目的:观察电针五脏俞对失眠大鼠睡眠时间及脑内神经递质、细胞因子水平的影响
探讨电针五脏俞治疗失眠的机制。方法:SD大鼠随机分为对照组、模型组、电针组和地西泮组
每组10只。采用氯苯丙氨酸诱导构建失眠大鼠模型。电针组予以电针"肺俞""心俞""肝俞""脾俞""肾俞"
频率60 Hz
强度1 mA
10 min/次
西药组予以地西泮灌胃治疗(0. 92 mg/kg)
均每日1次
共治疗6 d。评估各组大鼠睡眠持续时间变化
HE染色检测治疗后大鼠海马病理学改变
ELISA法检测下丘脑5-羟色胺(5-HT)、5-羟吲哚乙酸(5-HIAA)、肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-1β含量
免疫组织化学法、荧光定量PCR法和Western blot法检测海马组织外周型苯二氮卓受体(PBR)mRNA及蛋白表达。结果:与对照组比较
模型组大鼠昼夜节律紊乱
睡眠持续时间显著缩短、体质量下降(P<0. 05);与模型组比较
电针组及地西泮组大鼠睡眠持续时间增长、体质量增加(P<0. 05)。与对照组比较
模型组大鼠海马组织神经元数量减少
细胞排列不规则
下丘脑5-HT、5-HIAA、TNF-α和IL-1β表达水平均显著降低(P<0. 05)
海马组织中PBR mRNA和蛋白表达水平显著升高(P<0. 05)。与模型组比较
电针组和地西泮组海马组织神经元数量增加
细胞排列逐渐规则
下丘脑中5-HT、5-HIAA、TNF-α和IL-1β表达水平均显著升高(P<0. 05)
海马组织中PBR mRNA和蛋白表达水平显著降低(P<0. 05)。电针在增加睡眠持续时间、体质量、下丘脑5-HT和5-HIAA表达方面与地西泮疗效相当
在升高下丘脑TNF-α和IL-1β表达水平、降低海马组织PBR mRNA及蛋白表达方面优于地西泮(P<0. 05)。结论:电针五脏俞能够降低海马组织中PBR表达
促进下丘脑神经递质及细胞因子的合成和释放
缓解海马组织的病理损伤
改善失眠大鼠的症状。
Objective To observe the therapeutic effect of electroacupuncture(EA)at five Back-Shu points on sleep
hippocampal peripheral benzodiazepine receptor(PBR) expression and hypothalamic 5-hydroxytryptamine(5-HT)
5-hydroxyindoleacetic acid(5-HIAA)
tumor necrosis factor alpha(TNF-α)and interleukin(IL)-1β contents in insomnia rats
so as to explore its mechanisms underlying improvement of insomnia.Methods Forty male SD rats were randomly divided into control
model
EA and medication(Diazepam)groups(n=10 rats in each group). The insomnia model was established by intraperitoneal injection(i. p.)of para-Chlorophenylalanine(PCPA
300 mg/kg)once daily for2 days. EA(60 Hz
1 mA)was applied to bilateral five Back-Shu points
i. e.
Feishu(BL13)
Xinshu(BL15)
Ganshu(BL18)
Pishu(BL20)and Shenshu(BL23)for 10 min
once daily for 6 days. Rats of the medication group were treated by gavage of Diazepam(0. 92 mg/kg)once daily for 6 days. The sleep duration was recorded after i. p. of Pentobarbital Sodium(45 mg/kg). Histopathological changes of the hippocampus were displayed by H. E. staining. The contents of 5-HT
5-HIAA
TNF-α and IL-1β in the hypothalamus were assessed by using ELISA. The expression levels of PBR mRNA and protein in the hippocampus were detected by quantitative real-time PCR
immunohistochemistry and Western blot
separately.Results Following modeling
the sleep duration was considerably shortened in rats of the model group relevant to the control group(P<0. 05). After the treatment
the sleep duration was significantly increased in both EA and medication groups relevant to the model group(P<0. 05). After modeling
the hippocampal neurons were obviously decreased in number
and disordered in the arrangement

TNF-α
and IL-1β were significantly down-regulated(P<0. 05)
and the expression of PBR mRNA and protein in hippocampus was obviously increased relevant to the control group(P<0. 05). Following the treatment
the hippocampal neurons were increased in number and arranged regularly
and the contents of hypothalamic 5-HT
5-HIAA
TNF-α
and IL-1β were considerably increased(P<0. 05)
and the expression of PBR mRNA and protein in hippocampus was significantly decreased in both EA and medication groups relevant to the model group(P<0. 05). The therapeutic effect of EA was comparable to that of medication in increasing sleep duration
body weight
5-HT and 5-HIAA contents(P>0. 05)
and significantly superior to that of the medication in increasing TNF-α and IL-1β levels(P<0. 05)
and considerably superior to that of medication in down-regulating PBR mRNA and protein expression(P<0. 05).Conclusion EA at five Back-Shu-points of the five Zang-organs can significantly improve the sleep in insomnia rats
which is closely associated with its effects in reducing the expression of PBR in hippocampus and up-regulating the levels of 5-HT
5-HIAA
TNF-α and IL-1β in hypothalamus.
