目的:动态观察头针对局灶性脑缺血模型大鼠缺血后脑微血管内皮基质金属蛋白酶(MMP-9)表达的影响
探讨头针对缺血脑组织的保护作用机制。方法:将Wistar大鼠80只随机分为正常组、假手术组各8只
缺血模型组、头针治疗组各32只
后两组又分为造模后1、3、51、0 d 4个时间段组
每时间段各8只。采用大脑中动脉线栓阻塞法建立大鼠局灶性脑缺血模型。头针组选取"百会""曲鬓"穴进行手捻针治疗。各组实验前后分别进行神经行为学评分;分别在规定的时间点处死大鼠
光镜下观察梗死局部脑组织海马CA 3区神经细胞的形态学变化;免疫组化法检测各组海马CA 3区微血管内皮MMP-9的表达。结果:模型组大鼠各时间段的神经行为学评分明显高于正常组(均P<0.01)
头针10 d组的神经行为学评分明显低于同时段模型组(P<0.05)。HE染色显示
模型组海马CA 3区神经细胞出现明显的病理改变
针刺后完整的神经细胞明显增多
血管病变主要以内皮细胞肿胀、空泡形成为主。模型组MMP-9在脑缺血后1 d即开始表达
3 d逐渐增多达到高峰
5 d持续高峰
10 d开始下降
而头针各时段组缺血侧海马CA 3区MMP-9的阳性细胞表达较同时段模型组减少
其中3、5、10 d的差异有统计学意义(P<0.05
P<0.01)。结论:头针能够有效改善脑缺血状态下海马CA 3区微血管内皮MMP-9的表达;随治疗次数的增加
针刺的疗效出现累加蓄积效应。
Objective To observe the effect of scalp-acupuncture on the expression of matrix metalloproteinase 9(MMP-9) in the endothelium of the microvessels in the hippocampal CA 3 area in local cerebral ischemia(CI) rats
so as to study its underlying mechanism in improving CI. Methods Eighty male Wistar rats were randomized into normal control(n=8)
sham-operation(n=8)
CI model(n=32)
and scalp-acupuncture(n=32) groups.The later two groups were further equally and respectively divided into 1

5 and 10 d subgroups.CI model was established by occlusion of the middle cerebral artery(MCAO)."Baihui"(GV 20) and "Qubin"(GB 7) were punctured with filiform needles and stimulated by twirling the needle at a frequency of about 200 revolutions/min for 1 min.The needles were retained for 30 min and re-manipulated once again every 10 min.The treatment was given once daily

5 and 10 days respectively in different subgroups.MMP-9 immunoactivity of hippocampal CA 3 area was assayed by immunohistochemistry(SABC). Results Following CI
the CI rats' neural deficit scores in the model and scalp-acupuncture(treatment) groups had no apparent difference(P>0.05).After acupuncture

the total area and IOD values of MMP-9 immunoreaction positive products in the corresponding 3 subgroups of scalp-acupuncture group were decreased significantly(P<0.05
P<0.01).Conclusion Scalp-acupuncture can effectively down-regulate MMP-9 expression in the hippocampal CA 3 area in focal cerebral ischemia rats
which may contribute to its effect in improving the neural deficit.
