目的:观察电针对急性手术创伤大鼠术后苏醒质量和下丘脑、海马促肾上腺皮质激素释放因子(CRF)表达的影响
探讨大鼠下丘脑与海马CRF表达的相关性在针刺调节急性手术创伤所导致的下丘脑-垂体-肾上腺(HPA)轴功能异常中的作用。方法:雄性SD大鼠20只
随机分为正常组、模型组、假电针组和电针组。肝叶切除术建立急性创伤模型。电针组电针右侧"足三里"和"三阴交"
假电针组仅针刺"足三里"和"三阴交"
不接电针
于造模前24h及造模后各治疗1次。观察各组大鼠麻醉苏醒时间及翻正反射情况。应用Western blot方法检测各组动物下丘脑和海马CRF的表达。结果:电针组大鼠麻醉苏醒时间显著缩短
翻正反射迅速恢复
和模型组、假电针组相比差异有统计学意义(P<0.001
P<0.01)。苏醒时间与翻正反射有相关性(r=0.646
P<0.05)。模型组大鼠下丘脑CRF表达升高
和正常组相比差异有统计学意义(P<0.05)
电针组大鼠下丘脑CRF表达降低
和模型组、假电针组相比差异有统计学意义(P<0.05)。模型组大鼠海马CRF表达降低
和正常组相比差异有统计学意义(P<0.05)
电针组大鼠海马CRF表达升高
和假电针组、模型组相比差异有统计学意义(P<0.05)。术后大鼠下丘脑CRF表达与海马CRF表达呈负相关(P<0.001)
与翻正反射呈正相关(P<0.01)
即Y下丘脑=0.452-0.546X海马+0.200X翻正反射。海马CRF表达与刺激方式呈负相关(P<0.001)
与电针干预呈正相关(P<0.01)
该表达式为Y海马=0.773-0.129X刺激方式+0.120X电针。结论:电针可缩短手术大鼠术后苏醒时间。下丘脑和海马CRF的表达与大鼠术后苏醒时间及干预方法均呈一定相关性
提示下丘脑和海马CRF在调节机体应激反应中可能起关键作用。电针可能通过激活海马CRF神经元
抑制下丘脑CRF神经元
进而调整急性手术创伤所致的大鼠HPA轴功能异常
减轻术后应激。
Objective To observe changes of anesthesia analepsia time and hypothalamic and hippocampal corticotropin-releasing factor(CRF)protein expression after electroacupuncture(EA)intervention in acute traumatic rats
in order to explore its mechanism underlying relief of post-operative stress.Methods Twenty male Sprague-Dawley rats were randomly divided into normal control group
model group
sham EA group and EA group(5rats/group).Acute traumatic model was established by left lobectomy.For rats of the sham EA group
two filiform needles were inserted into the right"Zusanli"(ST 36)and"Sanyinjiao"(SP 6)respectively
without needle-twirling and without electrical stimulation.For rats of the EA group
the right ST36and SP 6were electrically stimulated at 2Hz/100Hz and 2mA for 30minutes after needle insertion.Anesthesia analepsia time was recorded and righting reflex was tested for the acute traumatized rats.The expression of CRF protein in the hypothalamus and hippocampus was analyzed with Western blot.Results The anesthesia analepsia time and righting reflex recovery time were significantly shorter in the EA group than in the model group and sham EA group(P<0.01).There was a close correlation between the anesthesia analepsia time and righting reflex recovery time(P<0.05).In comparison with the normal group
the expression level of hippocampal CRF protein was significantly decreased whereas that of hypothalamic CRF protein evidently increased in the model group(P<0.05).Compared with the model group
hippocampal CRF protein expression was considerably upregulated and hypothalamic CRF protein remarkably down-regulated in the EA group(P<0.05).A markedly negative correlation was found between hypothalamic CRF and hippocampal CRF protein expression
and between hippocampal CRF expression and the stimulating mode(P<0.001)
and a positive correlation was found between hypothalamic CRF expression and righting reflex
and between hippocampal CRF expression and EA intervention(P<0.01).Conclusion EA intervention can shorten anesthesia analepsia time and righting reflex recovery time in liver-traumatized rats
which is closely associated with its effects in upregulating hippocampal CRF protein expression and down-regulating hypothalamic CRF expression.It suggests that CRF mediated hippocampus-hypothalamus-pituitary-axis is involved in EA induced reduction of post-surgical stress.
