目的:观察针刺对类风湿关节炎(rheumatoid arthritis
RA)患者核转录因子(NF)-κB信号通路的影响
探讨其通过调控NF-κB信号通路治疗RA的机制。方法:选取江西省新余市中医院2018年6月至2019年6月收治的RA患者120例
随机分为对照组和治疗组
每组60例。对照组口服甲氨蝶呤
(10~15)mg/次
1次/周
共治疗10周;治疗组在口服甲氨蝶呤治疗的基础上联合针刺治疗
1次/d
主要穴位为曲池、足三里、太冲、合谷、肾俞、膈俞以及大椎穴等
连续6次为1个疗程
每个疗程间隔1 d
治疗10个疗程。比较两组患者治疗前后血清C-反应蛋白(CRP)和血沉(ESR)、疼痛视觉模拟量尺(VAS)评分、肿胀关节数、晨僵时间
用酶联免疫吸附法和Western blot法检测患者治疗前后关节液中炎性反应因子和NF-κB p65蛋白表达水平
比较两组患者治疗后的临床疗效。结果:治疗后
两组患者CRP、ESR、VAS评分、肿胀关节数、晨僵时间以及关节液中NF-κB p65蛋白及肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、IL-6水平均较治疗前下降(P<0.05)
转化生长因子-β(TGF-β)、IL-10水平较治疗前升高(P<0.05);治疗组CRP、ESR、VAS评分、晨僵时间以及关节液中NF-κB p65、TNF-α、IL-1β和IL-6水平均低于对照组(P<0.05)
TGF-β水平高于对照组(P<0.05)。对照组总有效率为75.0%(45/60)
治疗组总有效率为85.0%(51/60)
治疗组总有效率高于对照组(P<0.05)。结论:针刺可通过抑制RA患者关节液中NF-κB信号通路p65蛋白的表达
调控相关炎性因子的水平
改善RA患者的症状。
Objective To observe the effect of acupuncture on nuclear factor kappa-B(NF-κB)signaling pathway in patients with rheumatoid arthritis(RA)
so as to explore the mechanism of acupuncture in the treatment of RA. Methods A total of 120 RA patients admitted to the Xinyu Hospital of Traditional Chinese Medicine in Jiangxi Province from June 2018 to June 2019 were selected and randomly divided into a control group and a treatment group
with 60 cases in each group. The patients in the control group received oral administration of Methotrexate
(10-15) mg/time
once a week
for a total of 10 weeks. On the basis of oral administration of Methotrexate
patients in the treatment group were treated with acupuncture at Quchi(LI11)
Zusanli(ST36)
Taichong(LR3)
Hegu(LI4)
Shenshu(BL23)
Geshu(BL17) and Dazhui(GV14) once daily. The treatment was conducted 6 consecutive times as a course for 10 courses. The serum C-reactive protein(CRP) and erythrocyte sedimentation rate(ESR)

IL-1β and IL-6 in the treatment group were down-regulated(P<0.05)
while the level of TGF-β was up-regulated(P<0.05). The treatment group had a higher total effective rate than the control group[85.0%(51/60) vs 75.0%(45/60)
P<0.05]. Conclusion Acupuncture can inhibit the expression of NF-κB signaling pathway in RA patients
regulate the levels of related inflammatory factors
and thus improve the symptoms of RA patients.
