目的:从脑-肠轴角度探讨针刺在治疗胃黏膜损伤及改善睡眠方面的作用机制。方法:将雄性Wistar大鼠随机分为空白组、模型组、和胃组、安神组和联合组
每组8只。采用无水乙醇灌胃法造成大鼠胃黏膜损伤模型
和胃组选"中脘""足三里"穴
安神组选"照海""申脉"穴
联合组选"中脘""足三里""照海""申脉"穴
针刺治疗5d。末次针刺后
用戊巴比妥钠溶液腹腔注射
记录注射后大鼠的睡眠诱导时间。肉眼观察大鼠的胃黏膜形态
Guth法计算胃黏膜损伤指数
酶联免疫法测定血清和海马中肿瘤坏死因子-α(TNF-α)及白介素-25(IL-25)的含量。结果:与空白组相比
模型组睡眠诱导时间和胃黏膜损伤指数显著增高(P<0.01)
血清与海马中TNF-α及IL-25含量显著升高(P<0.01)。与模型组相比
3个治疗组睡眠诱导时间和胃黏膜损伤指数显著降低(P<0.01)
血清与海马中TNF-α及IL-25含量显著降低(P<0.01
P<0.05)。和胃组与联合组较安神组胃黏膜损伤指数、血清IL-25含量、海马TNF-α和IL-25含量降低明显(P<0.01
P<0.05)
安神组与联合组在缩短睡眠诱导时间方面的效果均优于和胃组(P<0.01
P<0.05)
联合组血清TNF-α含量显著低于和胃组与安神组(P<0.05)。结论:"中脘""足三里""照海""申脉"联合针刺能够更好地促进大鼠胃溃疡的愈合
同时缩短睡眠诱导时间
这与其降低血清及海马中TNF-α及IL-25含量有关。说明胃肠疾病与睡眠之间相互影响
为临床治疗"胃不和则卧不安"提供了初步的实验依据。
Objective To observe the effect of acupuncture intervention on gastric ulcer(GU)and sleeping quality from the viewpoint of brain-gut axis which plays an important role in the regulation of many vital functions in the body.Methods Forty male Wistar rats were randomized into normal control
GU model
acupuncture of"Zhongwan"(CV 12)-"Zusanli"(ST 36
gastric function regulating acupoints)
acupuncture of"Shenmai"(BL 62)-"Zhaohai"(KI 6
sleep-promotion acupoints)
and acupuncture of CV 12-ST 36+BL 62-KI 6(combined treatment)groups
with 8rats in each group.GU model was established by intragastric perfusion of dehydrated alcohol(1mL/rat)
and sleep model established by intraperitoneal injection of pentobarbital sodium(40mg/kg)after the last treatment.The abovementioned acupoints were punctured with filiform needles and stimulated by manipulating the needle for about 30 s
once every 5min during 20 min of needle retention.The treatment was conducted once daily for five days.The contents of tumor necrosis factor-alpha(TNF-α)and interleukin-25(IL-25)in the serum and hippocampal tissues were detected by ELISA.Results Compared with the normal control group
the gastric ulcer index score
barbiturate-induced sleeping time
and TNF-αand IL-25 contents in both serum and hippocampus were significantly increased in the model group(P<0.01).Following acupuncture treatment

P<0.05).The effects of the CV 12-ST 36 and combined treatment groups were remarkably superior to those of the BL 62-KI 6group in down-regulating ulcer index score
serum IL-25
and hippocampal TNF-αand IL-25contents(P<0.01
P<0.05).In addition
the effects of the BL 62-KI 6and combined treatment groups was considerably better than that of the CV 12-ST 36 group in shortening barbiturate-induced sleeping time(P<0.01
P<0.05).The effect of the combined treatment group was markedly better than that of the CV 12-ST 36 and BL 62-KI 6groups in lowering serum TNF-αcontent(P<0.05).Conclusion Acupuncture stimulation of CV 12
ST 36
KI 6and BL 62 can relieve the gastric mucosal lesion
and shorten barbiturate-induced sleeping time in gastric ulcer rats
which may be related to its effects in reducing TNF-αand IL-25 contents in the serum and hippocampus tissues
suggesting a correlation between the gastrointestinal disorder and sleeping.
