目的:探索穴位敷贴疗法对哮喘的治疗效果及可能的作用机制。方法:将大鼠随机分为正常组(n=6)、模型组(n=9)、地塞米松组(n=9)及穴位贴药组(n=9)。以卵蛋白致敏并诱发哮喘模型。地塞米松组在哮喘大鼠雾化后予以腹腔注射1 mg/kg地塞米松
同时在背部备皮予以空白药物敷贴;穴位贴药组在哮喘大鼠雾化后予以腹腔注射1 mg/kg的生理盐水
同时在背部备皮予以治疗药物敷贴。穴位取“大椎”“肺俞”“脾俞”“肾俞”穴。疗程结束24 h内取各组大鼠肺组织固定、HE染色
观察其炎症情况
并用免疫组化方法比较各组中白细胞介素-4(IL-4)、γ干扰素(IFN-γ)在肺组织中的表达。结果:模型组大鼠肺组织中可见嗜酸性粒细胞、淋巴细胞及巨噬细胞浸润且较正常组明显增加(P<0.01);地塞米松组大鼠肺组织中嗜酸性粒细胞、淋巴细胞及巨噬细胞浸润与模型组比较均明显降低(P<0.01
0.05);穴位贴药组对嗜酸性粒细胞浸润有明显抑制作用(P<0.01)
但对于淋巴细胞及巨噬细胞浸润无明显作用(P>0.05)。模型组大鼠肺组织中IL-4表达较正常组明显增加(P<0.01)
而IFN-γ表达较正常组明显降低(P<0.01);地塞米松组及穴位贴药组大鼠肺组织中IL-4表达较模型组明显降低(P<0.01);地塞米松组IFN-γ表达与模型组比较无明显差异(P>0.05)
且明显低于正常组(P<0.01);穴位贴药组IFN-γ表达明显高于模型组(P<0.05)
且与正常组无明显差异(P>0.05)。结论:中药穴位敷贴疗法对IL-4、IFN-γ在哮喘大鼠肺组织中的表达具有明确的影响
并对哮喘大鼠肺组织中嗜酸性粒细胞的浸润有明显的抑制作用。以上影响对大鼠体内Th1/Th2的平衡可能起到了良性的调整作用
从而对哮喘的治疗产生效果。
Objective: To observe the effect of acupoint application of Chinese herbal medicines on pathological changes of pulmonary tissue in asthma rats so as to explore its underlying mechanism.Methods: A total of 33 Wistar rats were randomized into normal control(n=6)
model(n=9)
dexamethasone(DX
n=9) and acupoint-application(A-A
n=9) groups.Asthma model was established by intraperitoneal injection of 10% ovalbumin(OVA
1 mL)and forced inhalation of atomized 2% OVA(25 mL) for 40 min
once daily for 2 weeks."Dazhui"(GV 14)
"Pishu"(BL 20)
"Feishu"(BL 13) and "Shenshu"(BL 23) were selected for external application of Chinese medicinal herbs(pricklyash peel
white mustard seeds
asarum herb
etc.) for rats of A-A group
and that of control drugs(ang-kak
black rice
ginger juice
etc) for rats of DX group.Infiltration degrees of eosinophils(Eos)
lymphocytes(L) and macrophages(M) in the pulmonary tissue were observed under microscope.IL-4 and IFN-γ expression was displayed by using immunohistochemical method.Results: In comparison with control group

L and M increased significantly in model
DX and acupoint-application groups(P<0.05
0.01);while compared with model group
the infiltration of Eos
L and M in DX group
Eos in A-A group decreased significantly(P<0.05

IFN-γ expression decreased considerably in model and DX group(P<0.01);when compared with model group
IL4 in both A-A group and DX group decreased significantly
and IFN-γ expression in A-A group increased considerably(P<0.05
0.01).No significant differences were found between DX and A-A groups in IL-4 expression(P>0.05).Conclusion:Acupoint application of Chinese herbal medicines can balance Th1/Th2 and effectively reduce the infiltration of eosinophils
which may contribute to its therapeutic effect in relieving asthma.
