponding countermeasures to the problems.

      摘要:The acupuncture treatment scheme contains several key elements including acupoint selection and prescription, duration of needle retention, treatment frequency, needling manipulation technique, etc. which have, in precision degree, an important influence on the clinical efficacy, and their precise determination needs the guidance of the dose-effect relationship of acupuncture stimulation. In the present article, we discuss the basic concept and content of the dose-effect relationship of acupuncture and analyze its relationship with the precise treatment scheme. The precision and standardization of clinical treatment scheme of acupuncture are definitely not the negation of rich clinical experience, but rather, they are the transcendence of rich clinical experience. The dose-effect relationship of acupuncture is the rule that the acupuncture effect changes with the quantitative change of each key element, and its main contents include: the dose-effect relationship of acupoint prescription, the dose-effect relationship of needling manipulation, and the time-effect relationship of acupuncture.

      摘要:The regularity of acupoint-viscera correlation is a basic problem in the study of acupuncture and moxibustion. At present, there are two main theories of the regularities:(1) the law of acupoint effect determined by meridian circulation;(2) segmental dominance of acupoint effect. These two regularities belong to different paradigm systems, they are not complementary relations within the same conceptual system. Based on the evidence of clinical practice of acupuncture and moxibustion, combined with the research on biological mechanisms, this paper analyzed the objectivity and universality of the regularities of acupoint-visceral correlation. The law determined by meridian of acupoint effect does not manifests the universality guiding role in the clinical practice of modern acupuncture and moxibustion, which is obviously divorced from the needs of modern clinical practice. The segmental dominance of acupoint effect has been strongly supported by the clinical practice of modern acupuncture and moxibustion, which is highly consistent with the clinical practice. The regularity of acupoint effect needs to be tested and re-identified in practice.

      摘要:Objective To explore the rules of acupoints selection of acupuncture and moxibustion in the treatment of allergic rhinitis(AR) by using data mining technology, as well as to compare the efficacy of different acupoints selection methods. Methods Papers about acupuncture and moxibustion for treating AR published from January 2002 to August 2022 was retrieved from Chinese and English databases including CNKI, Wanfang, VIP, SinoMed and PubMed by using keywords of “acupuncture”, “moxibustion” and “allergic rhinitis”. According to the inclusion and exclusion criteria, the collected literature was screened to establish the AR database. Frequency statistic analysis was conducted for detecting high-frequency acupoints and specific acupoints frequency, and the curative effects of different acupoints selection methods were compared. SPSS26.0 software was used for factor analysis, cluster analysis and QUEST decision tree model identification. Results A total of 289 papers were included, with 384 acupuncture prescriptions extracted. A total of 99 acupoints were involved with the application frequency of 2 430 times. Among them, the application frequency of Yingxiang(LI20) is the highest(296 times, 12.18%), followed by Yintang(GV24+) and Hegu(LI4), etc. The main invloved meridians are the Bladder Meridian of Foot-Taiyang, the Large Intestine Meridian of Hand-Yangming and the Governor Vessel. The involved specific acupoints with the highest frequency of application is the crossing acupoints. Nine common factors of acupoints combinations units were extracted by factor analysis, and two cluster prescriptions of acupoints combinations correlation were obtained by cluster analysis. Three decision paths of simplified acupoints selection were simulated by the decision tree, with LI20 as the dependent variable. Conclusion In the treatment of AR with acupuncture and moxibustion, the regularities and characteristics of acupoints selection are as follows: 1) often selecting local acupoints and acupoint combinations along meridians, 2) focusing on combination of dispelling pathogenic factors with strengthening vital energy, 3) advocating diversification of acupoint matching methods. The combination of factor analysis, cluster analysis and QUEST decision tree application provides three directions for clinical acupoints selection of AR.

      摘要:Talent team construction is the foundation of translational medicine research of acupuncture and moxibustion. In college education and continuing education after graduation, with the idea of translational medicine, it is necessary to cultivate both the ability of basic research and clinical skills for the students of traditional Chinese medicine, especially those majored in acupuncture and moxibustion, as well as train the acupuncture-moxibustion researchers who qualified with medical practitioner to contact with clinical practice, discover clinical scientific problems and research priorities, and engage in basic research at the same time. Acupuncture-moxibustion practitioners with higher education should be trained to be able to engage in basic and development researches on the premise of improving their clinical ability, so as to cultivate more translational research talents, build translational research team, and promote the pace of research and application of acupuncture-moxibustion translational medicine.

      摘要:Objective To observe the effect of electroacupuncture(EA) on the morphology of prostate and degranulation of mast cells in prostate of rats with chronic nonbacterial prostatitis(CNP). Methods Male SD rats were randomly divided into sham operation group, model group and EA group, with 8 rats in each group. CNP model was established by surgical castration combined with 17-β estradiol injection once daily for 30 days. EA was applied to “Zhongji”(CV3), “Guanyuan”(CV4) and bilateral “Dahe”(KI12) for 20 min, once daily for 8 days. The mechanical pain threshold of scrotum skin area was tested before modeling, after modeling and after intervention. The pathological morphology of the prostate was observed by HE staining. Collagenous fiber was observed by Masson staining. The infiltration of mast cells was observed by toluidine blue staining. The contents of interleukin 6(IL-6) and tumor necrosis factor α(TNF-α) in prostate were determined by ELISA. The protein expression levels of tryptase and transforming growth factor β1(TGF-β1) in prostate were detected by Western blot. Results A normal appearance with little inflammatory cell infiltration was observed in the prostate of the sham operation group. Gland atrophy, epithelial destruction, hyperemia and edema, diffuse inflammatory cell infiltration and visible collagen fiber deposition were observed in prostate of the model group. The degree of infiltration of inflammatory cells and collagen fiber deposition were reduced in the EA group. Compared with the sham operation group, mechanical pain threshold was decreased(P<0.01), while the collagen volu-me fraction(CVF) of prostate, the degranulated rate of mast cells, the protein expression levels of tryptase and TGF-β1, and the contents of IL-6 and TNF-α were increased(P<0.01) in the model group. Following EA intervention, compared with the model group, the mechanical pain threshold was increased(P<0.01), CVF of the prostate, the degranulated rate of mast cells, the protein expression levels of tryptase and TGF-β1, and the contents of IL-6 and TNF-α were decreased(P<0.05, P<0.01) in the EA group. Conclusion EA can relieve pain and reduce inflammation and fibrosis of prostate in CNP rats, which may be related to inhibiting the degranulation of prostate mast cells and down-regulating the expression of inflammatory factors and TGF-β1.

      摘要:Objective To investigate the mechanism of electroacupuncture(EA) intervention in rats with primary dysmenorrhea(PDM) based on the Toll-like receptor 4(TLR4)/nuclear factor(NF)-κB signaling pathway. Methods Forty female SD rats were randomly divided into blank control, model, EA and medication groups, with 10 rats in each group. PDM rat model was established by subcutaneous injection of estradiol benzoate combined with intraperitoneal injection of oxytocin. At the same time of model procedures, EA(50 Hz, dense wave) was applied to “Guanyuan”(CV4) and bilateral “Sanyinjiao”(SP6) of rats in the EA group, with needles retained for 20 min, for 10 consecutive days. Rats in the medication group received ibuprofen(125 mg/100 mL, 0.8 mL) by gavage for 10 consecutive days. At the 11(th) day, writhing behavior of rats was assessed. Uterine morphology was observed by eyes and uterine pathological changes were observed after HE staining. Content of prostaglandin E2(PGE2) and prostaglandin F2α(PGF2α) in serum and uterine tissues was detected by ELISA; NF-κB p65 positive expression in nucleus was detected by immunofluorescence; protein expression levels of TLR4, NF-κB p65, p-NF-κB p65 and inflammatory factors interleukin(IL)-1β and IL-18 were detected by Western blot. Results After modeling, uterus tissues were congested and edematous, with necrosis of luminal epithelium, severe edema and extensive shedding of endometrium, nuclear pyknosis, fragmentation and disappearance, neutrophils infiltration, and slight expansion of glandular cavity, which was milder in the EA and the medication groups. Compared with the blank control group, writhing times, scores and incubation period, HE pathological scores, PGF2α contents in serum and uterine tissues, ratio of NF-κB p65 positive expression in nucleus, TLR4, NF-κB p65, p-NF-κB p65, IL-1β and IL-18 protein expression levels in uterine tissues of rats in the model group were all significantly increased(P<0.01), while PGE2 contents in serum and uterine tissues were significantly decreased(P<0.01). Compared with the model group, writhing times and scores, HE pathological scores, PGF2α contents in serum and uterine tissues, ratio of NF-κB p65 positive expression in nucleus, TLR4, NF-κB p65, p-NF-κB p65, IL-1β and IL-18 protein expression levels in uterine tissues of rats in the EA and medication group were all significantly decreased(P<0.01), while writhing incubation period, PGE2 contents in serum and uterine tissues were significantly increased(P<0.05,P<0.01). Conclusion EA intervention could relieve inflammatory response and pain in PDM rats, which may be related to its effect in reducing TLR4 expression, inhibiting NF-κB activation and down-regulating inflammatory factors levels of IL-1β and IL-18.

      摘要:Objective To observe the effect of electroacupuncture(EA) pretreatment on inflammatory response in ven-tilator-induced lung injury(VILI) mice, so as to explore the underlying mechanism of EA pretreatment on prevention of VILI. Methods C57BL/6 mice were randomly divided into sham-operation group, model group, EA group and sham-acupoint group, with 8 mice in each group. The VILI model was established by ventilation with high tidal volume. Mice in the EA group and sham-acupoint group were given EA at “Zusanli”(ST36)and “Feishu”(BL13) or non-acupoints(located at 1—2 cm on both sides of the tail root of the proximal trunk) before mechanical ventilation, 30 min each time, once a day for 5 days. Arterial blood was collec-ted for blood gas analysis, the total protein content in bronchoalveolar lavage fluid(BALF) was detected by BCA method. The contents of interleukin-1β(IL-1β) and interleukin-18(IL-18) in BALF were detected by ELISA. Lung injury score was determined after HE staining. The protein expression levels of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3), apoptosis-associated speck-like protein containing a caspase-recruitment domain(ASC) and Caspase-1 in lung tissue was detected by Western blot. Results Compared with the sham-operation group, the arterial partial pressure of oxygen and oxygenation index were decreased(P<0.05), the levels of total protein, IL-1β and IL-18 in BALF, the W/D value and the pathological injury score of lung tissue and the protein expression levels of NLRP3, Caspase-1 and ASC were increased(P<0.05)in the model group. Following the interventions, the above mentioned increased or decreased indicators were reversed(P<0.05) in the EA group rather than in the sham-acupoint group. Conclusion EA pretreatment of ST36 and BL13 can reduce the damage of lung tissue caused by mechanical ventilation, which may be related to its effect in reducing the expression of NLPR3 inflammasome related proteins, reducing the activation of inflammasome, and thereby reducing the inflammatory response.

      摘要:Objective To observe the effect of “lingguibafa” moxibustion at “opening” time on the level of superoxide dismutase(SOD), malondialdehyde(MDA), the protein expressions of B-cell lymphoma-2(Bcl-2) and Bcl-2 associated X protein(Bax) in aging model rats. Methods SD rats were randomly divided into blank control, model and lingguibafa groups, with 8 rats in each group. The aging model was established by daily intraperitoneal injection of D-galactose(500 mg/kg) for 42 consecutive days. After successful modeling, moxibustion intervention was applied at“lingguibafa” acupoints, 3 moxa-cone for each acupoint, once a day for 28 consecutive days. The contents of SOD and MDA in serum were detected by ELISA. Morphological changes of testicular tissue and the number of Leydig cells were observed after HE staining. Apoptosis rate of testicular cells was detected by TUNEL staining. The protein expression levels of Bcl-2 and Bax in testis were detected by Western blot. Results Compared with the blank control group, the SOD content in the serum, the number of testicular Leydig cells, the expression level of Bcl-2 protein in testis were significantly decreased(P<0.01) in the model group, while the MDA content in the serum, the apoptosis rate of testicular cells and the expression level of Bax protein in testis were significantly increased(P<0.01). Compared with the model group, the SOD content in serum, the number of testicular Leydig cells, the Bcl-2 protein expression in testis were significantly increased(P<0.01), while the MDA content in serum, the apoptosis rate of testicular cells, the expression level of Bax protein in testis were significantly decreased(P<0.01) in the lingguibafa group. Conclusion Moxibustion on acupoints at “opening” time can delay testicular aging, and the mechanism may be related to balancing the metabolism of free radicals, reducing oxidative damage, and inhibiting the apoptosis of testicular cells.

      摘要:Objective To observe the synergistic effect and safety of combined use of houpo paiqi mixture and thumb-tack needle on promoting gastrointestinal function recovery after cesarean section. Methods Parturients receiving cesarean section were randomly divided into the control group(29 cases), the traditional Chinese medicine(TCM) group(30 cases) and needle+TCM group(30 cases). The control group received only routine postoperative treatment. Besides the treatment as the control group, parturients in the TCM group were given 50 mL houpo paiqi mixture 6 h and 10 h after cesarean section respectively. Besides the treatment as the TCM group, parturients in the needle+TCM group received thumb-tack needle treatment at bilateral Zusanli(ST36), Tianshu(ST25), Shangjuxu(ST37) and Sanyinjiao(SP6), Zhongwan(CV12), Qihai(CV6), with auricular pressure at bilateral otopoints Zigong(Uterus), Wei(Stomach) and Dachang(Large intestine), within 1 h after the parturients returned to the ward after the operation. Each acupoint was pressed for 10 s and performed acupressure every 4 h(except sleeping hours), continuously for 3 d. The time of bowel sound recovery, the time to the first postoperative exhaust and defecation, the time of postoperative semi-fluid diet recovery, incidence of postoperative nausea and vomiting(PONV) and abdominal distention, and abdominal pain VAS score were recorded and analyzed. The safety of the treatments was also evaluated. Results Compared with the control group, the time of bowel sounds recovery, the time to the first postoperative exhaust and the time of postoperative semi-fluid diet recovery were significantly shortened in the TCM and needle+TCM groups(P<0.01); the time to the first postoperative defecation were significantly shortened(P<0.01), incidence of both abdominal distention and PONV were significantly decreased in the needle+TCM group(P<0.05,P<0.01); incidence of abdominal distention were significantly decreased in TCM group(P<0.05). Compared with the TCM group, the time of bowel sounds recovery, the time to the first postoperative exhaust and defecation, and the time of postoperative semi-fluid diet recovery were significantly shortened(P<0.01), and incidence of both abdominal distention and PONV were significantly decreased in the needle+TCM group(P<0.05). VAS scores of the three groups decreased gradually over time(P<0.01). Compared with the control and TCM groups, VAS scores of the needle+TCM group significantly decreased at 24 h, 48 h and 72 h after operation(P<0.01). No treatment-related adverse reactions were observed during the whole trial. Conclusion On the base of the treatment with houpo paiqi mixture, the addition of thumb-tack needle treatment exerted positive synergistic effect on gastrointestinal function recovery after cesarean section, with high safety, which is worthy of clinical application.

      摘要:Objective To observe the clinical efficacy of shuanggu yitong acupuncture therapy(the therapy for both replenishment and unblocking) combined with domperidone on diabetic gastroparesis(DGP) of liver stagnation and spleen deficiency pattern and explore its effect mechanism. Methods DGP patients differentiated as liver stagnation and spleen deficiency pattern were divided into a control group(n=42) and an observation group(n=42) according to the random number table. The patients in the control group took domperidone tablets orally, 10 mg each time, 3 times a day for 28 days. In the observation group, on the base of the treatment as the control group, shuanggu yitong acupuncture therapy was applied to Baihui(GV20), Shenting(GV24), Zhongwan(CV12), bilateral Zusanli(ST36), Hegu(LI4)and Taichong(LR3), stimulated for 30 min in each treatment. Acupuncture was given once daily, 3 times a weeks for 28 days consecutively. Fasting blood glucose(FBG), 2-hour postprandial blood glucose(2 h PBG) and glycosylated hemoglobin(HbA1c) were detected before and after treatment in the patients of two groups separately. The score of symptom severity index of gastroparesis(GCSI), traditional Chinese medicine(TCM) syndrome score and gastric emptying rate were assessed in the patients of two groups. Using ELISA, radioimmunoassay and colorimentry methods, the contents of motilin in plasma, gastrin, tumor necrosis factor-α(TNF-α), interleukin-6(IL-6), interleukin-1β(IL-1β) and interferon-gamma(INF-γ) in serum, as well as the activity of superoxide dismutase(SOD), reactive oxygen species(ROS) and malondialdehyde(MDA) in the serum were determined in the two groups. The clinical curative effect was evaluated. Results After treatment, the levels of FBG, 2 h PBG and HbA1c, the scores of GCSI and TCM syndrome, the contents of motilin in plasma, gastrin, TNF-α and MDA, as well as the activity of ROS in serum were all reduced when compared with those before treatment in each group(P<0.05, P<0.01), while gastric emptying rate and SOD activity in the serum were higher than those before treatment(P<0.05, P<0.01). After treatment, the serological content of INF-γ was lower than that before treatment in the control group(P<0.05), and the contents of IL-6 and IL-1β were reduced than those before treatment in the observation group(P<0.05). Compared with the control group, the levels of FBG, 2 h PBG and HbA1c, the scores of GCSI and TCM symptoms, the contents of motilin in plasma, gastrin, TNF-α, MDA, IL-6 and IL-1β, and the activity of ROS in serum in the observation group were all lower significantly(P<0.05, P<0.01), while the SOD activity and gastric emptying rate in the observation group were higher than those in the control group(P<0.05,P<0.01). The total effective rate was 90.5%(38/42) in the observation group, better than the control group(73.8%, 31/42, P<0.05). Conclusion Shuanggu yitong acupuncture therapy combined with domperidone remarkably relieves the clinical symptoms and improves the gastric emptying rate, effectively reduces motilin and gastrin and regulates oxidative stress and inflammatory responses in the patients with DGP of liver stagnation and spleen deficiency.

      摘要:Objective To evaluate the efficacy and safety of moxibustion at Baihui(CV20) combined with acupuncture in treatment of cervical vertigo. Methods From the databases, such as CNKI, VIP, WanFang, CBM, PubMed, Cochrane Library and Embase, the studies of randomized controlled trials(RCTs) on moxibustion at CV20 combined with acupuncture for cervical vertigo were searched from inception to September 15(th), 2021. The Cochrane risk of bias assessment tool was utilized to evaluate the quality of the included literature. Using RevMan5.3, Stata12.0 and TSA0.9.5.0 10 Beta software, the Meta-analysis and trial sequential analysis(TSA) were performed. Results Seventeen RCTs with 1 232 patients were included. Meta-analysis showed that in the trial group(moxibustion at CV20 combined with acupuncture), the total effective rate(RR=1.17, 95%CI[1.12, 1.22], P<0.000 1), the curative and remarkably-effective rate(RR=1.28,95%CI[1.20,1.36],P<0.000 1) and the score of cervical vertigo(WMD=2.88, 95%CI[1.87, 3.89], P<0.000 1) were all better when compared with the control group(simple acupuncture or electroacupuncture group). The results of trial sequential analysis indicated that for the cumulative Z-score of each RCT, the Z-curve crossed the conventional test boundary and TSA boundary, which further confirmed the clinical efficacy of moxibustion at CV20 combined with acupuncture on cervical vertigo. Conclusion The clinical efficacy of moxibustion at CV20 combined with acupuncture is determined in treatment of cervical vertigo.

    • Discussion on the safety of acupuncture in experimental animals

In the present paper, we make a discussion on the current situations, causes, its influence on research results and countermeasures of acupuncture safety events in experimental animals, so as to promote the safety evaluation of acupuncture in experimental animals and the standardized operation of acupuncture.

    • An anti-scald device for warm needling 增强出版

      摘要:Warm needling, i.e. acupuncture with the needle warmed by burning moxa stick or cone, is frequently employed in the treatment of cold and dampness type disorders. During treatment, accidental skin scald may occur if the burning moxa drops on the skin due to slight changes in patient's body position. Thus, we designed and developed an anti-scald device for warm needling which is suitable for any part of the body. This device is made up of two parts, a stainless steel-grid moxa cartridge(including half cylinder, hinge shaft, lug, limit bar, clamping arm, connecting arm, torsion spring, heat insulation pad, through holes) and a clamp holder which is in an integrated structure. The grid moxa cartridge can be used to wrap the burning mugwort cone in all directions to prevent the ignited moxa-cone from falling and skin scalding, and effectively collect the burned moxa ash. At the same time, the clamp holder can be used to help fix the moxa-cone to increase the stability of warm needling operation. The device is convenient to operate and novel in design, can effectively reduce the occurrence of scald accidents in clinical treatment, save time and manpower, and has both economic and ecological benefits, being helpful to the promotion and use of warm needling.
